Aortic Dissection: Branched, Fenestrated, and Parallel Aortic Stent Grafts in the Ascending Aorta and Arch.

Open surgical repair of dissections and post-dissection degenerative aneurysms involving the ascending thoracic aorta and aortic arch, whether in the acute or post-repair state, is associated with high rates of morbidity and mortality. Recent advancements in minimally invasive endovascular techniques have expanded the role of thoracic endovascular aortic repair (TEVAR) for dissections and dissection-related arch pathologies. Image-guided endovascular techniques, such as in situ fenestrated grafts, chimney and/or periscope grafts, along with newly developed commercially available branched aorti

[One- and two-dimensional echography for aneurysms and dissections of the aorta (author's transl)].

Aneurysms and dissections of the aorta--depending on their location--are the cause for a variety of cardiovascular symptoms. In most cases they are the result of a generalised disease of the vessel wall. Consequently, echographic examinations should include the entire aorta from the root to the bifurcation, systematically utilising all accessibilities. This procedure often permits a complete evaluation of the aorta and its major branches thus enabling more selective use of catheterization. Morphologic and topographic information is gained mainly from two-dimensional images, whereas functional

Expanded Petticoat technique to promote the reduction of contrasted false lumen volume in patients with chronic type B aortic dissection.

OBJECTIVE: This study examined the outcomes of our novel concept of expanded provisional extension to induce complete attachment strategy (Petticoat) for safety, durability, and remodeling of chronic type B dissections. METHODS: Twenty patients with chronic type B aortic dissection with aneurysmal degeneration qualified for an expanded Petticoat strategy (stent graft in the thoracic, plus additional distal bare stent into the abdominal and infrarenal aorta, followed by parallel stent grafts into common iliac arteries). Computed tomography was performed preoperatively and at 1, 6, and 12 month

Endovascular Repair of Thoracic Aortic Aneurysm Using Transcaval Aortic Access Technique.

Repair of thoracic aortic aneurysms has evolved in recent history. Open surgical intervention has largely been replaced with endovascular intervention. However, there are anatomic limitations to thoracic endovascular aneurysm repair. In patients with peripheral vascular disease or vessel tortuosity, it becomes dangerous to attempt percutaneous access or graft deployment for fear of severe complications, such as dissection or occlusion. We describe a technique whereby access to the aorta is obtained through the inferior vena cava, allowing the thoracic aneurysm to be repaired endovascularly.

Management of Type B Aortic Dissections: Treatment of Acute Dissections and Acute Complications from Chronic Dissections.

Aortic dissection is one of the most catastrophic vascular challenges faced by interventionalists, often misdiagnosed and frequently associated with devastating consequences. Stanford type B aortic dissections affects the descending thoracic aorta without any involvement of the ascending aorta. Dissections associated with refractory pain, rapid aneurysm formation, malperfusion syndromes, rupture, or impending rupture are categorized as complicated and distinct from initially uncomplicated type B dissections without the aforementioned complications. Traditionally, uncomplicated type B dissectio

A modified adventitial inversion with graft insertion technique in acute Type A aortic dissection.

Acute type A aortic dissection may originate from a primary intimal tear located in the ascending aorta and often extends retrogradely into the aortic root. How to prevent bleeding in the aortic root and eliminate false lumen is very important in aortic dissection. We have developed a modified anastomotic technique that involves inverting adventitial and graft into aorta and reinforcing with a felt strip on the external border of the aortic wall. Since 2020, 45 consecutive patients with type A aortic coarctation have undergone this aortic root reconstruction procedure, to date, none have been

Comparing neurological complications between endovascular and open surgical repair of the descending thoracic aorta.

BACKGROUND: Diseases of the descending thoracic aorta (DTA) can require endovascular, open, or a hybrid repair. Endovascular repair involves the minimally invasive placement of a stent graft, whereas open repair requires a thoracotomy to replace tissue with a graft. This study aims to compare the neurological morbidity associated with endovascular vs open repair of the DTA, and specifically for unruptured DTA aneurysm and DTA dissection. METHODS: We utilized TriNetX, a deidentified patient database to query adult patients with endovascular and open repair of DTA disease, unruptured aneurysm,

The functional limits of the aneurysmal aortic root. A unique pressure testing apparatus.

BACKGROUND: The aortic root has unique embryological development and is a highly sophisticated and complex structure. In studies that report on the biomechanical characteristics of the thoracic aorta, distinction between the aortic root and ascending aorta regions is nonexistent. Our objective is to determine the maximal pressures at which dissection occurs or tissue failure occurs in the aortic root compared to that of the ascending aorta in the presence of aortic aneurysms. This may help guide preoperative monitoring, diagnosis and the decision for operative intervention for aortic root aneu

[Successful Echo-guided Deployment of Stent Graft for Intraoperative Retrograde Aortic Dissection and Rupture;Report of a Case].

We report a case of chronic dissecting thoracoabdominal aneurysm with intraoperative retrograde aortic dissection and rupture at proximal descending aorta, which was successfully treated by echoguided stent-graft insertion. An 82-year old male underwent thoracoabdominal aortic replacement for dilatation of infra-diaphragmatic aorta. Under F-F bypass, his thoracoabdominal aorta was replaced by a Dacron graft with 4-branches. After he weaned from F-F bypass, we found massive bleeding from proximal descending aorta. Trans-esophageal echocardiography (TEE) showed aortic dissection from the clamp s

Excision of the aortic wall in the surgical treatment of acute type-A aortic dissection.

Between March 1986 and September 1988, 38 patients underwent extended aortic resection (aortic valve, ascending aorta, and arch) for acute type-A aortic dissection with aortic valve insufficiency; deep hypothermia and circulatory arrest were used. All patients were operated on within 17 hours of the onset of symptoms. In the first 24 patients, operation was performed by the "inclusion technique." In the last 14 patients, the "excision technique" was used: the ascending aorta and arch was excised, and the aorta was transected at the beginning of the descending thoracic tract. Excision and trans

Inflammatory response and renal function following endovascular repair of the descending thoracic aorta.

PURPOSE: To evaluate inflammatory response and renal function after thoracic endovascular aortic repair (TEVAR) of lesions in the descending thoracic aorta. METHODS: Thirty-two consecutive patients treated with TEVAR from January 2010 to August 2013 were enrolled in this prospective study. Two were excluded owing to dissecting thoracic aortic aneurysm (TAA) extending into the renal arteries with renal failure in one and a saccular TAA in which a multilayer flow-modulating stent was implanted in the other. This left 30 patients (28 men; mean age 68.8±5.9 years) with 28 TAAs, an aortic dissecti

A review of the surgical management of right-sided aortic arch aneurysms.

Aneurysms and dissections of the right-sided aortic arch are rare and published data are limited to a few case reports and small series. The optimal treatment strategy of this entity and the challenges associated with their management are not yet fully investigated and conclusive. We performed a systematic review of the literature to identify all patients who underwent surgical or endovascular intervention for right aortic arch aneurysms or dissections. The search was limited to the articles published only in English. We focused on presentation and critically assessed different management stra

Natural history of descending and thoracoabdominal aortic aneurysms.

Data on the natural history of descending and thoracoabdominal aneurysms are limited to a few studies. They demonstrated that, independent from the different mechanisms of injury and degeneration affecting the structural integrity of the aortic wall, the biologic fate of all aneurysms is progressive enlargement and rupture. Laminated thrombus and calcification do not prevent this process. The natural history is markedly influenced by size, location, symptoms, and etiology of thoracic aneurysms. They may develop symptoms related to mechanical compression of adjacent structures, but more frequen

Balloon Endoclamping for Postdissection Infrarenal Aorto-Iliac Aneurysm Repair.

BACKGROUND: Isolated post dissection infrarenal and iliac aneurysm is a rare condition that often requires surgical treatment. Surgical repair should involve the replacement of the aneurysmal segments and a wide fenestration in the residual proximal untreated abdominal aorta. However, in these patients proximal aortic clamping may be challenging. Indeed, infrarenal clamping may hamper an appropriate fenestration in the proximal dissecting lamella, and suprarenal or supraceliac clamping can be dangerous and highly demanding, especially in acute and subacute patients. Here we report our initial

Acute cardiac dysfunction and rapid descending aortic expansion following surgery for acute Stanford type A aortic dissection: a case report.

BACKGROUND: Severe true lumen stenosis of the aorta can increase left ventricular afterload and impair cardiac function. We report a rare case of significant true lumen stenosis and cardiac dysfunction after undergoing total arch replacement for acute type A aortic dissection. CASE SUMMARY: A 39-year-old man with acute Stanford type A aortic dissection and intimal tear in the ascending aorta underwent the Bentall procedure and total arch replacement with a frozen elephant trunk (FET). He was readmitted 1 month after discharge with progressive cardiac dysfunction. Myocardial scintigraphy exclu

Changing paradigm in endovascular treatment of descending thoracic aortic dissections.

Descending thoracic dissections originating distal to the origin of the left subclavian artery carry a significant mortality if left untreated. Past thinking advocated avoiding surgical treatment of acute Stanford type B or DeBakey type III dissections, reserving therapy for chronic dissections over 14 days to a month after presentation. The current evolution of endovascular devices for the treatment of thoracic aneurysms has proven helpful in treating this pathology in a less invasive manner when compared to open surgical repair. The paradigm for treatment has evolved beyond the nature of the

Volumetric analysis of type B aortic dissections treated with thoracic endovascular aortic repair.

BACKGROUND: Type B aortic dissections are being successfully treated by thoracic endovascular aortic repair (TEVAR). Postoperative false lumen patency has been associated with aneurysmal dilatation and rupture of the thoracic aorta, necessitating further intervention. This is the first volumetric analysis of type B aortic dissections comparing patients with and without false lumen thrombosis (FLT) after TEVAR. We hypothesized that a greater increase in postoperative true lumen volume will lead to FLT, and without this change, false lumen patency will result. METHODS: Preoperative and postoper

Two Cases of Endovascular Repair with the Stent Graft for Retrograde Type A Acute Aortic Dissection with Complications.

Thoracic endovascular aortic repair (TEVAR) is used to treat retrograde type A acute aortic dissection (RTAAD). In case 1, a 52-year-old man, who was conservatively managed, reported worsening back pain. Emergency TEVAR was performed 7 days after onset. After deploying two GORE Conformable TAG (CTAG) in the descending aorta, his symptoms disappeared. In case 2, a 52-year-old man with progressive worsening resistant hypertension, renal dysfunction, and respiratory failure despite maximal medical therapy underwent TEVAR 8 days after onset. A CTAG was deployed from the left subclavian artery unde

Early and long-term results of replacement of the descending aorta.

OBJECTIVES: The aim of this analysis was to evaluate our results of open surgery on the descending thoracic aorta as benchmark to define indications for endovascular treatment. METHODS: Between January 1981 and December 2000, 115 patients underwent replacement of the descending or thoraco-abdominal aorta. Follow-up to 20 years was complete in 98%. RESULTS: Early mortality was 19% and paraplegia rate was 7%. Surgery before 1990 and coronary artery disease were independent predictors for early mortality. Thoraco-abdominal repair and normothermia were independent predictors for paraplegia. Ten
