OBJECTIVES/HYPOTHESIS: Spikes in cochlear implant impedance are associated with inner ear pathology after implantation. Here, we correlate these spikes with episodes of hearing loss and/or vertigo, with a comparison between lateral wall and peri-modiolar electrode arrays.
METHODS: Seven hundred seventy recipients of Cochlear's slim-straight, lateral wall electrode (CI422), or peri-modiolar (CI512) electrode were investigated for impedance spikes. Impedance fluctuations were defined as a median rise of ≥ 4 kΩ across all intracochlear electrodes from baseline measurements taken 2 weeks after sw

We study subjects with pharmacologically intractable epilepsy who undergo semi-chronic implantation of electrodes for clinical purposes. We record physiological activity from tens to more than one hundred electrodes implanted in different parts of neocortex. These recordings provide higher spatial and temporal resolution than non-invasive measures of human brain activity. Here we discuss our efforts to develop hardware and algorithms to interact with the human brain by decoding ensemble activity in single trials. We focus our discussion on decoding visual information during a variety of visual

Reaction at the site of pacemaker electrode was examined at post-mortem by morphological and histological means in a group of 18 deceased patients with pacemakers implanted through the endovasal channels. The changes were classified into 5 basic types depending on their nature and rate of incidence: changes in the endocardium (connective tissue bridging of the electrode, and changes in the tricuspid valve) and in the myocardium (electrode penetration, reaction under the tip of the electrode, and no changes). The particular types occurred either independently of each other, in in mutual combina

INTRODUCTION: In cases with residual-hearing (RH) loss after cochlear implantation, a safe method is needed to provide full spectral resolution and as much auditory information as possible without implant replacement. Aim of this study was to prove the feasibility of accessing a partially inserted cochlear-implant-electrode for complete insertion to its maximum length through the external ear canal using a transcanal approach.
METHODS: Two CI electrodes were customized with 18 stimulating channels. The electrode design enables the use of 12 active channels available for electrical stimulation

Foreign body response (FBR), inflammation, and fibrotic encapsulation of neural implants remain major problems affecting the impedance of the electrode-tissue interface and altering the device performance. Adhesion of proteins and cells (e.g., pro-inflammatory macrophages, and fibroblasts) triggers the FBR cascade and can be diminished by applying antifouling coatings onto the implanted devices. In this paper, we report the deposition and characterization of a thin (±6 nm) sulfobetaine-based coating onto microfabricated platinum electrodes and cochlear implant (CI) electrode arrays. We found t

Brain‒computer interfaces (BCIs) exhibit significant potential for various applications, including neurofeedback training, neurological injury management, and language, sensory and motor rehabilitation. Neural interfacing electrodes are positioned between external electronic devices and the nervous system to capture complex neuronal activity data and promote the repair of damaged neural tissues. Implantable neural electrodes can record and modulate neural activities with both high spatial and high temporal resolution, offering a wide window for neuroscience research. Auditory perceptions produced in a person deaf to acoustic stimulation were studied by electrically exciting the auditory nerve through permanently implanted electrodes. Pulsed current as small as 1 microampere peak-to-peak could be perceived. Pitch, as reported by the subject, varied with electrode selection, current amplitude, and pulse repetition rate from about 70 to at least 300 pulses per second. Loudness increased with amplitude and duration of pulse stimuli, and to a lesser extent with repetition rate. The total range in amplitude of the stimulus, from threshold to an uncomfortable lou

The auditory brainstem implant (ABI) can provide hearing sensation to individuals where the auditory nerve is damaged. However, patient outcomes with the ABI are typically much poorer than those for cochlear implant recipients. A major limitation to ABI outcomes is the number of implanted electrodes that can produce auditory responses to electric stimulation. One of the greatest challenges in ABI surgery is the intraoperative positioning of the electrode paddle, which must fit snugly within the cochlear nucleus complex. While there presently is no optimal procedure for intraoperative electrode

BACKGROUND: Deep brain stimulation is an effective and safe technique. Displacement of the electrode relative to the optimal stimulation site can lead to insufficient effect and sometimes to the need of operative electrode re-position.
OBJECTIVE: This study was aimed to analyse targeting accuracy of deep brain stimulation electrode implantation to subthalamic nucleus (STN) and globus pallidus internus (Gpi). It detected possible causes of inaccuracy and prevalent shift to certain direction.
METHODS: Targeting accuracy was analysed in 47 patients with Parkinson´s disease (PD) and 11 patients

Ectopic bone in the scala tympani is characteristic of cochlear otosclerosis and meningitis, which are the two most frequent etiologies of deafness in cochlear implant patients. The ectopic bone in a multiple-electrode implanted bone is probably caused by disease (syphilis) rather than by the electrode or electrical stimulation. The sensory elements being stimulated by intracochlear electrodes probably are the ganglion cells.

The electrical activity of the olfactory bulb was recorded in awake, unrestrained cats with electrodes permanently implanted. It was found that any kind of sensory stimulation producing alertness or arousal brought about the appearance of bursts of rhythmic activity, the magnitude of which was related to the degree of alertness of the cat.

Chronically indwelling percutaneous intramuscular electrodes were implanted in the upper extremity muscles of five adolescents with C5 or C5-6 tetraplegia in an effort to provide lateral and palmar prehension using the neuroprosthetic system designed by Case Western Reserve University. The responses from 177 electrodes were evaluated at 3-month intervals and included measurements of electrical impedance and an assessment of recruitment properties. Electrode failures were categorized as breakage, an altered stimulated response or adverse sensation during stimulation. Survival probabilities were

BACKGROUND: Electrical stimulation is an emerging therapy for urologic disorders. The interface between electrode and neural tissue is a critical region of interest given it affects thresholds of neural activation. Measures of electrode impedance are used clinically to estimate tissue fibrosis at the interface; however, the relationship between impedance and fibrosis is not well defined.
OBJECTIVE: This study aimed to assess the relationship between interface tissue fibrosis and nerve distance from electrode surface with measurements of impedance and evoked thresholds using a chronic model of
