BACKGROUND: Millions of U.S. women live in contraceptive deserts, with limited or no access to comprehensive reproductive health services. Barriers are intensified by Medicaid restrictions, provider shortages, and growing reproductive health inequities. The emergency department (ED), as the healthcare safety net, is uniquely positioned to expand contraceptive access.
OBJECTIVES: To highlight the public health need for ED-initiated contraception, describe feasible approaches for implementation, and address common challenges to integrating contraception into emergency care.
DISCUSSION: Women o

BACKGROUND: High staff turnover is a widespread issue across nearly all hospital departments, often exceeding 20% annually. This constant flux disrupts continuity of care and creates a recurring challenge: how to rapidly integrate new employees into complex clinical environments, both physically and functionally. Traditional onboarding methods struggle to meet this demand, particularly in services operating 24/7, such as emergency departments (EDs).
OBJECTIVE: This formative study presents the design and implementation of a web-based 3D gamified simulation platform aimed at improving staff on

INTRODUCTION: People who use substances increasingly access healthcare primarily through emergency medical services (EMS) and emergency departments (EDs). To meet the needs of these patients, EMS and EDs have become access points for medications for opioid use disorder (OUD), specifically buprenorphine. This systematic review aimed to quantify the efficacy of these programs, examining retention in treatment for OUD, rates of re-presentation to ED or EMS, and rates of precipitated withdrawal, as well as summarise clinician and patient perspectives on buprenorphine initiation in these settings.

INTRODUCTION: Many patients receive a suspected diagnosis of cancer through an emergency department (ED) visit. Time to treatment for a new diagnosis of cancer is directly associated with improved outcomes with little no describing the ED utilization prior to the diagnosis of cancer. We hypothesize that patients that have an ED visit in proximity to a diagnosis of cancer will have worse outcomes, including mortality.
METHODS: This study is a retrospective cohort study of all patients with cancer diagnosed at Eskenazi Health (Indiana) between 2016 and 2019. Individual health characteristics, E

A 16-year-old female presents to a community emergency room following a suicide attempt by hanging. Prehospital, on arrival of paramedics, the patient was in a pulseless electrical activity rhythm. Paramedics provided advanced cardiac life support for 20 minutes before they obtained return of spontaneous circulation. In the emergency department, she had another 25-minute cardiac arrest with ultimate return of spontaneous circulation. She is now hemodynamically stable on Levophed 0.2 µg/kg/min. Her neurological exam shows pupils to be 3 mm and fixed bilaterally, absent cough and gag, and no res

BACKGROUND: hospital admissions of residents from nursing homes often lead to delirium, infections, mortality and reduced functional capacity. We initiated a new service, 'emergency department-based acute care service', maintained by consultants from an emergency department (ED) moving emergency care from the hospitals into nursing homes.
OBJECTIVE: this study explored healthcare professionals' experiences with this service.
DESIGN: qualitative semi-structured focus group discussions.
INTERVENTION/SETTING: the new service provides acute on-site evaluation and treatment to nursing home resid

PURPOSE: Emergency departments (EDs) across the country have grappled with overcrowded conditions for decades, which has only intensified in recent years, leading to the current crisis of ED boarding. The magnitude of boarding in the ED today is an unprecedented challenge faced by hospitals across North America and results in ambulance diversions, complications due to delays in care, and unacceptable numbers of individuals leaving without being seen by a provider. Boarding has significant immediate and downstream effects, including delayed care, medication errors, delirium, higher rates of mor

BACKGROUND: The aim of the study was to appraise and compare the influence of community‑based interventions (C-BIs) for childhood asthma.
METHODS: We examined meta-analysis data and employed dichotomous or continuous random or fixed-effect models to obtain odds ratios (ORs) and mean differences (MDs) with 95% confidence intervals (CIs). This review examined 13 papers that included a total of 8824 people with asthma.
RESULTS: Children with C-BI had significantly lower asthma-connected emergency department visits (OR, 0.29; 95% CI, 0.22-0.39, p < 0.001), hospitalizations (OR, 0.24; 95% CI, 0.1

INTRODUCTION: An Emergency Medical Service (EMS) is defined as a complete system that responds to public medical and surgical emergencies with prompt and adequate emergency care. Ambulance services are also classified as EMS in modern medical history. In the Nepalese context, prehospital care is very limited, and the EMS system is still a new concept in Nepal. In a study in the emergency room at Patan Hospital in Kathmandu, only 9.9% of patients came by ambulance, 53.6% by taxi, 11.4% by private car, 13.5% by bus, 5.4% by bicycle, and another 6.2% came with alternative routes.

OBJECTIVE: The objective of this study was to assess the impact of the 2010 Chilean earthquake on hospital functions and services. Hospitals functioning in a post-disaster environment must provide emergency medical care related to the event, in addition to providing standard community health services. This study focused on damage to both structural and nonstructural components, as well as to utility services.
METHODS: Site visits were made to every hospital in a single province (Bio-Bio). Engineers conducted damage assessments while interviews of hospital administrators were conducted. The su

INTRODUCTION: In-flight medical emergencies are common occurrences that require medical professionals to manage patients in an unfamiliar setting with limited resources. Emergency medicine (EM) residents should be well prepared to care for patients in unusual environments such as on an aircraft.
METHODS: We developed a simulation case for EM residents featuring a 55-year-old male passenger who suffers a cardiac arrest secondary to a tension pneumothorax. We conducted this case eight times during a 5-hour block of scheduled simulation time. Participants included EM residents of all training le

Emergency clinicians must rapidly evaluate the acutely ill or injured child. In a resource-stressed environment, "spotting the sick child" is essential for appropriate stabilization, treatment, and further management. Overlooking clinical features in a child's presentation may impede timely care. Complicating factors include the volume of patients seeking care, unfettered access to emergency services, parental perceptions and expectations, and clinician biases. Notwithstanding, after an appropriate history and physical exam, some children do not fall under the standard rubric of "sick or not s

OBJECTIVES: The focus on improving patient safety has mainly been by learning from errors and near misses (Safety-I). We applied a novel Safety-II approach to identify and learn from practice variations in the diagnostic process in the emergency department (ED), and subsequently design and implement practice changes.
METHODS: In this single-center study, we used action research (cycles of micro-experiments to study and improve processes with active stakeholder involvement) in the diagnostic process. We used three subsequent observation cycles of the following six steps: observations, gatherin

Short-term exposure to air pollution has been associated with exacerbation of asthma and chronic obstructive pulmonary disease (COPD). This study investigated the relationship between emergency hospital admissions for asthma, COPD and episodes of poor air quality in an English city (Southampton) from 2008-2013. The city's council provides a forecasting service for poor air quality to individuals with respiratory disease to reduce preventable admissions to hospital and this has been evaluated. Trends in nitrogen dioxide, ozone and particulate matter concentrations were related to hospital admis
