The mode of movement selection. Movement-related cortical potentials prior to freely selected and repetitive movements.

In two previous studies, the readiness potential (RP) has been reported to be influenced by the mode of movement selection. Freely selected movements were found to have a higher RP amplitude than fixed repetitive movements. This was attributed to the higher demands on planning for the performance of freely selected movements. However, movements in the free mode are distinct from movements in the fixed mode in more than one respect. For example, they are also associated with a higher degree of alteration of the side and/or the finger of movement execution and hence serial "novelty" across block

Intermittent horizontal saccade failure ('ocular motor apraxia') in children.

BACKGROUND: Ocular motor apraxia (OMA) in childhood is a poorly understood condition involving a failure of horizontal saccades. OMA is thought to be rare but the literature indicates wide clinical associations. OMA is often identified by abnormal head movements, but failure of reflexive quick phases has been reported in all but a few patients. The extent of this oculomotor disorder was examined in a large group of children with diverse clinical backgrounds. METHODS: The degree of quick phase failure during horizontal vestibular and optokinetic nystagmus was measured using DC electro-oculogra

Effects of transcranial magnetic stimulation over the region of the supplementary motor area during sequences of memory-guided saccades.

Transcranial magnetic stimulation (TMS) over the region of the supplementary motor area (SMA) was used to study the cortical control of sequences of memory-guided saccades. In ten healthy subjects, TMS was applied during (a) the target presentation (learning) phase, (b) the memorization phase, and (c) the execution phase of such saccade sequences. Stimulation during the presentation phase resulted in a significant increase in errors, compared to the results without stimulation. In contrast, stimulation during the memorization or execution phases had no significant influence on the performance

The electrooculogram in heterozygote carriers of Usher syndrome, retinitis pigmentosa, neuronal ceroid lipofuscinosis, senior syndrome and choroideremia.

Electrooculographic studies were performed in 77 carriers of tapetoretinal dystrophies: Usher syndrome (20), retinitis pigmentosa (32), neuronal ceroid lipofuscinosis (6), Senior syndrome (2), and choroideremia (17). The carriers were matched for sex and age with normal controls. In carriers of Usher syndrome the EOG Lp/Dt ratio was significantly lowered with 30% of the recordings having a subnormal value. There was a trend in carriers of retinitis pigmentosa to a subnormal EOG. In contrast to previous studies there was no decrease in the EOG Lp/Dt ratio in carriers of neuronal ceroid lipofusc

Electrophysiology of type II mesangiocapillary glomerulonephritis with associated fundus abnormalities.

The retinal electrophysiology is reported in four patients with type II mesangiocapillary glomerulonephritis and partial lipodystrophy with associated fundus abnormalities and no visual symptoms. The histological hallmark of the condition is that of widespread electron dense deposits in the renal glomerulus and in the choriocapillaris and Bruch's membrane of the eye. Three of the four patients had the typical fundal appearance of multiple, yellow, drusen-like lesions at the posterior pole of the eye with normal visual acuity. These three patients had abnormally low Arden ratios on electro-ocul

[Multiple system atrophy with macro square wave jerks and pendular nystagmus].

A 51-year-old woman was admitted to our hospital because of gait disturbance and dysuria. Neurological examination revealed limb and truncal ataxia, orthostatic hypotension, cogwheel rigidity in all limbs, generalized hyperreflexia without pathological reflex, and horizontal gaze nystagmus. She became progressively worse and bedridden at age 52. Then she developed abnormal eye movements. Electrooculogram revealed vertical, horizontal or oblique macro square wave jerks and pendular nystagmus. Macro square wave jerks appeared during fixation and disappeared with eye closure or in the dark room.

Evaluation of retinal integrity in eyes with retained intraocular metallic foreign body by ERG and EOG.

In three eyes with a retained intraocular metallic foreign body, the neural retina and retinal pigment epithelial (RPE) integrity were evaluated by means of ERG (a-wave, b-wave, oscillatory potentials) and non-photic EOG responses from the RPE (hyperosmolarity response, Diamox response, bicarbonate response). All three cases were judged in an early stage of retinal impairment on the basis of a well-preserved or supernormal b-wave amplitude and showed good postoperative prognosis. Out of three non-photic responses from the RPE, only the bicarbonate response was preoperatively reduced in two out

Ocular motor apraxia following cardiac surgery.

Ocular motor apraxia is characterized by a deficiency in the generation of voluntary saccadic eye movements despite the presence of spontaneous saccades. We report two cases of isolated ocular motor apraxia, both in children, that developed following cardiac surgery. Electro-oculography was done in one case and showed slow saccades and decreased pursuit gain. The authors review the neuroanatomic and neurophysiologic features relevant to supranuclear gaze mechanisms.

[Contribution of MRI to the topography of oculomotor disorders in multiple sclerosis].

Magnetic resonance imaging (MRI) was performed in 20 patients with multiple sclerosis and abnormal electro-oculographic examination. All but 2 patients showed MRI abnormalities in the infratentorial region: hypersignal on T2-weighted sequences and/or images of atrophy. Usually, each patient had multiple abnormalities, which could prevent anatomico-oculographic correlations. With oculomotor disorders of cerebellar origin, correlations between clinical findings and MRI images were satisfactory, but with disorders due to brainstem lesions correlations were not so good, as shown by the results in

[Sleep disorders from the neurological viewpoint].

Sleep can be measured by EEG or a monitor which records motor activity. In this way objective data on sleep behavior can be obtained from patients. Total sleep time over 24 hours remains remarkably constant above age 20. Therefore, any changes in total sleep time can be considered to be pathologic. A true shortening of sleep time is rare and is usually associated with a psychosis. More often "common sleep disorders" are encountered, which show a normal total sleep time per 24 hours. Prolonged sleep, usually associated with increased day-time somnolence, is often caused by chronic meningitis or

[Sleep and wakefulness in Greenland seal pups].

ECoG of both hemispheres, EOG, neck EMG and EKG were recorded in 2 white (age 10 days) and 2 gray pups (age 1 month) of harp seal. The active wakefulness occupied 23.4 +/- 3.8% of total recording time, the relaxed wakefulness--32.6 +/- 3.6%, drowsiness--4.8 +/- 1.1%, slow wave sleep--31.5 +/- 3.0%, paradoxical sleep--7.7 +/- 0.8%. The sleep cycle averaged 18.2 +/- 1.4 min. Interhemispheric asymmetry of the ECoG was not seen in all 4 pups. The respiration was fast and regular in the relaxed wakefulness, then long respiration pauses were alternated with episodes of hyperventilation during slow w

Acute effects of two dosages of orally administered midazolam on psychomotor performance in healthy volunteers.

The acute effects of two doses of orally administered midazolam (Dormicum, 10 and 30 mg) which as a putative anaesthetic premedicant is clinically expected to provide valid effects already after acute dosing on sedation parameters, were evaluated vs placebo within a randomized double-blind 3-period crossover design in 12 healthy male volunteers. The washout period was 7 days. Objective sedation measures were obtained with the oculodynamic test (ODT), which is a multidimensional computerized psychological testing device. Cardiorespiratory parameters were simultaneously sampled throughout the OD

Obese patients with sleep apnea syndrome treated by progesterone.

Two obese patients with sleep apnea syndrome were administered chlormadinone acetate (CMA), a synthetic progesterone, known as a potent respiratory stimulant to augment load compensation response as well as CO2 chemosensitivity. Before CMA administration, both cases showed normal chemosensitivity of hypoxic and hypercapnic ventilatory responses (HVR and HCVR) at daytime, although marked oxygen desaturation with sleep apnea was observed. During CMA administration for 7 days, HVR, HCVR and occlusion pressure response to flow-resistive loading were altogether augmented. In one case obstructive sl

Observations on the Tullio phenomenon.

Vestibular responses (vertigo, nystagmus-like eye movements) to acoustic stimuli are known as the "Tullio phenomenon". Detailed electro-oculographic analysis of this reaction, as observed in a 30-year-old patient, revealed the following: a maximum amplitude of eye movement (mainly vertical) was achieved by sine wave bursts of high intensity, a frequency of 500 to 1000 Hz and a duration of 100 ms. The ocular deviation was composed of a fast initial component, followed by a slower resetting movement that was often divided into two parts of different velocities. At longer stimulus durations (more

Laurence-Moon-Bardet-Biedl syndrome: electrophysical and psychophysical findings.

Four cases of Laurence-Moon-Bardet-Biedl syndrome were studied through electrophysiological and psychophysical examinations, and findings were correlated with those of fluorescein angiography. In all cases the macula showed degenerative changes, including a bull's eye appearance. The fundus periphery could be devoid of clumps of pigmentation, but all cases showed profound functional abnormalities that affected the macula and the periphery, impairing both the cone and rod systems equally. Bull's eye macula associated with severe, diffuse functional abnormality of the macula and peripheral retin

[Albescent punctate retinopathy with central pigment epithelium atrophy].

A 45-year-old Italian patient suffering from punctate retinopathy associated with a central atrophy of the retinal pigment epithelium is described. He has suffered from nyctalopia since early childhood, and over the last 3 years has become increasingly susceptible to dazzling. Funktion tests revealed pathologic monofunctional dark adaptation of cones, centrocecal scotoma, a pathologic electro-oculogram and residual a-waves at high light stimulus intensities in the electroretinogram. An autosomal-recessive mode of inheritance is assumed, as 4 of the patient's 9 siblings have the same signs.

Hypnotic efficacy and clinical safety of midazolam in shift-workers.

The effect of 15 mg midazolam and 15 mg oxazepam compared with placebo was investigated in 12 shift-workers in a randomized cross-over sleep laboratory study with psychometric testing. Sleep latency was normal under all experimental conditions. Midazolam shortened the sleep latency, reduced the frequency of nocturnal awakenings, and increased the length of sleep stages 3 and 4. Both midazolam and oxazepam reduced the total waking time and prolonged the total sleep time. Neither of the two benzodiazepines negatively influenced performance after awakening. On the basis of these findings, midazol

Voluntary nystagmus masquerading as Tullio's Phenomenon.

Nystagmoid eye movements elicited by humming loudly was initially misdiagnosed as Tullio's phenomenon (abnormal acoustical stimulation of the vestibular apparatus owing to labyrinthine fistula). Further workup lead to a final diagnosis of a form of voluntary nystagmus characterized in some reports as "hysterical mystagmus". Some of the characteristics of this example of voluntary nystagmus differed from generally accepted descriptions.

[Value of functional and electrophysiological studies in amblyopia].

In a group of patients suffering from amblyopia, the authors confront the results after orthoptic treatment with data provided by functional and electrophysiological examination. In 37 patients the clinical diagnosis of functional amblyopia had been made, but in 8 of these cases complimentary functional and electrophysiological examinations showed organic perturbations. This explains the failure, in these cases, of the orthoptic treatment. In the remaining 29 patients, clinical forms of functional amblyopia were found. In this group, the authors studied 20 cases showing no improvement with tre
