Acute intrapartum twin-to-twin transfusion following successful fetoscopic laser ablation.

Fetoscopic laser ablation of placental vascular anastomoses is an intrauterine therapy targeting the source of twin-to-twin transfusion syndrome (TTTS). Our patient had successful laser treatment, with resolution of stage II TTTS, suggesting closure of all significant anastomoses. After an uneventful pregnancy course, she elected a trial of labor. Following spontaneous onset of labor, acute intrapartum twin-to-twin transfusion was suspected, suggested by sinusoidal heart rate changes, confirmed postdelivery by twins' complete blood counts. The potential risks of labor with prior fetoscopic las

Prenatal intervention for isolated congenital diaphragmatic hernia.

PURPOSE OF REVIEW: We aim to review the recent literature regarding early prenatal prediction of outcome in babies diagnosed with isolated congenital diaphragmatic hernia, as well as results of fetal therapy for this condition. RECENT FINDINGS: Current survival rates in population-based studies are around 55-70%. Highly specialized centers report 80% and more, but discount the hidden mortality, mainly in the antenatal period. Fetuses presenting with liver herniation and a lung-to-head ratio of less than 1.0 measured in midgestation have a poor prognosis. Other volumetric techniques are being

Pseudo-arterio-arterial anastomoses in twin-twin transfusion syndrome.

OBJECTIVE: It has recently been claimed that fetoscopic recognition of a haemodynamic equator within an arterio-arterial anastomosis (AAA) suggests minimal net intertwin flow. This was based on blood from one fetus being dark and from the other bright red, the boundary between them reciprocating with the fetal heart beats. However, bright red indicates that the blood had passed through a cotyledon and been freshly oxygenated, which should be impossible in an AAA. We applied a computer model of chorionic vessels to determine a configuration that reproduced this phenomenon. METHODS: A previousl

[Twin-twin transfusion syndrome--front line in fetal medicine].

Twin-twin transfusion develops in 10-20% of monochorionic twin pregnancies. As far as we know the underlying disturbance is an unbalanced anastomosis between the two fetuses on the placental surface. Without treatment mortality is as high as 80% if diagnosed before viability. One of this article's authors spent two months in Hamburg at the "Allgemeine Krankenhaus", Barmbeck in Germany and describes the technique used there to coagulate these anastomoses via fetoscope. The outcome seems very promising with overall survival of 80% using this method. In the article pathogenesis and alternative tr

Endoscopic techniques in fetal surgery.

Fetal endoscopic surgery (FETENDO) involves many techniques that allow surgical procedures to be performed inside the uterus without an hysterotomy. The impetus for developing these minimal access techniques for fetal surgery is the unusual occurrence with an open hysterotomy of preterm labor, premature rupture of membranes, and maternal complications resulting from tocolytic therapy. The unique requirements of this approach necessitated a modification of existing endoscopic techniques, the development of novel fetoscopic instruments, and the inclusion of a wide variety of specialists. Twin-twin transfusion syndrome: the 'Select' procedure.

OBJECTIVES: To assess physicians' preferences on diagnostic pathways and treatment priorities for systemic lupus erythematosus (SLE) using a discrete choice experiment (DCE). METHODS: A board of 11 SLE experts and a DCE expert statistician defined informative profiles of diagnostic pathways, pharmacological therapies, and two distinct profiles of mild-moderate and severe SLE. An independent panel of 115 clinicians involved in SLE management was invited to participate. Parameter estimates from the model were interpreted as relative preference weights (PWs). The mean PWs were used to calculate

A model for fetal surgery through intrauterine endoscopy.

Video-endoscopic technology may offer new hope for the future of fetal surgery. Ten time-dated pregnant ewes (95 to 105 days; term, 145 days) were used to develop a model of fetal surgery through intrauterine endoscopy. Special 5-mm ports were designed with short, flexible, balloon-tipped shafts. The cannulas were inserted through purse-stringed hysterostomies. Amnioinfusion of warmed physiological fluid (Hartmann) was used as the work medium. Fetal heart rate and oxygen saturation (pulse oximetry), temperature, and amniotic pressure were monitored continuously through endoscopically placed se

[Significance of cardiotocography and amnioscopy in establishing indications for cesarean section].

In the period of 2 years (1974-1975), out of 11,672 deliveries, 398 (3.41%) were induced by cesarean section. An important role in inducing labour surgically was played by cardiotocography and amnioscopy (in addition to other methods), i.e. in 84.22% (366 cases). At present it appears necessary to use cardiotocography and aminiscopy both in the last trimester and during the labour itself, so as to facilitate the estimation of the degree of a possible danger for the foetus.

Risk Stratification and Fetal Therapies for Congenital Diaphragmatic Hernia.

Congenital diaphragmatic hernia (CDH) remains one of the most challenging congenital anomalies encountered by neonatologists and pediatric surgeons, with outcomes largely determined before birth. Although surgical repair of the diaphragmatic defect is essential, postnatal morbidity and mortality are primarily driven by antenatal pulmonary hypoplasia, abnormal pulmonary vascular development, and associated cardiac dysfunction that culminate in respiratory failure and pulmonary hypertension. Advances in prenatal imaging have enabled increasingly refined risk stratification, allowing for the iden

Persistent compression of uterine insertion on maternal blood dynamic change in fetoscopic laser photocoagulation surgery: a novel method.

BACKGROUND: Fetoscopic laser photocoagulation (FLP) has been performed as first-line approach for twin-to-twin transfusion syndrome (TTTS) in monochorionic twin pregnancies. The majority of researches are focusing on advances in fetal treatment and long-term outcome, few researches on maternal surgery-related complications are reported. This study is to compare the maternal blood dynamic changes in women with and without persistent compression on the uterine insertion after the FLP procedure. METHODS: This was a retrospective study conducted at two tertiary referral centers in China between N

The role of superficial anastomoses on fetal survival after laser treatment for twin-twin transfusion syndrome: a subanalysis of the sequential trial.

BACKGROUND: The role of placental superficial anastomoses in twin-twin transfusion syndrome remains unclear. Arterioarterial anastomoses may serve a protective role in preventing twin-twin transfusion syndrome. However, after progression to twin-twin transfusion syndrome and requisite laser treatment, recent studies suggest that the presence of an arterioarterial anastomoses may impart a worse prognosis. OBJECTIVE: To examine the association between placental superficial anastomoses and survival after laser surgery for twin-twin transfusion syndrome in a large cohort of patients who prospecti
