The association between foot posture, single leg balance and running biomechanics of the foot.

OBJECTIVES: A mal-aligned foot posture (high-arched and flat feet) and poor single leg balance ability have been separately associated with foot injuries during running. Therefore, clinicians assess these routinely. However, the extent to which foot posture and single-leg balance ability affect actual foot biomechanics during running is not known. This study aims to investigate the association of foot posture, single-leg balance ability, and foot biomechanics during running. METHOD: This is a cross sectional study of sixty-nine participants who had their foot postures and single-leg balance a…

Managing Complications of Foot and Ankle Surgery: Reconstruction of the Progressive Collapsing Foot Deformity.

Our understanding of the cause and principles of treatment of progressive collapsing foot deformity (PCFD) has significantly evolved in recent decades. The goals of treatment remain improvement in symptoms, correction of deformity, maintenance of joint motion, and return of function. Although notable advancements in understanding the deformity have been made, complications still occur and typically result from (1) poor decision making, (2) technical errors, and (3) patient-related conditions. In this article, we discuss common surgical modalities used in the treatment of PCFD and further highl…

Influence of a Foot Insole for a Down Syndrome Patient with a Flat Foot: A Case Study.

Background and Objectives: Patients with Down syndrome have many orthopedic problems including flat foot. Insertion of an insole for a flat foot provides support to the medial longitudinal arch; thus, insole therapy is often used to treat a flat foot. However, the influence of an insole insertion on the knee joint kinematics for a patient with Down syndrome is unknown. This study aimed to elucidate the influence of an insole for a flat foot on the knee kinematics during gait for a patient with Down syndrome. Materials and Methods: The subject was a 22-year-old male with Down syndrome who had a…

Insights into the pathogenesis and treatment of split/hand foot malformation (cleft hand/foot).

Cleft hand or split hand foot malformation is a sequence of phenotypes, from a minor shortening of the central digit to a complete absence of the third ray and in the most severe cases, absence of two, three or four rays. It is a rare but spectacular presentation usually involving both hands and feet. Inheritance is mostly autosomal dominant but sporadic cases without family history are also reported, resulting from a de novo mutation/deletion/duplication. Intra-familial clinical variability is the rule, with incomplete penetrance. X-linked or autosomal recessive inheritance has also been desc…

A shift in priority in diabetic foot care and research: 75% of foot ulcers are preventable.

Diabetic foot ulceration poses a heavy burden on the patient and the healthcare system, but prevention thereof receives little attention. For every euro spent on ulcer prevention, ten are spent on ulcer healing, and for every randomized controlled trial conducted on prevention, ten are conducted on healing. In this article, we argue that a shift in priorities is needed. For the prevention of a first foot ulcer, we need more insight into the effect of interventions and practices already applied globally in many settings. This requires systematic recording of interventions and outcomes, and well…

Freeing the foot: integrating the foot core system into rehabilitation for lower extremity injuries.

The intrinsic muscles of the foot play a critical role in the regulation of absorption and propulsion during dynamic activities. Dysfunction of these may lead to an increased demand on the remaining components within the foot core system to maintain dynamic foot control, leading to a more rapid breakdown of these contributors and those proximal to the foot. Training the intrinsic foot muscles through a systematic progression of isolation via the short foot exercise offers the opportunity to reincorporate their contribution into the foot core system. This article discusses the function of the i…

Foot loading with an ankle-foot orthosis: the accuracy of an integrated physical strain trainer.

PURPOSE: To investigate the value of a built-in physical strain trainer for the monitoring of partial weight bearing with an ankle-foot orthosis. METHODS: 12 healthy volunteers were asked to perform three trials. Plantar peak pressure values from normal gait (trial one) were defined as 100% (baseline). The following trials were performed with the Vacoped® dynamic vacuum ankle orthosis worn in a neutral position with full weight bearing (trial two) and a restriction to 10% body weight (BW) (trial three), as monitored with an integrated physical strain trainer. Peak plantar pressure values were…

The Māori foot exhibits differences in plantar loading and midfoot morphology to the Caucasian foot.

The question being addressed in the current study was whether the diabetic Māori foot was more or less prone to ulceration than the diabetic New Zealand Caucasian (NZC) foot. Harris mat and pedobarographic analyses were employed to assess static and dynamic foot morphology and plantar loading in 40 Māori and NZC diabetic and non-diabetic participants. Significantly higher peak pressures were exhibited by the diabetic Māori participants compared to their NZC peers at the central forefoot. Significantly higher static and dynamic arch index values and significantly higher sub-arch angle values we…

The impact of foot insole on the energy consumption of flat-footed individuals during walking.

BACKGROUND: The human foot contains one of the most variable structures of the body, which is the medial longitudinal arch. Decrease in the height of this arch results in a flat foot. Although there is some evidence regarding the influence of flat foot on gait performance of flat-footed individuals, there is no strong evidence to support the theory that being flat-footed has an effect on energy consumption. Therefore, the aim of this study was to find the relationship between flat foot and energy consumption. METHOD: Two groups of normal and flat-footed participants were recruited in this res…

New Sagittal Plane Reference Parameters for Foot Deformity Correction Planning: The Vitruvian Foot.

Currently available methods for analysis and planning of post-traumatic or congenital deformity correction of the foot have some limitations. The aim of this retrospective study was to establish reference lines and angles (RLAs), and the resulting ratios, based on reproducible anatomic points on sagittal feet radiographs. The key starting point of our evaluation was the previously undescribed length and position of the talus joint line (TJL), from the border of the articular surface of the talus and the posterior process of talus. First, we calculated the relationships between the TJL and the …

Segmental foot and ankle kinematic differences between rectus, planus, and cavus foot types.

The presence of multiple foot types has been used to explain the variability of foot structure observed among healthy adults. These foot types were determined by specific static morphologic features and included rectus (well aligned hindfoot/forefoot), planus (low arched), and cavus (high arched) foot types. Unique biomechanical characteristics of these foot types have been identified but reported differences in segmental foot kinematics among them has been inconsistent due to differences in neutral referencing and evaluation of only select discrete variables. This study used the radiographica…

Australian Diabetes Foot Network: management of diabetes-related foot ulceration - a clinical update.

Appropriate assessment and management of diabetes-related foot ulcers (DRFUs) is essential to reduce amputation risk. Management requires debridement, wound dressing, pressure off-loading, good glycaemic control and potentially antibiotic therapy and vascular intervention. As a minimum, all DRFUs should be managed by a doctor and a podiatrist and/or wound care nurse. Health professionals unable to provide appropriate care for people with DRFUs should promptly refer individuals to professionals with the requisite knowledge and skills. Indicators for immediate referral to an emergency department…

Reliability of the Dynamic Foot Index for Observational Assessment of Foot Motion During Gait.

BACKGROUND AND PURPOSE: Reliable clinical tools for observational assessment of dynamic foot motion during gait are limited. Although three-dimensional motion analysis is considered the reference standard, its cost and complexity restrict routine clinical use. The Dynamic Foot Index (DFI) is a simple, low-cost observational tool designed to assess foot motion during gait. METHODS: This methodological study investigated the intra- and inter-rater reliability of the Dynamic Foot Index (DFI) in healthy adults. Forty-nine participants (98 feet) walked barefoot on a treadmill at a self-selected sp…

[The management of diabetic foot: the new guidelines of the International Working Group on Diabetic Foot].

. The management of diabetic foot: the new guidelines of the International Working Group on Diabetic Foot. The publication of the new guidelines on the diabetic foot are an opportunity for a fine-tuning of the (few) new developments, but of the soundness of the evidence and knowledge known so far. Compared to the previous 2019 guidelines, the 2023 update included the analysis of randomised clinical trials only, a more accurate application of the GRADE method, a leaner and more current bibliography, and an update of the strength of some recommendations from low to conditional. The real big news…
