A 5-month-old male infant with no history of serious illness died suddenly and unexpectedly. He was found dead under a heavy 'futon', a coverlet, which covered the whole of his body. As it was suspected he may have been smothered, a medicolegal autopsy was performed, and it was discovered that the infant had Ebstein's anomaly with double mitral valve. The cause of death appeared to be due to heart failure caused by this congenital heart disease.

The echocardiographic and cineangiographic features of an unusual type of tricuspid stenosis in Ebstein's anomaly are described. The displaced septal and mural leaflets of the tricuspid valve are inserted to an anomalous apical muscle shelf and cause a restrictive communication between the atrialized portion and the trabecular zone of the right ventricle. The antero-superior leaflet is not involved.

Renewed interest in the natural history of Ebstein's anomaly has been created by recent surgical innovations in its management. Tricuspid valve replacement and various types of plastic reconstruction are the two methods of treating Ebstein's anomaly. Replacement of the tricuspid valve has been more successful in older children and adults than in infants, but overall, less successful than procedures involving mitral or aortic valve prosthesis. At the Mayo Clinic, 42 patients with Ebstein's anomaly, aged 11 months to 62 years, underwent surgical repair. Plastic repair was accomplished in 34 of t

The rare and curious congenital malformation of the tricuspid valve known as Ebstein's disease or Ebstein's anomaly has been a subject of considerable interest to the cardiologist and the morphologist since its first description by Wilhelm Ebstein in 1866. With the evolution of various surgical techniques for the correction of this anomaly, a thorough understanding of the pathologic anatomy of the malformation assumes greater practical importance. It is now clear that although all hearts with Ebstein's anomaly have in common displaced basal insertions of the septal and posterior leaflets of th

The authors report 12 cases of Ebstein's disease, and have studied the echocardiographic findings in these cases. The results have been compared with those of a control series of 12 patients with an atrial septal defect and a large shunt effect. There are only two conditions which are important in the diagnosis of Ebstein's disease: the finding of the tricuspid valve beyond the midclavicular line, and a delay in the closure of the tricuspid valve greater than or equal to 0.06 seconds when compared with the mitral valve. Using these two criteria, the diagnosis could be made by echocardiography

The results of a follow-up of 35 patients (including 18 children and adolescents) for periods of 6 months to 11 years after radical correction of Ebstein's anomaly are analysed. The results were good in 25 patients, satisfactory--in 6 and poor--in one patient; 3 patients died. The result was poor in one of the patients because the thin walled arterialized part of the right ventricle was not plicated. Death in the remote period was caused by disorders of the cardiac rhythm (atrioventricular block--Morgagni-Adams-Stokes syndrome), "prosthetic" endocarditis, and influenzal pneumonia. Complication

Nine patients with Ebstein's anomaly of the tricuspid valve were studied by two-dimensional echocardiography, using the standard long and short axis views as well as the apex four chamber view. With this latter view, the displacement of the tricuspid valve into the right ventricle was clearly seen in all nine cases of Ebstein's anomaly and was not noted in a control population. The severity of the tricuspid displacement was assessed by comparing the position of the mitral and tricuspid valves relative to the cardiac apex. The apex four chamber view allowed visualization of the atrioventricular

The typical textbook description of Ebstein's anomaly of the heart usually singles out and emphasizes the downward displacement of septal and posterior leaflets of the tricuspid valve. An anatomic reappraisal of this uncommon anomaly suggests that other structural abnormalities of import should be equally stressed. Among the 15 well preserved autopsy specimens in this series, enlargement of the right atrioventricular (A-V) junction and malalignment of the giant and sometimes muscularized anterior leaflet of the tricuspid valve were consistently found. In addition, massive aneurysmal dilation o

Sixteen patients aged between one day to 18 years with Ebstein's malformation of the tricuspid valve were studied with ultrasound. The findings were compared with a group of 74 patients without Ebstein's malformation. Two features were considered specific for Ebstein's malformation: 1) ability to record the anterior tricuspid leaflet (ATL) farther to the left of the left sternal border than in the control group; and 2) abnormally prolonged interval between the "C" points of the tricuspid and the mitral valve echoes (McTcI). All other parameters measured were nonspecific for Ebstein's anomaly o

Electrophysiologic characteristics of five patients with Ebstein's anomaly of the tricuspid valve were defined with studies using luminal intracardiac electrode catheters. The diagnosis was made in each case from clinical data and confirmed at cardiac catheterization by the presence of an atrialized right ventricular chamber with atrial mechanical activity and ventricular electrical activity. In three cases intra-right atrial conduction was prolonged (P-A intervals of 50, 50, and 65 msec), a finding which reflected the presence of a characteristically large right atrium. The bundle of His elec
