Thermal epiglottitis in adults: a new complication of illicit drug use.

Four cases of acute epiglottitis due to thermal injury were identified in a larger study of 407 cases of epiglottitis in Rhode Island from 1975 through 1992. All occurred in young adults (aged 22-33 yr) and were caused by the inhalation of heated objects when smoking illicit drugs (a tip of a marijuana cigarette in 1 case and metal pieces from crack cocaine pipes in 3 cases). Symptoms, signs, and X-ray and laryngoscopic findings were similar to infectious epiglottitis. All recovered with observation and intravenous antibiotics; none required intubation. Emergency physicians should be aware of …

Nosocomial epiglottitis associated with penicillin- and cephalosporin-resistant Streptococcus pneumoniae bacteremia.

We cared for a 4-year-old male with nosocomially acquired epiglottitis caused by Streptococcus pneumoniae. He had been receiving ceftazidime therapy when this infection was recognized. The S. pneumoniae isolate was of serotype 15B and was resistant to beta-lactam antibiotics, cephalosporins (including those with extended spectra), and trimethoprim-sulfamethoxazole. Clinicians and clinical microbiologists must be aware that cephalosporin susceptibility may no longer be assumed for penicillin-resistant S. pneumoniae isolates and that susceptibility testing for the extended-spectrum cephalosporin…

Acute airway obstruction in a seven-month-old infant with epiglottitis.

A seven-month-old male infant with epiglottitis developed acute airway obstruction in the operating room during halothane induction. This case suggests that epiglottitis should be suspected in any child, regardless of age, who presents with stridor, respiratory distress, dysphagia, or "tripoding," and it reiterates the need for prompt airway management.

Acute epiglottitis. A management protocol based on a 15-year material.

During a 15-year period 141 patients were treated for acute epiglottitis. 70 of the patients needed an artificial airway which was established by means of nasotracheal intubation. The mean intubation time was 41 h. All patients except 1, who suffocated before a free airway could be established, were discharged from the hospital without any serious sequelae. A follow-up study revealed no serious physical or psychological damage caused by the intubation treatment. On the basis of our 15-year material and the literature a protocol for the handling of patients suspected for acute epiglottitis is p…

Diagnosis and management of acute epiglottitis -- report of 90 consecutive cases.

Ninety children with acute epiglottitis were admitted from 1971 - 1977. Lateral extended neck radiograph was found to be a quick, safe and reliable way to establish the diagnosis. Out of 79, 77 (97%) had blood cultures yielding Hemophilus influenzae type b. Immediate institution of airway and antibiotics were the mainstay of treatment. All patients in the series survived. For short-term airway management, as in acute epiglottitis, nasotracheal intubation under general anesthesia was found to be the airway of choice.

Recurrent Steroid-Responsive Epiglottitis Following Cord Blood Transplantation: A Case Report.

Epiglottitis is an acute inflammation of the epiglottis, typically caused by bacterial infections and potentially life-threatening due to rapid airway obstruction. There are extremely rare case reports of epiglottitis as a complication following allogeneic stem cell transplantation (allo-SCT). We report a 56-year-old female case with adult T-cell lymphoma (ATL) who suffered from recurrent epiglottitis after cord blood transplantation (CBT). Her epiglottitis was refractory to empirical antibiotic therapy. However, it responded promptly to corticosteroids, preventing the development of severe re…

Breakthrough COVID-19 infection inducing acute epiglottitis in an immunized host.

We present the case of a 32-year-old man vaccinated with the Johnson & Johnson coronavirus disease 2019 (COVID-19) vaccine who presented with pharyngodynia and breakthrough COVID-19 infection. The patient was diagnosed with severe acute respiratory syndrome coronavirus 2 infection via nasal swab and acute epiglottitis by imaging and flexible laryngoscopy. While COVID-19 is known to present with pharyngodynia, nasal congestion, rhinorrhea, and olfactory dysfunction, epiglottitis is not a typical presentation. Fewer than 10 cases of acute epiglottitis in the setting of COVID-19 have been reporte…

Factors Affecting Patients with Concurrent Deep Neck Infection and Acute Epiglottitis.

Deep neck infection (DNI) is a serious disease of deep neck spaces that can lead to morbidities and mortality. Acute epiglottitis (AE) is a severe infection of the epiglottis, which can lead to airway obstruction. However, there have been no studies of risk factors in patients with concurrent DNI and AE. This study was performed to investigate this issue. A total of 502 subjects with DNI were enrolled in the study between June 2016 and August 2021. Among these patients, 30 had concurrent DNI and AE. The relevant clinical variables were assessed. In a univariate analysis, involvement of the par…

Long-term outcome of critically ill adult patients with acute epiglottitis.

BACKGROUND: Acute epiglottitis is a potentially life threatening disease, with a growing incidence in the adult population. Its long-term outcome after Intensive Care Unit (ICU) hospitalization has rarely been studied. METHODOLOGY AND PRINCIPAL FINDINGS: Thirty-four adult patients admitted for acute epiglottitis were included in this retrospective multicentric study. The mean age was 44 ± 12 years (sex ratio: 5.8). Sixteen patients (47%) had a history of smoking while 8 (24%) had no previous medical history. The average time of disease progression before ICU was 2.6 ± 3.6 days. The main reaso…

[Descending necrotizing mediastinitis resulting from acute epiglottitis; report of a case].

Descending necrotizing mediastinitis (DNM) is a serious condition due to abscess spreading from neck into mediastinum. We report a case of DNM following acute epiglottitis and abscess. A 59-year old male visited our hospital for parotid swelling, pyrexia, and dyspnea, and was diagnosed as having acute epiglottitis at otology and hospitalized. Despite right tonsillectomy, drainage and tracheostomy on day 6 after hospitalization, he developed DNM and was referred to our department. Drainage of mediastinal regions( anterior to the trachea and tracheal bifurcation, inferoposterior mediastinum and …

[Acute obstructive epiglottitis in intensive care unit: which airway management strategy?].

Acute epiglottitis is an infection of supraglottic structures, with a high risk of life-threatening respiratory obstruction. We report a case of a 52-year-old patient who presented with a prehospital hypoxic respiratory arrest caused by obstructive oedema with a successful resuscitation. Laryngoscopy after a sudden unplanned extubation permits diagnosis of acute obstructive epiglottitis. The growing threat of laryngeal dyspnea prompts emergency tracheal airway protection, by means of the combined preparation of transtracheal oxygenation access and the use of fiber-optic laryngoscopy. This case…

Acute epiglottitis as the initial presentation of pediatric Systemic Lupus Erythematosus.

We report a case of a 5-year old girl, who initially presented with acute epiglottitis, sepsis and multi-organ failure. She was subsequently diagnosed as having Systemic Lupus Erythematosus. To the best of our knowledge, this article describes the first case of Haemophilus influenzae type f epiglottitis as the initial presentation of SLE in childhood.

Epiglottitis and necrotizing fasciitis: a life-threatening complication of infectious mononucleosis.

Life-threatening cervical complications associated with infectious mononucleosis are rare. The combination of acute epiglottitis and subsequent necrotizing fasciitis of the head and neck in a patient with infectious mononucleosis has not been reported to date.A 47-year-old female with infectious mononucleosis and epiglottitis was admitted to hospital for i.v. therapy. Owing to her poor clinical condition and the spread of the infection to the throat and superior mediastinum, as evidenced by CT, a cervical debridement was performed. After cervical debridement, histological findings were consist…

Acute epiglottitis caused by Haemophilus influenzae type b: a case report.

Acute epiglottitis is an inflammatory, edematous disease of the epiglottis and adjacent structures, usually caused by Haemophilus influenzae type b. It is a life-threatening condition, occurring mainly in childhood. There have never been any reports of this condition in Taiwan. We report a case of 4-year-old boy who presented with characteristics of systemic illness combined with respiratory distress on arrival at the emergency room. His mouth was open and his neck was hyperextended. The diagnosis of epiglottitis was established on the basis of physical examination, lateral neck x-ray, and the…

[Acute epiglottitis in adults: a rare cause of acute respiratory insufficiency].

Acute epiglottitis is an infective disease which may have a rapid and aggressive course to total upper airway obstruction. Due to its rarity (mostly in adults) and aspecific initial clinical features, it may present diagnostic problems to less experienced medical personnel. This is the principal reason why the mortality rate is still unacceptably high. Indirect or direct laryngoscopy is mandatory for diagnosis: as this might be difficult for non-specialist physicians, their awareness of the potentially severe consequences of epiglottic infections is fundamental to minimizing diagnostic delay. …
