Carcinoid tumor arising in a Meckel's diverticulum is a rare lesion with only 52 cases described in the literature. Most of these tumors are found incidentally at autopsy or during an operation performed for an unrelated reason. The lesion can, however, in about one-third of cases, be the cause of symptoms necessitating surgical intervention; the diagnosis is nearly never apparent till the specimen has been removed. Meckel's diverticulum is a vestigial remnant of the omphalomesenteric duct, which is rarely affected by neoplasia. The authors report the first case of malignant melanoma in Meckel's diverticulum in a middle-aged man without skin or ocular lesions. Five other Meckel's diverticuli were evaluated histologically and by immunoperoxidase technics for neural crest elements, which could have given rise to this lesion.

This case report of a patient with a large Meckel's diverticulum with associated stagnation and bacterial proliferation demonstrates the resultant metabolic and nutritional alterations that have classically been described with the blind loop syndrome. Meckel's diverticulum can be detected with a high degree of accuracy by radionuclide scintigraphy using technetium-99m pertechnetate. The technique is without risk and should precede roentgenographic studies when the diagnosis is suspected. The method is described and the causes for false positive and false negative examinations are discussed. False negatives are rare and false positive are usually secondary to other surgical entities. Overall accuracy is 85 to 90%.

Three cases of Meckel's diverticulum are described. All were investigated by the conventional barium meal and follow-through examination. The methods of radiological investigation are considered and, contrary to the opinion of many authors, the value of the conventional barium examination is stressed.

A urethral diverticulum was diagnosed on pelvic ultrasonography. Its ultrasonic features and differential diagnosis are discussed. An awareness of the ultrasonic appearance should result in more frequent recognition of this lesion.

Examination for Meckel's diverticulum is generally performed in each case of laparotomy where the incision allows observation of the small bowel without danger of spreading the infection. In 65 cases, pathologic alterations were found in 14 (= 21,5%). Postoperative bowel obstruction due to resection of Meckel's diverticulum has been found twice (3%), one patient died. It is suggested that Meckel's diverticulum be removed whenever possible.
