Scheuermann's thoracic kyphosis in the adolescent patient.

Scheuermann's thoracic kyphosis is a condition characterized by increased posterior rounding of the thoracic spine in association with structural deformity of the vertebral elements. It is a structural deformity of the spine that is classically characterized by anterior wedging of 5° or more of 3 adjacent thoracic vertebral bodies with kyphosis measuring greater than 45° between T5 and T12. The cause of the deformity remains unknown but it is believed to be multifactorial, and it likely has a genetic component as well. Most adolescent patients seek orthopaedic evaluation for increased rounding

Triaging dysphagia: nurse screening for dysphagia in an acute hospital.

AIMS AND OBJECTIVES: To (1) develop a dysphagia screening tool to triage all patients at risk of aspiration/dysphagia on admission to acute hospital wards, (2) evaluate tool reliability, (3) evaluate nursing compliance and (4) develop a robust dysphagia training programme. BACKGROUND: Failure to diagnose dysphagia has significant medical and economic costs. Dysphagia screening reduces pneumonia threefold. Most nurse-screening tools have focused on stroke. However, many other conditions are associated with dysphagia. A multidisciplinary team developed a nurse-administered, evidence-based swall

Bone loss. An emerging problem following obesity surgery.

The incidence of obesity worldwide has increased markedly in the past 2 decades, with estimates of increases of 50% in the United States alone. Research indicates that weight loss produced by diet alone is not sustained and that 75% of dieters regain most of the weight lost within 1 year and 90% within 2 years. Morbid obesity is associated with comorbid conditions, including heart disease, hypertension, diabetes, mechanical arthropathy, sleep apnea, and numerous other serious disorders and a shortened life expectancy. Because of limited success with medical management, surgical treatment of mo

Levodopa responsiveness in disorders with parkinsonism: a review of the literature.

A literature review was conducted to investigate whether or not levodopa (LD) responsiveness (LR) is a useful criterion in the diagnosis of parkinsonian disorders. Although LR does appear to differ among the parkinsonian disorders, there is considerable confusion in the literature. While most patients with Parkinson's disease (PD) have a sustained benefit from LD, a small minority of patients with documented PD do not respond. The literature suggests that the LR rate is higher for multiple system atrophy (MSA), progressive supranuclear palsy (PSP), and corticobasal degeneration (CBD) than base

Childhood vitiligo.

Vitiligo is a common skin disorder in which pigment is lost from the skin and adnexae. Half of all cases begin before the age of 20 years. Until recently, there was a paucity of published data regarding effective therapy of vitiligo. Current therapy is quite effective, and while many patients may not experience complete repigmentation, 50% to 75% repigmentation of vitiligo with recent onset routinely can be expected with available therapeutics.

Direct nucleic acid diagnostic tests for bacterial infectious diseases: streptococcal pharyngitis, pulmonary tuberculosis, vaginitis, chlamydial and gonococcal infections.

Partnering with a reference lab can actually help grow a referring lab's own business. The reference lab provides a hospital's physicians with access to unique tests and clinical information and broadens the scope and depth of the referring lab's menu. As medicine moves toward prescribed treatments based on a patient's individual genotype or phenotype, the reference lab will play an even bigger role in helping the hospital to educate physicians and their patients to the most efficient testing strategies consistent with good patient management.

Education on-demand: the development of a simulator-based medical education service.

OBJECTIVES: Clinical medical education depends on the availability of instructive patient encounters, or "good teaching cases." While all medical students hope to see enough patients of sufficient scope and variety, exposure to good teaching cases has been traditionally limited by time and chance. Students may graduate from medical school without having seen a number of important cases, each of which may represent a knowledge gap they will carry forward into internship and future patient care. Recently, however, the advent of high-fidelity patient simulators has enabled instructors to recreate

Assessing the need for Clinical Breast Examination (CBE) training: the Oregon experience.

BACKGROUND: Despite the widely held perception that CBE training has high priority among the continuing medical education (CME) needs of breast health care providers, there is actually little published information to support this notion. METHODS: The authors conducted a statewide needs assessment mail survey of providers regarding a number of potential CME needs, including CBE training. RESULTS: Of the 4,179 surveys from the single mailing, 1,427 were returned (34% response rate). Six categories of provider types responded; 51% were physicians and 23% were nurse practitioners. Short course in pharmacoepidemiology and pharmacovigilance at the London School of Hygiene and Tropical Medicine.

Demand for education in pharmacoepidemiology is widespread, and has resulted in the short course at the London School of Hygiene and Tropical Medicine. This course is designed as an introduction but requires some scientific background to understand it. The course occurs annually, and covers basics of epidemiology, statistics and drug surveillance systems as applied particularly to drug safety issues. A successful completion results in a certificate from the University of London, and the course is accredited for continuing pharmacy and medical education purposes.

"I liked having my voice heard:" a feasible, rapid approach for codesigning a contraceptive counseling curriculum.

Few youth- and family-engaged codesigned models of medical education programs exist. One area that may benefit from community perspectives is designing education to improve counseling on sensitive topics. The authors' codesigned an adolescent-focused contraceptive counseling continuing medical education training program for practicing primary care clinicians in partnership with community advisory boards and assessed the feasibility and acceptability of their codesign approach. From September 2023 to January 2024, the authors recruited female teens, caregivers, and clinicians residing in North

Enhancing Creative Thinking and Innovation Competencies of Oncology Nurses Through an Innovation Workshop: A Mixed-Methods Study.

BACKGROUND: Creativity and innovation are essential for oncology nurses in addressing complex care challenges; however, few brief continuing education interventions are offered in this field. METHOD: This study used a mixed-methods, single-group pretest-posttest design with assessments conducted at baseline, immediately post-workshop, and at 4-week follow-up (n = 38). Outcomes were evaluated with the Marmara Creative Thinking Dispositions Scale and the Framework for Innovation Competencies Assessment. Post-workshop interviews were analyzed with Colaizzi's method. RESULTS: A significant impro

The role of the emergency physician in providing pediatric emergency care--a membership survey.

A survey of the membership of the American College of Emergency Physicians (ACEP) was undertaken to identify members with special interest or expertise in pediatric emergency medicine. A questionnaire was published in the August 1988 issue of ACEP News, which was distributed to 12,079 members. One hundred seventy-one responses were returned, revealing a subset of the membership (1.42%) with a special interest or expertise in pediatric emergency care. This group was characterized as to type of residency training, board certification (completed or planned), practice location, and percentage of p
