Chronic kidney disease and dementia: an epidemiological perspective.

Ageing populations worldwide face increasing challenges of multimorbidity (that is, the co-occurrence of two or more chronic conditions). The combination of chronic kidney disease (CKD) and dementia occurs more frequently than it would by simple coincidence, owing to several underlying biological and clinical mechanisms. Population-based cohort studies are an important epidemiological tool and have contributed to improved understanding of these mechanisms. These mechanisms include uniquely shared haemodynamic features of vasculature, overlapping risk factor profiles, and direct neurotoxic effe

Prenatal origins of adult disease.

PURPOSE OF REVIEW: Human epidemiological and animal studies show that many chronic adult conditions have their antecedents in compromised fetal and early postnatal development. Developmental programming is defined as the response by the developing mammalian organism to a specific challenge during a critical time window that alters the trajectory of development with resulting persistent effects on phenotype. Mammals pass more biological milestones before birth than any other time in their lives. Each individual's phenotype is influenced by the developmental environment as much as their genes. A

Air pollution exposure and cataract outcomes: a systematic review and pollutant-specific meta-analysis.

BACKGROUND: Air pollution is increasingly recognized as a potentially modifiable environmental determinant of age-related ocular disease, but its association with cataract remains incompletely characterized. Epidemiological evidence remains heterogeneous across pollutants and study methods, although emerging experimental findings support biological plausibility for pollution-related lens injury. OBJECTIVES: To systematically evaluate the epidemiologic evidence linking air-pollution exposure with cataract-related outcomes and to quantitatively synthesize pollutant-specific associations where s

The effects of cardiovascular risk factors on cognitive compromise.

As life expectancy in the United States continues to increase, the projected numbers of elderly people who will develop dementia will grow rapidly. This paper reviews four well-established cardiovascular risk factors (type 2 diabetes, hypertension, cholesterol, and inflammation), for which there is longitudinal epidemiological evidence of increased risk of dementia, Alzheimer's disease, mild cognitive impairment, and cognitive decline. These risk factors are of special interest because of their potential modifiability, which may affect the course of cognitive compromise. Diabetes is the cardio

Why we should offer routine vitamin D supplementation in pregnancy and childhood to prevent multiple sclerosis.

Multiple sclerosis (MS) is a demyelinating disease of the central nervous system that runs a chronic course and disables young people. The disease is more prevalent in the geographic areas that are farthest from the equator. No form of treatment is known to be effective in preventing MS or its disabling complications. A number of epidemiological studies have shown a protective effect of exposure to sunlight during early life and a recent longitudinal study confirmed that vitamin D supplementation reduced life-time prevalence of MS in women. Very little is known regarding the role of vitamin D

What Is the Impact of Maternal Pertussis Immunization in Pregnancy on the Quantity, Quality and Longevity of Infant Vaccine Responses?: A Review of the Current Evidence.

Immunizing pregnant women against pertussis has been a powerful tool adopted in many countries to effectively reduce morbidity and mortality from whooping cough in young infants when they are most vulnerable to complications and death. One of the main trade-offs, however, may be that maternally derived antibodies modify or modulate the infant's own antibody responses to routine childhood vaccines, although whether this is both a quantitative and a qualitative phenomenon, and its long-term clinical and epidemiological implications remain largely unclear. This review will summarize our current u

A critical review of therapeutic interventions in sickle cell disease: Progress and challenges.

Sickle cell disease (SCD) is an autosomal recessive genetic disorder that occurs due to the point mutation in the β-globin gene, which results in the formation of sickle hemoglobin (HbS) in the red blood cells (RBCs). When HbS is exposed to an oxygen-depleted environment, it polymerizes, resulting in hemolysis, vaso-occlusion pain, and impaired blood flow. Still, there is no affordable cure for this inherited disease. Approved medications held promise but were met with challenges due to limited patient tolerance and undesired side effects, thereby inhibiting their ability to enhance the qualit

Biomechanical modeling of deep squatting: Effects of the interface contact between posterior thigh and shank.

Epidemiological studies indicate that occupational activities that require extended deep knee flexion or kneeling are associated with a higher prevalence of knee osteoarthritis. In many sport activities, such as a catcher in a baseball or a softball game, athletes have to make repetitive deep squatting motions, which have been associated with the development of osteochondritis dissecans. Excessive deep knee flexion postures may cause excessive loading in the knee joint. In deep knee flexion postures, the posterior aspect of the shank will contact the posterior thigh, resulting in a compressive

Drug therapies for polymyalgia rheumatica: a pharmacotherapeutic update.

INTRODUCTION: Polymyalgia rheumatica (PMR), a common disease in individuals older than 50 in the western world, is characterized by bilateral inflammatory pain involving the shoulder girdle and less commonly the neck and pelvic girdle. The main goals of the currently available treatment are to induce remission and prevent relapse. AREAS COVERED: This review briefly presents the main epidemiological and clinical features of PMR and discusses in depth both its classical management as well as new therapies used in PMR. EXPERT OPINION: In general, patients with isolated PMR experience a rapid re

[The link between obesity and cancer].

While some genetic factors may explain the development of cancer, its main causes are related to environmental exposure to carcinogenic agents as well as to the effect of determined lifestyles and habits. Several epidemiological studies have shown a consistent relation between obesity and cancer. In non smokers, obesity is the most relevant risk factor in the development of malignant tumors. There is a clear association between obesity and endometrial cancer, breast cancer in postmenopausal women, pancreatic, esophageal and colon cancer. Sexual steroids, insulin like growth factor axis and adi

Expert opinion on emerging drugs: chronic low back pain.

INTRODUCTION: It is difficult to overestimate the personal and socioeconomic impact of chronic low back pain (CLBP). It is the leading cause of years lost to disability and poses the highest economic toll among chronic illnesses. Despite the strong need for extensive research efforts, few drugs have consistently demonstrated effectiveness for this condition. AREAS COVERED: In this review, the epidemiology, rationale for mechanism-based treatment, competitive environment and market trends, and the preclinical and clinical evidence supporting over 15 different classes of analgesic medications s

Parasite responses to resource provisioning can be altered by within-host co-infection interactions.

Anthropogenic changes to the environment significantly impact wildlife infectious diseases by modifying food resources and impacting host-parasite interactions through changes in host demography, behaviour and immune defences. Supplemental resource provisioning has been found to both enhance and mitigate parasite transmission; however, the role of co-infecting parasites in mediating these effects remains understudied. We developed a mathematical model to explore these dynamics, motivated by the empirical system of wood mice (Apodemus sylvaticus) infected with the nematode Heligmosomoides polyg

Environmental Signals.

Environmental factors have long been known to play a role in the pathogenesis of congenital heart disease (CHD), but this has not been a major focus of research in the modern era. Studies of human exposures and animal models demonstrate that demographics (age, race, socioeconomic status), diseases (e.g., diabetes, hypertension, obesity, stress, infection, high altitude), recreational and therapeutic drug use, and chemical exposures are associated with an increased risk for CHD. Unfortunately, although studies suggest that exposures to these factors may cause CHD, in most cases, the data are no

From Inhalation to Neurodegeneration: Air Pollution as a Modifiable Risk Factor for Alzheimer's Disease.

Air pollution, a growing concern for public health, has been linked to various respiratory and cardiovascular diseases. Emerging evidence also suggests a link between exposure to air pollutants and neurodegenerative diseases, particularly Alzheimer's disease (AD). This review explores the composition and sources of air pollutants, including particulate matter, gases, persistent organic pollutants, and heavy metals. The pathophysiology of AD is briefly discussed, highlighting the role of beta-amyloid plaques, neurofibrillary tangles, and genetic factors. This article also examines how air pollu

Effect of mixed infections of Sindbis and La Crosse viruses on replication of each virus in vitro.

Mixed viral infections are ubiquitous in natural populations and may have significant but unpredictable biological and epidemiological consequences. These infections may be acquired simultaneously (coinfection) or result from two single infections at different time intervals (superinfection). This study investigated the effect of mixed infections of two mosquito-borne viruses, Sindbis (SINV) and La Crosse (LACV), on the replication of each virus in vertebrate baby hamster kidney (BHK) and invertebrate Aedes albopictus (C6/36) cell lines. SINV and LACV were inoculated onto confluent monolayers

Ambulatory blood pressure monitoring: assessing the diurnal variation of blood pressure.

The purposes of this paper are to examine the effects of activity, situation of measurement, mood, and occupation on the daily variation of blood pressure and to discuss the potential utility of ambulatory blood pressure monitoring in physical anthropological studies. The subjects of the blood pressure variability study are 125 men who were referred to the Hypertension Center at New York Hospital--Cornell Medical Center for evaluation of hypertension. There were 1,386 blood pressure measurements from these subjects available for study, which were taken using noninvasive ambulatory blood pressu

Extreme temperatures and the epithelial barrier theory.

Extreme temperatures are now recognized as significant global health threats, driving morbidity and mortality across diverse diseases by compromising epithelial barrier integrity and causing periepithelial inflammation. The epithelial barrier theory posits that environmentally induced barrier dysfunction, driven by modern industrial and urban exposures, leads to compromised surfaces of the exposed tissues accompanied by epithelial activation and barrier dysfunction, microbial dysbiosis, loss of commensal bacteria, opportunistic pathogen colonization, bacterial translocation, inflammation and i

Six Years of COVID-19: Lessons from Epidemiology, Vaccination Campaigns, Clinical Risk Stratification, and Thromboembolic Surveillance.

Six years after the emergence of SARS-CoV-2, COVID-19 remains a dynamic public health challenge shaped by viral evolution, heterogeneous population immunity, changing vaccine strategies, and the long-term consequences of acute infection. Although the transition from pandemic emergency to endemic circulation has reduced the global burden of severe disease, COVID-19 continues to affect vulnerable populations, particularly older adults, frail patients, immunocompromised individuals, and subjects with multiple comorbidities. Vaccination has substantially modified the clinical course of infection,

Serial cycle threshold to assess the infectious potential of SARS-CoV-2: A systematic review.

We sought to assess predictive factors for SARS-CoV-2 infectiousness using a meta-analytic approach. We searched LitCovid, medRxiv, Google Scholar, and the WHO COVID-19 database until June 30 2025, including studies which cultured SARS-CoV-2, relating them to clinico-epidemiologic and laboratory variables and RT-PCR cycle threshold (Ct) values. Using linear mixed-effects regression models, we tested for independent associations with Ct values with 95%CIs and adjusted P-values in a multivariable model. We used a modified QUADAS criteria to assess risk-of-bias. We included 50 studies, with 39 in
