Extracranial internal carotid artery (ICA) aneurysms account for <1.0% all aneurysms and a rare indication for carotid intervention. Causes include atherosclerotic degeneration, trauma, dissection, previous carotid surgery, connective tissue disorders, and infection. Authors report a case of a middle-aged male found to have a large aneurysm of the left ICA who underwent repair by resection and reconstruction with end-to-end anastomosis under neuroprotection with flow reversal. Our discussion includes a recommendation for this particular surgical repair. The patient in this case report has gran

The SAPPHIRE trial has established carotid artery stenting (CAS) in high-surgical-risk patients as an effective alternative to carotid endarterectomy. Since the occurrence of distal embolization with CAS is still a major concern, an embolus protection device is usually employed during the procedure. Although the advantage of the filter device is continuation of blood flow during the procedure, blood flow is occasionally reduced due to plaque debris or thrombus. Here, we report a case of internal carotid artery (ICA) stenosis complicated with filter obstruction during CAS. A 74-year-old woman w

BACKGROUND: Endovascular internal trapping is the first-line treatment for ruptured vertebral artery dissection (VAD). However, when critical branches such as the posterior inferior cerebellar artery (PICA) or the anterior spinal artery (ASA) are involved with the dissected segment, the risk of surgery-related ischemia is high.
OBSERVATIONS: Two patients with ruptured VAD involving critical branches were treated using the temporary frame coil technique. In the first case, a distal temporary frame coil protected the PICA origin during proximal coil embolization and was subsequently retrieved.

Carotid artery stenosis remains a major cause of ischemic stroke worldwide, and its management continues to evolve in parallel with advances in surgical, endovascular, and medical therapies. Carotid endarterectomy (CEA) was established as the standard of care for symptomatic high-grade stenosis following landmark randomized trials, while carotid artery stenting (CAS) subsequently emerged as a less invasive alternative for appropriately selected patients. This review aims to summarize the historical evolution of carotid artery stenting, critically appraise evidence from major clinical trials co

BACKGROUND: Saphenous vein graft (SVG) percutaneous coronary intervention (PCI) carries a high-risk of slow or no-reflow (SNR)-up to 15% in stable ischemia and 29% in acute coronary syndrome (ACS)-leading to higher major adverse cardiovascular event rates. Evidence on high-dose intracoronary adenosine for SNR prevention remains limited.
METHODS: We analyzed 171 patients with stable ischemic heart disease or ACS who underwent SVG PCI without embolic protection devices from 2010 to 2024 at our center. All received 200 to 400 μg intracoronary adenosine within 30 seconds just prior to each balloo

ObjectiveTo evaluate a novel peel-away sheath technique for sheathless transradial delivery of an 8 Fr balloon guide catheter (BGC), using a 6 Fr peel-away sheath and a 6 Fr inner catheter, without requiring device exchange or dedicated inner dilators.MethodsIn this technique, a 6 Fr peel-away sheath is first inserted into the radial artery, through which a 6 Fr inner catheter and guidewire are advanced. The sheath is then peeled and removed. The preloaded 8 Fr BGC (Branchor X), coaxially mounted over the inner catheter, is subsequently advanced into the radial artery. We retrospectively analy

BACKGROUND: Carotid artery stenting (CAS) is an alternative to carotid endarterectomy (CEA) for revascularization of the internal carotid artery (ICA). CAS from the femoral approach may be problematic due to peripheral vascular disease, anatomical variations of the aortic arch, and access site complications. The purpose of this study was to evaluate the right radial approach (RRA) for CAS.
METHODS: A retrospective analysis of all patients who had undergone transradial (TR) CAS at two centers was performed. Demographics, the technique used to deploy the sheath in the common carotid, procedural

INTRODUCTION: Two and a half million Americans with atrial fibrillation are at an elevated risk for embolic stroke. Warfarin therapy is standard treatment for high-risk patients, yet 40-65% of elderly patients do not receive anticoagulation therapy due to bleeding complications. To address this clinical need, we are evaluating a minimally invasive stent-based stroke prevention device to divert emboli from entering the arterial supply of the brain.
METHODS: The feasibility of a J-shaped stroke prevention device was tested in a mock circulatory loop. Sixteen sets of 100 simulated emboli (1-5 mm

The authors report their experience with the temporary placement of inferior vena caval filters to prevent pulmonary embolism in acute deep venous thrombosis patients. Twenty devices--6 Filcard and 4 Bruneau type--were positioned and left in situ over a time period ranging 4 to 14 days (mean: 9.8). In one patient the filter was positioned and no adjunctive medical therapy given to provide protection before nephrectomy; five patients were treated with i.v. heparin that provided no vein patency but prevented disease progression. Due to failure in positioning infusion guide catheters within the t

BACKGROUNDS: Stroke remains a serious complication of transcatheter aortic valve implantation (TAVI). The Sentinel cerebral embolic protection (CEP) system is designed to mitigate this risk by capturing embolic debris, but its clinical benefit remains uncertain.
AIMS: This meta-analysis evaluated the impact of Sentinel CEP on stroke and related outcomes in TAVI.
METHODS: PubMed, Embase, and Cochrane were searched for randomized controlled trials (RCTs) and propensity score-matched (PSM) studies comparing TAVI with and without Sentinel CEP. The outcomes were periprocedural ischemic stroke, to

BACKGROUND: Percutaneous coronary intervention (PCI) of saphenous vein grafts (SVG) can be challenging due to no reflow phenomenon from distal embolization of debris and microvascular vasoconstriction, resulting in myocardial injury post-procedure. Guidelines promote the use of distal embolic protection devices (EPD) to protect the distal arterial bed during SVG PCI. However, this approach has shown less-than-optimal results in many studies. We report our data using the Borgess protocol [prophylactic intracoronary (IC) nicardipine injection and direct stenting], as an alternative to EPDs in a

Elimination of the embolic potential of existing thrombus, restoration of unobstructed flow, prevention of further thrombosis, and preservation of venous valve function are the ideal goals of therapy for acute deep vein thrombosis (DVT). Meeting these goals will not only prevent pulmonary embolus, but will also minimize long-term sequelae of venous hypertension and development of postthrombotic syndrome (PTS). Treatment strategies aimed at eliminating or reducing risk of PTS should focus on preserving valvular function and eliminating risk of continued venous obstruction after acute DVT. Throm

Percutaneous treatment of saphenous vein graft (SVG) lesions has been associated with higher rates of periprocedural complications and restenosis compared with non-SVG lesions. Whether these outcomes are similar in contemporary clinical practice, particularly when drug-eluting stents are used, is unknown. We evaluated outcomes of 110 consecutive patients who were treated with stent-assisted percutaneous coronary intervention for 145 SVG lesions (drug-eluting stents used in 91.0% of lesions). Embolic protection devices were used in 52.1% of treated grafts. Adverse events were recorded up to 1 y

Percutaneous coronary intervention (PCI) has evolved significantly with technological advances, allowing for higher procedural success rates and improved patient outcomes. However, the growing complexity of cases involving severely calcified lesions, tortuous vessels, and chronic total occlusions has increased procedural challenges. In such scenarios, the guide extension catheter (GEC, or guiding catheter extension) plays a critical role by enhancing device passage and providing additional backup support, making it indispensable in modern PCI. Various types of GECs offer distinct characteristi

The authors carried out a prospective study aimed at revealing predictors of acute embolic lesions of cerebral vessels during angioplasty with stenting of the internal carotid artery. The study enrolled a total of 54 patients who between May 2015 and December 2018 underwent carotid angioplasty with stenting performed at the Department of Vascular and Endovascular Surgery of the Research Centre of Neurology. The procedure of internal carotid artery stenting may be accompanied by intraoperative acute embolic lesions. In order to reveal intraoperative acute embolic lesions of cerebral vessels all

INTRODUCTION: Endovascular treatments, such as transcatheter aortic valve implantation (TAVI), carry a risk of embolization due to debris dislodgement during various procedural steps. Although embolic filters are already available and marketed, mechanisms underlying cerebral embolism still need to be elucidated in order to further reduce cerebrovascular events.
METHODS: We propose an experimental framework with an in silico duplicate allowing release of particles at the level of the aortic valve and their subsequent capture in the supra-aortic branches, simulating embolization under constant

AIMS: Proximal occlusion (PO) and distal filter (DF) serve for cerebral embolic protection during carotid artery stenting (CAS). New cerebral lesions at diffusion-weighted magnetic resonance imaging (DW-MRI) represent a surrogate endpoint for embolisation, though their clinical impact is controversial. We performed a meta-analysis of randomised and observational DW-MRI studies comparing PO and DF during CAS.
METHODS AND RESULTS: We searched electronic scientific databases. The primary endpoint was the incidence of new cerebral lesions at DW-MRI; secondary endpoints were the incidence of new i

PURPOSE: To investigate the phenomenon of distal embolization during subintimal angioplasty with or without stenting of femoropopliteal chronic total occlusions (CTOs).
METHODS: This prospective study included 40 consecutive patients (35 men; mean age 65.4±9.3 years) who underwent subintimal angioplasty alone (n=14) or with stenting (n=26) of CTOs in the femoropopliteal segment. A Spider protection filter was used to capture any possible macro- or microemboli generated during balloon inflation/stenting. Arterial outflow was angiographically checked during each consecutive procedural step. All
