Neonatal chylothorax is a rare presentation leading to significant respiratory distress, thus requiring timely diagnosis. A preterm neonate was resuscitated and ventilated, following which she clinically improved but subsequently developed respiratory distress with a right-sided pleural effusion. Interestingly, thoracentesis fluid appeared 'milky' with elevated triglycerides and lymphocytes, suggesting chylothorax. PURPOSE OF REVIEW: The causes of exudative pleural effusions are diverse and frequently remain unclear despite exhaustive examinations. Recently recognized IgG4-related disease (IgG4-RD) is a fibroinflammatory disorder that can affect nearly any organ including the lungs. This review will focus on the involvement of IgG4 in exudative pleural effusion of unknown cause.

SUBJECT AND METHODS: Most malignant tumors of the breast originate from ductal epithelium. A direct examination of the ductal system, could significantly improve diagnostics of breast cancer as well as its preinvasive stages (DCIS) and to influence mortality. The article is devoted to a combined method of treatment of malignant tumorous lesions of the pleura accompanied by exudative pleurisy. The main result of using the method is cytoreduction, discontinuation of exudation and loss of protein, reduction of inflammation, intoxication and pain syndrome. It results from argonoplasmic electocoagulation of the pleura followed by photodynamic therapy. The treatment was used in 10 patients with a palliative aim. The molecular mechanisms used by the gonococcus to initiate infection exhibit gender specificity. The clinical presentations of disease are also strikingly different upon comparison of gonococcal urethritis to gonococcal cervicitis. By performing a thoughtful evaluation including a detailed history and careful physical examination, appropriate studies can be selected to allow the practitioner to arrive at the correct diagnosis of nipple discharge in a nonmorbid, expeditious, and inexpensive manner. This article has presented a simple, cost-effective, minimally morbid algorithm for the evaluation of nipple discharge.

PURPOSE/METHODS: Acute syphilitic posterior placoid chorioretinitis causes visual loss in patients with secondary syphilis. The condition is characterized by vitreitis and large, yellow placoid lesions in the macula. We examined a patient with syphilis who had a serious retinal detachment and an exudative fluid meniscus (pseudohypopyon).
RESULTS/CONCLUSIONS: Visual recovery and complete resolution of the chorioretinal lesion occurred after intravenous penicillin therapy. Secondary syphilis should be considered in the differential diagnosis of any patient with a macular pseudohypopyon.

Patients with a palpable mass often benefit most from aspiration. As the clinician tries to aspirate, fluid may be obtained if the lesion is cystic. A solid lesion can be assessed with FNA biopsy with a very high degree of accuracy. Excisional biopsy should be used when a cystic lesion recurs, the mass does not disappear after the cyst is aspirated, or if the fluid obtained is bloody. A solid lesion may need to be excised if the aspirate is negative. The overall detection rate of breast cancer is approximately 20% for excisional biopsies. Several reports indicate that patients with macrocysts have a two- to fourfold higher risk of developing cancer.
