CONTEXT: Worldwide, only nine cases of revealing slipped capital femoral epiphysis (SCFE) associated with primary hyperparathyroidism (PHP) have been reported.
CASE ILLUSTRATION: This study included adolescent subjects with the described association, the clinical course, and exhibiting the leading pathogeneses.
METHODS: Here, we reviewed all known cases and developed an effective approach to the management of SCFE and PHP.
RESULTS: In cases of emergency, SCFE fixation is primarily done regardless of any preexistent hypercalcemia due to PHP and followed by parathyroidectomy as soon as possib

Slipped capital femoral epiphysis (SCFE) is characterized by displacement of the capital femoral epiphysis from the metaphysis through the physis. The term is confusing, because the metaphysis moves upward and outward while the epiphysis remains in the acetabulum. The SCFE is considered stable when the child is able to walk with or without crutches, and it is considered unstable when the child cannot walk with or without crutches. Patients with SCFE present with pain in the groin, knee and limp. The current treatment of stable SCFE is in situ stabilization with a single screw.

INTRODUCTION: The acute slipped capital femoral epiphysis can result in development of avascular necrosis of the femoral head which is very difficult condition to treat. Orthopedic surgeon can influence the development of avascular necrosis.
OBJECTIVE: The main objective in our study was to identify factors influencing the development of avascular necrosis in nonoperative treated patients for the acute slipped capital femoral epiphysis.
METHOD: A total of 53 patients and 59 hips treated for the acute slipped capital femoral epiphysis at the Institute for Orthopedic Surgery,Banjica" between 1

Slipped capital femoral epiphysis (SCFE) and Blount's disease are reported to have a common etiology, but there is only one report describing two cases in which adolescent Blount's disease coexisted with SCFE. In this article, we describe a case of SCFE following contralateral infantile Blount's disease in an 11-year-old boy. This report is the first known case of SCFE associated with infantile Blount's disease. In this patient, pelvic tilt caused by leg length discrepancy associated with infantile Blount's disease and possible general weakness of the growth plate may be related to the occurre

BACKGROUND: Treatment of a slipped capital femoral epiphysis (SCFE) should stabilize the epiphysis and prevent complications. Attempting to obtain physeal closure is still controversial. The purpose of this study was to evaluate the clinical results and complications after treatment of SCFE.
METHODS: From 1989 to 2000, 12 patients (14 hips) underwent pinning for treatment of SCFE. For acute and acute-on-chronic slippage, longitudinal traction was attempted for reduction. Patients with chronic slippage received fixation in situ. All patients were available for follow-up for an average of 63 mo

We retrospectively reviewed 28 pin-removal procedures in 20 patients with slipped capital femoral epiphysis, who were all treated by in-situ pinning with Kowles pins. Failure to extract the pins occurred in 8 cases (28.6%), and in another 8 cases (28.68%), pin retrieval was only possible after major excavation of the lateral femoral cortex. In view of these data we do not recommend routine removal of Knowles pins used for in-situ pinning of slipped capital femoral epiphysis.

Biomechanics of pin fixation for acute slipped capital femoral epiphysis (SCFE) was studied in an in vitro immature canine model. Acute SCFE were created in 24 paired femurs. The paired specimens were pinned with either one or two pins and loaded to failure. Strength and stiffness of the paired limbs were expressed as percentages of the loads necessary to create the initial SCFE in the intact specimen. Strength and stiffness were equivalent statistically for the intact physis and the fractured physis fixed with two pins. Single pinning was only 83% as strong and 78% as stiff as the intact phys

External epiphysiolysis with valgus slip of the femoral epiphysis is rare. The authors report two such chronic cases, one unilateral and the other bilateral. This disorder complicates a coxa valga with a reversed cephalic growth plate, downward and externally. This coxa valga which appears constitutional is sometimes associated with an increase in femoral antetorsion. Treatment is the same as classical varus slip of the femoral epiphysis.

Among 56 slipped capital femoral epiphysis (SCFE) treated over 12 years, the authors report 10 particular cases: 3 hypopituitarisms, 1 adrenal hyperplasia, 1 case after radiotherapy, 2 osteochondrodysplasia, 2 cases with major defect of the posterior wall of femoral neck and 1 coxa valga slip. Literature is analysed with special reference to etiological forms representing a predisposition to slippage. Awareness of these conditions under which SCFE can occur is an important factor. Surgical stabilization is classical, but two points are discussed: stabilization may need to be augmented by autol

Legg-Calvé-Perthes disease and slipped capital femoral epiphysis in the same child occurs rarely. Three cases from Indiana University Medical Center (Indianapolis, IN) are reported. The two disease entities appear to be unrelated. However, a child with Legg-Calvé-Perthes disease in one hip may develop a slipped capital femoral epiphysis in the contralateral hip.

In 50 consecutive cases of slipped capital femoral epiphysis (SCFE) from 49 families, the heredity was analyzed by radiographic examination of the first-degree relatives and by interview regarding the second-degree relatives. In four of the 49 families, SCFE was obvious in one or more first-degree relatives; and in another 13 families (14 relatives), radiographic signs of SCFE were found besides the primary case. The familial accumulation was much higher than expected from incidence studies, indicating a hereditary factor in the etiology.

Slipped capital femoral epiphysis is a common malady which must be managed by virtually every orthopedic surgeon. While each of us has a "foolproof method" of managing the disease, we also have somewhere in our files a patient with a disastrous outcome. The next several instructional cases will focus on the trials and tribulations of this disorder. Each of the discussants has had to formulate his opinions regarding management at each step in the case presentation, without knowledge of the subsequent outcome. We hope through this process we all will learn from our own and others' mistakes.

Five patients with slipped capital femoral epiphysis were conclusively diagnosed as being hypothyroid. One was a cretin, who was diagnosed and begun on thyroid medication at four months of age but subsequently many years later developed the slip. Suspicion of hypothyroidism should exist for the clinician who finds himself with a child below age 12 with a delayed bone age, borderline mental status and a slipped capital femoral epiphysis. His attention in addition to the orthopedic management of the slip, should be directed towards laboratory studies to rule out hypothyroidism.

Experimental and clinical studies have shown that the capital femoral epiphysis slips by rotating backwards and downwards around the curved surface of the metaphysis and remains in a plane that lies posteriorly at right angles to the anteversion plane of the neck of the femur. The relevance of these findings to the diagnosis and treatment of the condition is discussed. A geometric flexion osteotomy designed to correct the deformity is described.
