OBJECTIVE: To assess the effects of tendon vibration on isometric torque and electromyographic activity of fatigued muscles.
DESIGN: Subjects performed a 30-trial isometric (2-s maximal effort contractions alternating with 6-s rest periods) elbow extensor fatigue series. Three additional trials with the same work-to-rest ratio were then performed in which the triceps extensor tendon was subjected to a 60-Hz, 1-mm vibration during the rest intervals.

Multiple sclerosis frequently results in a wide range of symptoms which often coexist, creating a complex pattern of disability. Chief among these symptoms, both in relation to their frequency and their impact on the patient, are spasticity, ataxia and fatigue. This chapter discusses the pathological basis and current treatment of these symptoms and stresses the importance of a multidisciplinary approach to their management, producing a comprehensive care plan which incorporates these and any other coexisting problems.

This prospective, cohort study examined the prevalence of sleep disorders among highly selected patients with chronic fatigue. On the basis of responses suggestive of sleep pathology on a screening questionnaire, 59 patients from a university-based clinic for chronic fatigue who had undergone a medical and psychiatric evaluation underwent polysomnography. Intolerance to various foods is reported often by patients seeking evaluation for chronic fatigue, a common symptom in primary care practice. We have examined the muscle biopsies of 50 patients who had postviral fatigue syndrome (PFS) for from 1 to 17 years. We found mild to severe atrophy of type II fibres in 39 biopsies, with a mild to moderate excess of lipid. On ultrastructural examination, 35 of these specimens showed branching and fusion of mitochondrial cristae. Mitochondrial degeneration was obvious in 40 of the biopsies with swelling, vacuolation, myelin figures and secondary lysosomes. We report on a 36-year-old female patient suffering from bilateral inguinal pain. The x-ray revealed significant osteoporosis of both proximal femurs with an impacted fatigue fracture of the right calcar and Looser zones at the left subtrochanteric femur. The intestinal biopsy proved coeliac disease, resulting in a secondary malabsorption. The authors conclude that osteomalacia in young patients could indicate coeliac-induced malabsorption.

The epidemiology of the post viral fatigue syndrome was studied for the years 1985-86. With a strict definition of the syndrome, it was found that there were many misconceptions about this illness. The sex incidence was nearly equal with a similar pattern of twin peaks at 25-29 years and 40-45 years. At diagnosis, 56% were ill for three to six months and only 9% for more than two years. It is estimated that this syndrome is more common than infectious mononucleosis.

We studied the pressure developed by the diaphragm in response to stimulation of the phrenic nerve in the neck, in three normal men. When the phrenic was electrically stimulated at increasing frequencies the diaphragm responded by increasing transdiaphragmatic pressure to give a frequency-pressure curve similar to the frequency-force curve for other skeletal muscles.
