BACKGROUND: Schwannomas (neurinomas) are among the most frequent peripheral nerve tumors. Nevertheless, these are seldom located in the lower extremities in association with the femoral nerve.
CLINICAL PRESENTATION: In this case, the occurrence of a schwannoma adjacent to the femoral nerve is described in a patient presenting with nonspecific pain in the thigh accompanied by a palpable mass. Sensory and motor deficits were not present. Further examinations by MRI and biopsy followed by histopathology, revealed the diagnosis of a schwannoma. The tumor was completely excised.

Previously, we showed that case-specific non-linear finite element (FE) models are better at predicting the load to failure of metastatic femora than experienced clinicians. In this study we improved our FE modelling and increased the number of femora and characteristics of the lesions. We retested the robustness of the FE predictions and assessed why clinicians have difficulty in estimating the load to failure of metastatic femora. A total of 20 femora with and without artificial metastases were mechanically loaded until failure. These experiments were simulated using case-specific FE models.

CASE: A 64-year-old man diagnosed with prostate cancer was incidentally found to have a lesion in his distal femur. Although initially concerning for metastatic prostate cancer, after biopsy by an orthopaedic oncology specialist, a diagnosis of liposclerosing myxofibrous tumor (LSMFT) was confirmed. In long bones, the most common site for metastases is the proximal femur. For lesions involving this region, osteosynthetic devices frequently fail, and for this reason, endoprosthetic reconstruction may be the optimal choice for treatment. Specific anatomical and pathological criteria for resectability of so-called central chondrosarcoma of the extremities were set forth and a method of treatment based on autoclaving the segment and surrounding it with massive amounts of fresh autogenous cancellous bone was used in eight patients. Follow-up ranged from 3.2 to 13.5 years. The advantages of the method are: ease of procurement of bone graft, absence of storage and measurement problems, and lack of need for rigid immobilization. No secondary procedures were required because of fracture, resorption, or infection.
