Open unstable metaphyseo-diaphyseal fractures of the tibia in adolescents: treatment by flexible intramedullary nails augmented by external fixator.

INTRODUCTION: The treatment of open and unstable metaphyseo-diaphyseal fractures of the tibia in adolescents is challenging. It is important to choose a fixation method that can maintain alignment, allow wound care and not violate the growth plate. The aim of this work was to evaluate the efficacy of using flexible intramedullary nails (FIN) augmented by external fixator (EF) in the management of such fractures. PATIENTS AND METHODS: A total of 26 males, with a mean age of 14.08 years and average body weight of 49.8 kg, presented with open metaphyseo-diaphyseal tibial fractures. All cases wer

Complications after plate fixation and elastic stable intramedullary nailing of dislocated midshaft clavicle fractures: a retrospective comparison.

PURPOSE: The incidence of operative treatment of dislocated midshaft clavicle fractures (DMCF) is rising due to unsatisfactory results after non-operative treatment. Knowledge of complications is important for selection of the surgical technique and preoperative patient counselling. The aim of this study is to compare complications after plate fixation and elastic stable intramedullary nailing (ESIN) with a titanium elastic nail (TEN) for DMCF. METHODS: A retrospective analysis of our surgical database was performed. From January 2005 to January 2010, 90 patients with DMCF were treated with p

Locked intramedullary fixation vs plating for displaced and shortened mid-shaft clavicle fractures: a randomized clinical trial.

BACKGROUND: Recent literature supports surgical intervention for shortened, displaced, mid-shaft clavicle fractures. We present the results of a randomized clinical trial comparing locked intramedullary fixation and plate fixation for short, displaced, mid-shaft clavicle fractures. MATERIALS AND METHODS: Local ethical approval was obtained and power analysis and sample size calculations were performed prior to commencement. Patients randomized to 2 groups to be treated with either locked intramedullary fixation or plating. Patients regularly followed up to clinical and radiographic union. The

Open reduction internal fixation of supracondylar fractures above total knee arthroplasties using the intramedullary supracondylar rod.

Seven patients who had low supracondylar fractures above total knee arthroplasties were treated using the intramedullary supracondylar rod. Six of the seven patients were steroid-dependent, long-standing severe polyarticular rheumatoid arthritics with marked osteopenia. The intramedullary supracondylar rod provided stable fixation that allowed early range of motion of the knee. Union occurred in good position in all patients; return to prefracture function was achieved in three months. The surgical procedure was reliable and was associated with minimal morbidity.

[Intramedullary nailing osteosynthesis with epiphyseal fixation of fractures of the proximal humerus in adults. 1 to 3-year follow-up].

The authors used a medullary pinning with epiphyseal fixation in the treatment of superior metaphyseal fractures of the humerus for 41 patients aged 22 to 82 years. There were 28 cases with epiphyseal fractures and 12 with metaphyseal fractures. The follow-up was 1 to 3 years. The pins were introduced through the triceps brachii after perforation of the diaphysis, 1 cm above the ulnar fossa. Results were very good. Strength returned within 8.5 weeks. There was only one case of nonunion of a proximal diaphyseal fracture due to a technical fault (diastasis between fragments). Rehabilitation bega

Treatment of complex proximal humeral fractures with minimally invasive fixation of the humeral head combined with flexible intramedullary wire fixation - introduction of a new treatment concept.

The treatment of three- or 4-part proximal humerus fractures is still a matter of scientific discussion. The following study presents the results of a combined procedure using limited invasive fixation techniques for reconstruction of the humerus head and retrograde intramedullary wiring with elastic nails for the treatment of displaced three- or 4-part fractures. A prospective study of 24 patients with 3-part and 4-part fractures was performed from September 1995 to December 1998. Combined biologic fixation for reconstruction of the humerus head, including intramedullary wiring, was utilized.

Stable Fixation and Immediate Weight-Bearing After Combined Retrograde Intramedullary Nailing and Open Reduction Internal Fixation of Noncomminuted Distal Interprosthetic Femur Fractures.

With an aging population, treatment of interprosthetic femur fractures continues to pose a challenge to the orthopaedic surgeon. Retrograde intramedullary nailing combined with open reduction internal fixation using a tissue-preserving plating technique was used in our series of 9 patients with noncomminuted, distal femur fractures. No interfragmentary screws, cables, cerclage wires, or supplemental bone grafts of any type were used. Each patient initiated weight-bearing as tolerated after operative intervention. Every fracture healed at an average of 20 weeks (range 18-24 weeks). Use of a lat

Salvage After Patient-Driven Delayed Conversion From External Fixation to Intramedullary Nailing in an Open Tibial Shaft Fracture: A Case Report.

BACKGROUND: Open tibial shaft fractures require timely management of both skeletal instability and soft-tissue injury. Temporary external fixation is useful for early stabilization, but prolonged retention without progression to definitive fixation may increase the risk of delayed union and other complications. This case illustrates a patient-driven delay from the preferred staged pathway and subsequent salvage after restoration of mechanical stability and biological support. CASE PRESENTATION: A 40-year-old man sustained a Gustilo-Anderson Type II open fracture of the right tibial shaft with

Plate or intramedullary fixation: A comparative analysis of surgical treatment options for adult both-bone forearm fracture - A retrospective clinical study.

BACKGROUND: Both-bone forearm fractures (BBFx) are common, high-energy injuries that significantly disrupt the anatomical and functional integrity of the forearm. While plate-screw osteosynthesis (PSO) has long been the standard treatment, intramedullary nailing (IMN) and hybrid methods combining both approaches have gained attention due to their potential advantages in minimally invasive procedures and functional outcomes. This study aims to compare the surgical and clinical outcomes of four different treatment strategies for BBFx. METHODS: A retrospective review was conducted on 191 adult p

Recovery of Gait in Children and Adolescents After Pediatric Femoral Shaft Fracture Treated With Intramedullary Nail Fixation: A Longitudinal Prospective Study.

BACKGROUND: Femoral shaft fractures in school-aged children are commonly treated with intramedullary nail fixation. Outcomes such as time to healing, alignment and non-union, leg length discrepancy, and refractures, and other complications are often reported based on radiographic findings. There are limited reports on physical function, including objective quantitative measures. The aim was to study the progress and recovery of gait after femoral shaft fracture in children and adolescents. METHODS: Inclusion criteria were individuals 6 to 16 years of age with a femoral shaft fracture treated

Intramedullary Nail vs. Plate Fixation for Pathological Humeral Shaft Fracture: An Updated Narrative Review and Meta-Analysis of Surgery-Related Factors.

(1) Background: Pathological humeral shaft fracture (PHSF) is a frequently observed clinical manifestation in the later stages of tumor metastasis. Surgical interventions are typically recommended to alleviate pain and restore functionality. Intramedullary nail fixation (INF) or plate fixation (PF) is currently recommended for the treatment of PHSF. However, there is still no standard for optimal surgical treatment. Thus, we conducted a meta-analysis comparing the clinical outcomes of INF with PF for PHSF treatment. (2) Methods: We conducted searches in databases, such as Scopus, EMBASE, and P

Distal Humerus Metaphyseal-Diaphyseal Junctional Fracture of a Four-Year-Old Child Treated With Intramedullary Steinmann Wire Fixation: A Case Report.

Distal humeral metaphyseal-diaphyseal fractures are rare and inherently unstable injuries. Non-operative treatments can make it hard to maintain reduction. Open or closed reduction with percutaneous K-wire fixation may be the preferred treatment option for these fracture types. This case report describes successfully managing a rare distal humerus metaphyseal-diaphyseal junctional (MDJ) fracture in a four-year-old child using intramedullary Steinmann wire fixation. A four-year-old male child applied to the emergency service with a swollen elbow. He had a history of trauma 10 days ago. There wa

Incomplete Diaphyseal Atypical Femoral Fracture due to Increased Anterolateral Bowing: Treatment with Corrective Osteotomy and Intramedullary Nailing with Augmented Plate Fixation.

BACKGROUND: Increased anterolateral bowing of the femur is associated with atypical femoral fractures among older patients (including both those receiving and those not receiving treatment with bisphosphonates) as well as a greater likelihood of occurring in the diaphyseal versus the subtrochanteric region. Prophylactic intramedullary (IM) nailing is recommended for the treatment of incomplete atypical femoral fractures, which are radiographically evident. However, this bowing deformity is considered to be a major obstacle to IM nailing as it may be associated with postoperative complications.

Length of stay and 30-day readmissions after isolated humeral shaft fracture open reduction and internal fixation compared to intramedullary nailing.

INTRODUCTION: Open reduction with internal fixation (ORIF) and intramedullary nailing (IMN) have similar union rates for treating humerus shaft fractures, but IMN leads to increased incidence of shoulder impingement and reoperation. The difference in 30-day readmission rate and length of stay (LOS) between these procedures is unknown. The objective of the study was to compare 30-day unplanned readmissions and length of stay between humeral shaft fractures fixed with either ORIF or IMN. METHODS: The nationwide readmissions database (NRD) was queried for patients with humeral shaft fractures tr

Comparison of outcome of femoral shaft fracture fixation with intramedullary nail in elderly patient and patients younger than 60 years old.

BACKGROUND: Although intramedullary nailing (IMN) is an established and accepted operative treatment for femoral shaft fracture in patients younger than 60, there is a lack of data on the results of this treatment on those over 60. The purpose of this study was to determine if the outcome of IMN for femoral shaft fracture in elderly patients is also acceptable. Particular challenges in this group of patients included osteopenia and other associated multiple medical problems frequently observed. METHODS: The outcome of 84 patients who had IMN for femoral shaft fracture was reviewed and the res
