Ipsilateral foot drop after aortobifemoral bypass.

Peripheral nerves can be injured by a direct result of the anesthetic technique of regional anesthesia or it can be contributed/compounded by poor perioperative positioning and padding, tourniquets, and the nature of surgery and diseases affecting the microvasculature of nerves. We present a case of perioperative peripheral nerve injuries which could not be explained by anesthesia technique nor surgery per se.

[A Case of Meralgia Paresthetica Treated with Neurolysis].

A 60-year-old woman presented with a 1-year history of pain and numbness in the left anterolateral thigh. The symptoms aggravated on walking and standing. Her visual analogue scale(VAS)score was 7.1/10. Tinel's like sign was positive over the lateral femoral cutaneous nerve(LFCN), in the inguinal ligament region. LFCN block at the trigger point, in the inguinal ligament, resulted in relief of the symptoms and we diagnosed meralgia paresthetica(MP), which is the entrapment neuropathy of the LFCN. Initially, we performed observation therapy with oral medication and LFCN blocks. However, these tr

Saphenous neuropathy secondary to extraneural ganglion cyst 15 years after reconstruction of the anterior cruciate ligament.

This case report describes a 43-year-old woman with a previous anterior cruciate ligament reconstruction and medial meniscectomy who presented with right knee dysesthesias radiating into her medial ankle with associated allodynia. An extensive work-up was pursued, without a definitive diagnosis found or improvement in the patient's symptoms. Eventually, she was referred to Pain Medicine for an ultrasound-guided saphenous nerve (SN) block, where a scan before the injection revealed a large cyst anteromedial to the sartorius in the expected location of the SN. Magnetic resonance imaging and surg

Complete paralysis of the quadriceps secondary to post-traumatic iliopsoas hematoma: a systematic review.

PURPOSE: Paralysis of the femoral nerve secondary to compression by a hematoma of the iliopsoas is rarely post-traumatic. The acute surgical removal of hematoma seems the treatment of choice. The main objective of this systematic review was to determine the optimal delay between the trauma and surgery, to obtain a total functional recovery. METHODS: A search was performed via PubMed. The inclusion criteria were the studies in English language, reporting the results of the treatment of femoral nerve palsy secondary to compression by a post-traumatic hematoma of the iliopsoas. The primary evalu

Electrodiagnostic testing in lumbosacral plexopathies.

Patients presumed to have lower limb symptoms localizing to the lumbar or lumbosacral plexus require rigorous electrophysiological evaluation. Entities that cause lumbosacral plexopathies may be patchy, asymmetrical and more diffuse than initially suspected. As a result, bilateral nerve conduction studies and needle examination outside those routinely tested and clinically affected may be needed to document the extent of involvement including needle examination of the thoracic paraspinals and consideration of upper limb studies. This article outlines the lumbar and lumbosacral plexus anatomy,

Lumbar disc herniation mimicking meralgia paresthetica: case report.

BACKGROUND: Meralgia paresthetica, a syndrome of pain and/or dysesthesia in the anterolateral thigh, is normally caused by an entrapment of the lateral femoral cutaneous nerve (LFCN) at the anterior superior iliac spine. In a few cases compression of the nerve in the retroperitoneum has been reported to mimic meralgia paresthetica. CASE DESCRIPTION: A 67-year-old woman presented with a 5-year history of permanent paresthesia in the anterolateral thigh. Motor weakness was not detected. Electromyography showed a neurogenic lesion at the level of L3. Lumbar spine MRI detected a foraminal-extrafo

[Fasting and postprandial blood glucose in subjects with crural pain and sciatica].

The search for diabetes in a patient with crural neuralgia is well established practice. The prevalence of diabetes in the general population is not known, and studies attempting to demonstrate a relationship between diabetes and radiculopathies have been inconclusive. In the present study, fasting and postprandial blood glucose determinations, as well as 14 other parameters, were analyzed in 88 patients with classical sciatica, 27 with crural neuralgia, and 42 with only back pain. Multidimensional statistical analysis revealed that patients with crural neuralgia were older than those with cla

Rare Common Peroneal Nerve Neuropraxia after Removal of Upper Tibial Pin Traction.

Common peroneal nerve (CPN) injury is an uncommon complication associated with skeletal traction. Neurological deficits occurring after removal of upper tibial pin traction (UTPT) are exceedingly rare and sparsely reported in the literature. A 27-year-old male sustained a closed left subtrochanteric femur fracture following a road traffic accident. Preoperative UTPT was applied with no immediate neurological deficit. UTPT was removed intraoperatively before definitive fixation. Postoperatively, the patient developed left-sided foot drop. Electromyography and nerve conduction velocity studies a

Foot drop in the setting of VA-ECMO: A possible alternative mechanism to ischemia.

Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is used widely as part of extracorporeal resuscitation (ECPR). Prolonged VA-ECMO may be associated with the development of foot drop, often secondary to limb ischemia. We report on three patients who developed foot drop following prolonged ECMO use, and we propose an alternative mechanism to ischemia. Three male patients, mean age 46.3 years, underwent peripheral VA-ECMO support, for an average of 22.6 days (17-25 days), two patients following ECPR and one post-cardiotomy shock. All patients underwent distal femoral artery perfusion c

Surgery for Meralgia Paresthetica: neurolysis versus nerve resection.

AIM: Neurolysis with transposition of the nerve and nerve resection are commonly performed surgical procedures for appropriate managing Meralgia Paresthetica (MP). But long-term outcome of these two procedures are uncertain. This case-series study came to address follow-up results of these two surgical procedures for managing MP. MATERIAL AND METHODS: We prospectively described 14 consecutive non-obese patients with clinical features of MP managed by one of the two studied procedures (nerve resection or neurolysis). Clinical variables were documented for each individual. The outcome of the su

Comparison of effectiveness of different surgical treatments for meralgia paresthetica: Results of a prospective observational study and protocol for a randomized controlled trial.

INTRODUCTION: Various surgical procedures can be applied in the treatment of meralgia paresthetica. The two main ones are neurolysis and neurectomy of the lateral femoral cutaneous nerve. To date, no prospective or randomized controlled trial has compared the effectiveness of these procedures with standardized outcome measures. In this study we present our results for two prospectively followed cohorts and we present the protocol for a double blind randomized controlled trial (RCT). METHODS AND ANALYSIS: All patients that had an indication for surgical treatment of idiopathic meralgia paresth

Endocrine disorders.

The effect of thyroxine replacement therapy on bone mass continues to attract attention. Although the advice of the American Thyroid Association to normalize serum thyroxine and thyroid-stimulating hormone concentrations is recommended, even physiologic doses of thyroxine may have an adverse effect on bone mass. The diagnosis and treatment of osteomyelitis in the diabetic foot is aided by magnetic resonance imaging, and long-term review of cases of diabetic femoral neuropathy emphasizes the good prognosis. Adrenal insufficiency as a complication of antiphospholipid syndrome is reviewed, and a …

Electrophysiological Study of Femoral Nerve Function After a Continuous Femoral Nerve Block for Anterior Cruciate Ligament Reconstruction: A Randomized, Controlled Single-Blind Trial.

BACKGROUND: A continuous femoral nerve block (CFNB) is an effective analgesic treatment after anterior cruciate ligament (ACL) reconstruction but may result in transient femoral nerve injuries and quadriceps muscle weakness, which in turn contribute to worsened functional outcomes. PURPOSE: To compare electrophysiological criteria of a femoral nerve injury as well as functional and pain-related outcomes after ACL reconstruction when analgesia was provided by a CFNB or intravenous patient-controlled analgesic of morphine (IV PCA). STUDY DESIGN: Randomized controlled clinical trial; Level of e

Sonographic and electrophysiologic findings in patients with meralgia paresthetica.

OBJECTIVE: This study was aimed to demonstrate the usefulness of ultrasonography in the diagnosis of meralgia paresthetica (MP). METHODS: Clinical data, sensory nerve conduction study and sonographic findings of 23 patients with unilateral MP were analyzed retrospectively. Twelve healthy subjects were recruited for the optimal cut-off value for the cross-sectional area of LFCN to diagnose MP using receiver operating characteristic curves. RESULTS: The median value of the cross-sectional area of LFCN in healthy subjects was 3mm(2) and in the affected and unaffected sides in patients with MP,

Complete occlusion after blunt injury to the abdominal aorta.

BACKGROUND: Injury to the abdominal aorta after blunt trauma is uncommon. When this injury results in complete vessel occlusion, the presentation is dramatic. Timely intervention is essential. METHODS: After a case report, we examined all reported cases of complete occlusion after blunt injury to the abdominal aorta and reviewed the cause, presentation, and management of this injury. RESULTS: Complete vessel occlusion arises from intimal injury. The most frequent mechanism is compression from a seat belt or steering wheel during a motor vehicle crash. Patients present with absent femoral and
