Acute diverticulitis (AD) is an increasing issue for health systems worldwide. As accuracy of clinical symptoms and laboratory examinations is poor, a pivotal role in preoperative diagnosis and severity assessment is played by CT scan. Several new classifications trying to adapt the intraoperative Hinchey's classification to preoperative CT findings have been proposed, but none really entered clinical practice. A 33-year-old pregnant woman presented with peritonitis at the right iliac fossa. Preoperative ultrasonography identified an 8-cm tubular structure at the tender point that was mistaken as appendicitis. Colovesical fistulas are the most common type of vesicointestinal fistula and the most prevalent of these are vesicosigmoid fistulas. In our environment, diverticulitis is the most common cause, accounting for approximately one half of all cases. The literature describes different methods for the diagnosis of colovesical fistula, with computed tomography and cystography being the most useful. We describe two cases of vesicosigmoid fistula secondary to diverticulitis diagnosed at our center using helical computed tomography and review the clinical and radiological findings for this entity.
