Posttraumatic loss of elbow motion can cause substantial disability, limiting the ability to put one's hand in the volume of a sphere in space. Although a number of conditions can lead to loss of motion, a greater understanding of the functional anatomy of the elbow has led to advances in surgical management. Elbow stiffness has been classified in a number of ways, but the consistent feature is capsular contracture. Treatment of the stiff elbow begins with clinical evaluation of elbow stiffness and identification of indications for surgical intervention. Techniques of open and arthroscopic elb…

Since its original description by O'Driscoll in 1991, PLRI is becoming increasingly recognized as a significant cause of elbow pathology. It is well documented that this problem results from an insufficiency of the radial ulnohumeral ligament and its related lateral structures, and that this insufficiency is usually the result of elbow trauma. Diagnosis has improved with the introduction of the PLRI test to identify the instability and advancing MR imaging capabilities of illustrating the injury. Whereas open reconstruction was previously the only definitive treatment, improving arthroscopic t…

Tuberculous synovitis in the elbow joint is extremely rare in developed countries. We describe a 68-year-old man who had had a gradually enlarging mass over the volar side of the left proximal forearm near the elbow joint for 4 months. Plain roentgenograms of the diseased elbow showed early osteoarthritic change. Magnetic resonance imaging revealed diffuse synovitis with a large 8 x 8 cm extra-articular synovial cyst. Synovectomy was performed and histopathologic examination of the surgical specimen revealed granulomatous inflammation with caseation, prominent Langhan's giant cells, and sparse…

Osteonecrosis is a disease that leads to joint destruction and often involves large joints, such as the hips, knees, and shoulders. Nontraumatic osteonecrosis of the adult elbow, to the best of the authors' knowledge, has not been reported. Nine adult patients with atraumatic osteonecrosis of 11 elbows were identified. The mean age at presentation was 36 years (range, 26-63 years); five patients were women and four were men. Six elbows involved the capitellum, three involved the lateral epicondyles, one involved the trochlea and radial head, and one involved medial and lateral epicondylar dise…

A girl 6 years 5 months old sustained a rare transverse (mediolateral) divergent dislocation of the elbow and was treated by closed reduction. The arm was immobilized in a cast. Followup was 7 years 5 months. Five years after injury, the patient had recurrent popping on the lateral aspect of the injured elbow during active flexion from complete extension and tenderness on the anterior aspect of the radiohumeral joint. To determine the exact anatomic relation of the proximal radius and ulna to each other and to the distal humerus after transverse dislocation, a skeleton of the elbow was assembl…

Chronic elbow tendinitis (medial or lateral epicondylitis and triceps tendinitis) are common disorders that, overall, have a good prognosis but, even with optimum management, require a minimum of 3 to 6 months to resolve. Patient education, activity modification, splinting, and corticosteroid injections each serve a role in symptom resolution; surgery is required in less than 10% of cases and has a similarly long period (3 to 6 months) of recovery. Acute tendon injuries (biceps and triceps ruptures) require early (< 4 weeks) surgical repair but may be misdiagnosed, thus delaying management and…

Arthroscopy of the elbow has become a valuable adjunct in the evaluation and treatment of elbow trauma and reconstruction. Arthroscopic visualization aids diagnosis and treatment while minimizing surgical trauma. The indications and techniques of elbow arthroscopy continue to expand and evolve. This article demonstrates the use of elbow arthroscopy as it relates to some of the topics in this volume. Arthroscopic setup, portal anatomy, technique, as well as an overview of trauma, loose bodies, bursitis, instability, arthrofibrosis, arthritis, and complications are discussed.

A prospective study from 1974 to 1996 was done to determine optimal treatment for chronic hemophilic synovitis of the knee and synovitis of the elbow. Sixty-five patients with synovitis affecting 65 knee joints and 40 patients who had synovitis of the elbow (44 elbows), despite a 3-month trial of prophylactic substitution therapy, were treated by synovectomy. Radiation synovectomies (Au-198 synoviorthesis) were done on 38 knees, open surgical synovectomy on 18, and nine had an arthroscopic procedure. Radioactive gold synoviorthesis was performed on 29 elbows, and 15 had a resection of the radi…

A 74-year-old woman was noted to have a mass lesion near the right elbow joint during medication for pulmonary tuberculosis. After discontinuation of medication, the mass gradually became enlarged with swelling and tenderness of the joint. Radiological evaluation disclosed tenosynovitis with an encapsulated abscess. Microscopic examination and culture of an aspiration biopsy specimen from the abscess showed no microorganisms. However, DNA extracted from the specimen contained mycobacterium tuberculosis DNA, permitting a diagnosis of tuberculous tenosynovitis. Mycobacterium is not always detect…

High-velocity trauma now often results in complex injuries to the upper extremity, and especially the elbow joint. These can lead to both an enormous reduction in the range of motion of the shoulder, elbow and wrist joints, in severe cases with complete loss of upper extremity function. A complex injury is defined as a fracture and/or dislocation of the elbow in association with (1) a serial injury of the upper extremity, (2) a severe soft tissue trauma, or (3) concomitant injury to vessels or nerves. Serial fractures, in particular can lead to enormous problems with treatment and are often as…

Arthroscopy has taken on a valuable role in the management of osteoarthritis of the elbow. Most patients have loose bodies, of which some are not detected on radiographs are all effectively removed arthroscopically. Excision of osteophytes from the olecranon and the coronoid as well as from the olecranon and coronoid fossae (to re-create the original fossae) eliminate the painful impingement at the extremes of motion that is typical of this condition. Release of contracted capsular tissues can also be performed at the same time by stripping the capsule from the humeral attachments. As with all…

Technical improvements in MR imaging have resulted in the ability to detect soft tissue and bone pathology, with great facility in the elbow joint. This allows application of MRI to the elbow in the same valuable ways it has been used in other joints for a much longer period.

Epicondylitis of the elbow involves pathologic alteration in the musculotendinous origins at the lateral or medial epicondyle. Although commonly referred to as "tennis elbow" when it occurs laterally and "golfer's elbow" when it occurs medially, the condition may in fact be caused by a variety of sports and occupational activities. The accurate diagnosis of these entities requires a thorough understanding of the anatomic, epidemiologic, and pathophysiologic factors. Nonoperative treatment should be tried first in all patients, beginning with an initial phase of rest, ice, nonsteroidal anti-inf…

Because of the popularity of sports participation, sports physical therapists must recognize in the athlete the many clinical conditions that occur about the elbow. The purpose of this paper is to present the most common elbow problems that an athlete may encounter and to provide information to facilitate recognition of elbow pathology. This information is essential before initiating treatment. An attempt is made to include sprains, strains, neuropathies, dislocations, fractures, contusions, vascular insults, and skin problems in the distal humerus, elbow, and proximal forearm of both the imma…

Arthroscopy of the wrist and elbow is a helpful diagnostic procedure in patients with chronic disability not demonstrated by clinical and radiological examinations. At the wrist, especially triangular fibrocartilage, chondral or ligamentous defects can often not be demonstrated by other tests. Arthroscopy of the wrist allows examination of more than a dozen separate articular surfaces and the function of eight discrete joints. The geometry of the elbow joint limits intra-articular inspection and the use of instruments. The risk of neurovascular injuries is great and depends on exact puncture s…

After a short review on literature the late result of an operated elbow with a follow-up time of 23 years is reported. The actual problems of this rare condition are discussed. Congenital hyperlaxity, a shallow semilunar notch and traumatic sequelae after previous dislocations reduce joint stability. Arthroscopy, X-ray tomography and MRI update accurate preoperative diagnosis. Early operation is recommended to reduce further damage in the joint. Nowadays well proved operative procedures provide good postoperative results.

Tennis elbow is due to a torque injury or sudden overstretching of tendons which insert into the epicondyles of the humerus. The predominant lesion is an enthesopathy--a pathological lesion at the insertion of tendon into bone. The most common site is at the lateral epicondyle and this is 3 times as frequent as at the medial epicondyle. Approximately 50% of tennis players can expect to get a tennis elbow at some time during their playing lifetime. In one-third of the players this will be severe enough to interfere with their tasks of daily living. The major unresolved question about the aetiol…
