Effects of Vibration Therapy on the Physical Function of Critically Ill Adults Trial: A Randomized Controlled Trial.

OBJECTIVES: Vibration therapy uses vibration to rehabilitate physical functions. Recently, it has been demonstrated to be safe for critically ill patients. However, its effects on physical functions are unclear. DESIGN: Randomized controlled trial. SETTING: A single-center, ICU. PATIENTS: Patients were randomly assigned to either vibration therapy coupled with protocolized mobilization or protocolized mobilization alone. We included patients who could sit at the edge of the bed or in a wheelchair during their ICU stay. The exclusion criteria were based on the early mobilization inhibition c

Safety and feasibility of early mobilization in patients submitted to cardiac surgery using subxiphoid drain.

INTRODUCTION: Patients submitted to heart surgery are restricted to the bed of the Intensive Care Units (ICUs), due to this period of immobility the individual is likely to present clinical and functional alterations. These complications can be avoided by early mobilization; however, in some hospitals, this is not feasible due to the use of subxiphoid drain in the immediate postoperative period. OBJECTIVE: To verify the safety and feasibility of mobilizing patients after cardiac surgery using subxiphoid drain. METHODS: This was a prospective cohort study. On the first day the patient was pos

A systematic review and meta-analysis of the effects of early mobilization therapy in patients after cardiac surgery.

BACKGROUND: Prolonged hospitalization and immobility of critical care patients elevate the risk of long-term physical and cognitive impairments. However, the therapeutic effects of early mobilization have been difficult to interpret due to variations in study populations, interventions, and outcome measures. We conducted a meta-analysis to assess the effects of early mobilization therapy on cardiac surgery patients in the intensive care unit (ICU). METHODS: PubMed, Excerpta Medica database (EMBASE), Cumulative Index of Nursing and Allied Health Literature (CINAHL), Physiotherapy Evidence Data

In adults, early mobilization may be beneficial for distal radius fractures treated with open reduction and internal fixation: a systematic review and meta-analysis.

OBJECTIVES: It remains debatable if early mobilization (EM) yields a better clinical outcome than the late mobilization (LM) in adults with an acute and displaced distal radial fracture (DRF) of open reduction internal fixation (ORIF). Therefore, we aimed to perform a systematic review and meta-analysis of randomized controlled trials (RCTs), comparing clinical results with the safety of EM with LM following ORIF. METHODS: Databases such as Medline, Cochrane Central Register, and Embase were searched from Jan 1, 2000, to July 31, 2021, and RCTs comparing EM with LM for DRF with ORIF were incl

Tibial eminence fractures in children: earlier posttreatment mobilization results in improved outcomes.

BACKGROUND: Arthrofibrosis and decreased range of motion (ROM) are well-described sequelae of tibial eminence fractures. We sought to evaluate the effects of timing of ROM rehabilitation and postsurgical immobilization on clinical outcomes in children with fractures of the tibial eminence. METHODS: We retrospectively reviewed the records of all children diagnosed with closed tibial eminence fractures between 2000 and 2010. Patients were treated by experienced surgeons with uniform requirements for return to full activity. RESULTS: Fourteen females and 26 males (40 knees) of mean age 12 years

Early mobilization after pacemaker implantation.

BACKGROUND: There are no international guidelines indicating how long a patient should stay strictly in bed after pacemaker implantation. In the present study, we tested a new protocol concerning the mobilization of patients 3 h after receiving a single or a dual-chamber pacemaker. METHODS: Consecutive patients who underwent single or dual-chamber pacemaker implantation were randomized to a 3 or 24 h immobilization protocol. Only bipolar passive fixation leads were computed. After the implant, an elastic bandage was put on the homolateral shoulder of all patients for 24 h. A complete clinical

Surgical stress response: does endoscopic surgery confer an advantage?

"Open" surgical procedures are followed by profound changes in endocrine metabolic function and various host defense mechanisms, impaired pulmonary function, and hypoxemia. These physiologic changes are supposed to be involved in the pathogenesis of postoperative morbidity. Endoscopic surgery, mostly studied during laparoscopy, when compared with similar open operations, has no important effects on classic endocrine metabolic responses but may slightly reduce inflammatory responses and various immune functions, although the data are not consistent. In contrast, most data show improvement of po

Weight bearing in Perthes' disease.

Results of two different types of treatment for 82 varus-derotation femur osteotomies to cure Perthes' disease are presented. Patients were treated between 1974 and 1984. Postoperatively, in 30 cases weight bearing was allowed, while in 52 cases, non-weight bearing braces were applied. Comparing the results of the two groups and considering the risk factors, the authors found that non-weight bearing does not influence end results after proximal femur osteotomy.

A custom-made prosthesis for the treatment of supracondylar femoral fractures after total knee arthroplasty: report of four cases.

Four supracondylar femoral fractures following total knee arthroplasty, two because of intraoperative notching of the anterior femoral cortex and two because of osteoporosis, were revised using a custom-made prosthesis with femoral condyle section. At 1-6 year follow-up (median 3 years) after the revision, all patients had an excellent clinical result, although 2 had a prolonged rehabilitation period because of severe osteoporosis. At roentgenographic examination, no signs of loosening of the prostheses at the cement-bone interface were present.

Early mobilizing treatment in lateral ankle sprains. Course and risk factors for chronic painful or function-limiting ankle.

In a prospective study, 150 patients with lesions of lateral ankle ligaments were treated with early mobilization without any fixation of the ankle. After 8 days, 67% were free from pain on ordinary walking and 81% had resumed work. After 1 month 90% were free from pain and 97% had resumed work. Sport was resumed by 70% of athletes after 1 month and 90% after 3 months. At the 1-year follow-up (n = 137), 18% were not fully recovered and experienced pain (14%) or functional instability (7%) or had not resumed sport at their normal level (7%). However, only 8% found the condition inconvenient. At

Enhanced recovery after laparoscopic or robotic-assisted ureteral reconstruction: identification of modifiable factors in a prospective study.

PURPOSE: To evaluate the bowel function recovery following ureteral reconstruction and identify the associated factors. METHODS: We conducted a prospective study enrolling 98 patients undergoing transperitoneal laparoscopic or robotic-assisted ureteral reconstruction. The time to resumption of gas evacuation and defecation were used to evaluate the bowel function recovery. Student's t-test, Chi-squared test, Pearson's correlation, and linear regression were performed to identify independent factors. Nomograms were developed to predict early resumption of gas evacuation and defecation. RESULT

[Prevention of venous thromboembolism in orthopaedics and traumatology: Austrian interdisciplinary consensus statement 2026].

Venous thromboembolism (VTE), a disease entity comprising deep vein thrombosis (DVT) and/or pulmonary embolism (PE), represents one of the most frequent and potentially life-threatening complications in orthopaedics and traumatology [1]. The previous Austrian recommendation on VTE prophylaxis in musculoskeletal procedures, dating from 2014 [2], no longer adequately reflects current evidence-based pharmacological and nonpharmacological strategies. Current international interdisciplinary guidelines on VTE prophylaxis, such as those issued by the European Society of Anaesthesiology and Intensive

Effects of Integration Sedation Interruption With Early Mobility on Patient Outcomes and Prevention of Intensive Care Unit-Acquired Weakness in Critically Ill Ventilated Patients.

BACKGROUND: Early mobilization and sedation reduction are critical strategies for improving patient outcomes. AIM: This study assessed the effects of integrating daily sedation interruptions with an early progressive mobility strategy on outcomes in critically ill, ventilated patients. STUDY DESIGN: A quasi-experimental research with a control group was conducted on a sample of 60 mechanically ventilated adult patients who met the inclusion criteria. We randomly divided the patients into 30 groups, with each representing the control and study groups. Data were gathered using 4 tools. RESULT

First out-of-bed mobilisation in adults with severe acquired brain injury in Scandinavian neurointensive care units: A survey of current clinical practice (FOOBScan).

BACKGROUND: The harm-benefit balance for early out-of-bed mobilisation of patients with severe acquired brain injury (ABI) in neurointensive care units (neuro-ICUs) is unclear, and there are no clinical guidelines. This study aimed to survey the current clinical practice and perceptions among clinicians involved in first out-of-bed mobilisation in Scandinavian neuro-ICUs. METHODS: This was a cross-sectional, anonymous, web-based survey; the reporting follows the recommended CROSS checklist. Clinicians involved in the first out-of-bed mobilisation in all 14 Scandinavian neuro-ICUs were eligibl

Effects, Feasibility, and Safety of an Early Mobilization Protocol With Immersive Virtual Reality for Dyspnea in Patients With Acutely Decompensated Heart Failure: The MOVE Randomized Clinical Trial.

BACKGROUND: Early mobilization seems to benefit patients with acutely decompensated heart failure (ADHF), but its initiation is challenging due to severe dyspnea, clinical instability, and low adherence to treatment. Understanding whether early mobilization has good acceptance, adherence, and safety is key for rehabilitation in this population. Virtual reality (VR) offers a less stressful environment and may reduce dyspnea, yet its effects on ADHF remain unknown. In addition, the feasibility and safety of combining VR with an early mobilization program in the intensive care unit (ICU) setting

Mobility outcomes of patients with an Impella 5.5® device in the acute care setting.

BACKGROUND: The Impella 5.5® device is a temporary ventricular assist device that improves the cardiac output in patients with cardiogenic shock. These patients are critically ill and are often placed on bed rest, which can result in a functional decline. Early mobility programs have helped mitigate some of the negative consequences of immobility while also reducing the incidence of hospital-related complications. OBJECTIVE: To describe the characteristics of patients with the Impella 5.5® device and to compare the initial mobility status of patients discharged home versus a post-acute facili

Patient as active partner - clue to successful early mobilization in intensive care.

BACKGROUND: The evidence for the benefits of early mobilization in intensive care is growing. Early mobilization differs from most other interventions in intensive care since the patient's participation is requested. What kind of challenges this entails for the intensive care clinicians, and what is crucial in successful early mobilization from their perspective, is sparsely explored and was therefore the purpose of this study. METHODS: Semi-structured interviews were held with 17 intensive care clinicians, seven nurses, five assistant nurses and five physiotherapists. The interviews were ana
