Memory performance, hippocampal volumes and memory fMRI in frontal versus temporal lobe epilepsy.

PURPOSE: Patients with temporal lobe epilepsy (TLE) and patients with frontal lobe epilepsy (FLE) show an increased risk of memory impairment relative to healthy controls, alongside structural and functional encoding-related cerebral alterations. To better specify syndrome-related differences, we directly compared TLE and FLE patients. METHODS: We evaluated 56 TLE (25 right-sided) and 24 FLE patients with respect to fMRI activation during encoding of scenes, faces, and words, post-scan recognition accuracy, neuropsychological verbal and figural memory tests, and hippocampal volumes. RESULTS:…

Neural correlates of emotion processing and regulation dissociate frontal and temporal lobe epilepsy.

When confronted with negative events, regulating emotions is essential for self-management and mental health. Temporal lobe epilepsy (TLE) patients, particularly right TLE (rTLE) patients, show altered responses towards emotional stimuli. However, neural correlates of emotion regulation and its interplay with stimulus-based emotion processing have not been studied in TLE so far. Therefore, we compared functional MRI activity during up- and downregulation of emotions towards negative scenes among 21 left TLE and 18 rTLE patients, 18 frontal lobe epilepsy (FLE) patients and 17 healthy controls. …

Twenty years of frontal lobe surgery: Clinical outcomes and lessons from the frontier.

BACKGROUND: Frontal lobe epilepsy (FLE) surgery often requires complex presurgical evaluations and intraoperative monitoring, which may not be readily available in resource-limited settings. We report our experience establishing FLE surgical services in Indonesia to develop strategies for a simple but reliable service in low- and middle-income countries. METHODS: We included patients who underwent FLE surgery between 2003 and 2023 in Semarang, Indonesia. These patients underwent surgery with at least 12 months of follow-up. Univariate and multivariate Firth penalized logistic regression analy…

Causal dependencies between frontal and temporal lobe regions underlying word search and retrieval.

Verbal fluency is a behavioral task that requires the generation of words from a semantic category (category fluency) or words beginning with a specific letter (letter fluency). Although word production engages a frontal-temporal-parietal network, no studies have tested how lesions to temporal and parietal lobe areas that represent semantic and phonological knowledge dampen neural responses in the left pars triangularis and the left pars opercularis, two adjacent regions in the left inferior frontal gyrus implicated in word search and retrieval. Here, 52 patients with temporal lobe lesions und

Dissociating decision-making deficits in temporal and frontal lobe epilepsy using Bayesian methods.

PURPOSE: People with epilepsy exhibit various cognitive impairments, including deficits in executive functioning that could impact their decision-making. This study examined differences in objective risk-based decision-making among individuals with focal epilepsy, specifically comparing right temporal lobe epilepsy (RTLE), left temporal lobe epilepsy (LTLE), and frontal lobe epilepsy (FLE) against healthy controls. METHODS: Decision-making was assessed in a case-control study design with 43 adults with pharmacoresistant focal epilepsy (15 RTLE, 18 LTLE, 10 FLE) and 15 controls using the Cambr…

Impact of anterior callosal disconnection on picture naming in frontal lobe epilepsy surgery.

Epilepsy surgery in focal, drug-resistant frontal lobe epilepsy can be curative and resection is aimed at seizure freedom. The cognitive impact of surgery, however, is less clear-cut. On one hand, resection of the epileptogenic zone can disconnect essential brain networks and therefore cause dysfunction. On the other hand, surgery may prompt recovery of normal brain function by restoring normal electrical activity as propagating epileptic discharges affect cognition outside the epileptogenic zone. To understand the impact of surgery on cognitive outcome, we investigated picture naming in 51 pa…

Topographic differences in EEG microstates: distinguishing juvenile myoclonic epilepsy from frontal lobe epilepsy.

UNLABELLED: Pseudomonas aeruginosa (P. aeruginosa) is a critical healthcare challenge due to its ability to cause persistent infections and the acquisition of antibiotic resistance mechanisms. Lack of preventive vaccines and rampant drug resistance phenomenon has rendered patients vulnerable. As new antimicrobials are in the preclinical stages of development, mining for the unexploited drug targets is also crucial. In the present study, we designed a B- and T-cell multi-epitope vaccine against P. aeruginosa using a subtractive proteomics and immunoinformatics approach. A total of five proteins

Parenting Influences on Frontal Lobe Gray Matter and Preterm Toddlers' Problem-Solving Skills.

Children born preterm often face challenges with self-regulation during toddlerhood. This study examined the relationship between prematurity, supportive parent behaviors, frontal lobe gray matter volume (GMV), and emotion regulation (ER) among toddlers during a parent-assisted, increasingly complex problem-solving task, validated for this age range. Data were collected from preterm toddlers (n = 57) ages 15-30 months corrected for prematurity and their primary caregivers. MRI data were collected during toddlers' natural sleep. The sample contained three gestational groups: 22-27 weeks (extrem…

Pork Liver Decomposition Product May Improve Frontal Lobe Function in Humans-Open Trial.

Porcine Liver Decomposition Product (PLDP) was obtained by treating pig liver homogenate with protease and filling it into capsules. We have … rbal memories. Furthermore, increasing scores on verbal fluency tasks suggest that PLDP may enhance frontal lobe function and prevent or improve depressive symptoms. The effects observed in this study may differ from the mechanisms of action of existing antidepressants, and we believe that this may lead to the discovery of new antidepressants.

Frontal lobe closed-lip schizencephaly as an atypical cause of adult-onset seizures.

Schizencephaly arises from irregular neuronal migration leading to clefts development in the cerebral cortex. The condition is often detected in childhood and it is extremely rare to go unnoticed. This case highlights a 45-year-old male with no previous medical history who presented with a seizure for the first time in the emergency department after which an MRI was done and detected schizcenphaly which at this point was the first time for the patient to be diagnosed with this condition. The available data on schizcencphaly in the literature is very little and the data on the condition when di

Association between glymphatic dysfunction and neurocognitive decline in patients with frontal lobe epilepsy.

BACKGROUND: The glymphatic system is essential for the maintenance of brain homeostasis. It may be impaired in patients with epilepsy, but its association with neurocognitive function remains unknown. In this study, we aimed to elucidate the association between changes in the glymphatic system and neurocognitive function in individuals diagnosed with frontal lobe epilepsy (FLE). METHODS: This retrospective case-control research engaged a group of patients with FLE and age-, sex-, and education-matched healthy volunteers. All participants were subjected to extensive neurocognitive assessments,…

Hemodynamic correlates of emotion regulation in frontal lobe epilepsy patients and healthy participants.

The ability to regulate emotions is indispensable for maintaining psychological health. It heavily relies on frontal lobe functions which are disrupted in frontal lobe epilepsy. Accordingly, emotional dysregulation and use of maladaptive emotion regulation strategies have been reported in frontal lobe epilepsy patients. Therefore, it is of clinical and scientific interest to investigate emotion regulation in frontal lobe epilepsy. We studied neural correlates of upregulating and downregulating emotions toward aversive pictures through reappraisal in 18 frontal lobe epilepsy patients and 17 hea…

Frontal lobe microglia, neurodegenerative protein accumulation, and cognitive function in people with HIV.

Microglia are implicated in Alzheimer's Disease (AD) pathogenesis. In a middle-aged cohort enriched for neuroinflammation, we asked whether microgliosis was related to neocortical amyloid beta (A[Formula: see text]) deposition and neuronal phosphorylated tau (p-tau), and whether microgliosis predicted cognition. Frontal lobe tissue from 191 individuals autopsied with detectable (HIV-D) and undetectable (HIV-U) HIV infection, and 63 age-matched controls were examined. Immunohistochemistry (IHC) was used to evaluate A[Formula: see text] plaques and neuronal p-tau, and quantitate microgliosis wit…

Bilateral Orbitofrontal Repetitive Transcranial Magnetic Stimulation in Frontal Lobe Epilepsy: A Case Report.

Epilepsy is a common and severe neurological disorder affecting millions of people worldwide. Nowadays, antiseizure medications (ASMs) are the main treatment for most epilepsy patients, although many of them do not respond to ASMs and suffer from drug-resistant epilepsy (DRE). Alternative and novel treatment methods have been offered nowadays, showing promising results for the treatment of DRE. Repetitive transcranial magnetic stimulation (rTMS) is a noninvasive method that has become increasingly popular in the last decades. This article reports a patient with frontal lobe epilepsy. We aimed …

Successful epilepsy surgery in frontal lobe epilepsy with startle seizures: a SEEG study.

Pre-surgical assessment and surgical management of frontal epilepsy with normal MRI is often challenging. We present a case of a 33-year-old, right-handed, educated male. During childhood, his seizures presented with mandibular myoclonus and no particular trigger. As a young adult, he developed seizures with a startle component, triggered by unexpected noises. During his ictal episodes, he felt fear and grimaced with sudden head flexion and tonic axial posturing. Similar seizures also occurred without startle. Neuropsychological assessment showed executive dysfunction and verbal memory deficit

Incidence of sudden unexpected death in nocturnal frontal lobe epilepsy: a cohort study.

OBJECTIVE: Most cases of sudden unexpected death in epilepsy (SUDEP) follow a seizure, and most deaths occur while people are in bed, presumably sleeping. Nocturnal seizures are reported to be a risk factor for SUDEP. People with nocturnal frontal lobe epilepsy (NFLE) have seizures predominantly or exclusively during sleep, often many times per night. The present study aimed to assess whether NFLE represents a high-risk condition for SUDEP. METHODS: The present study retrospectively assessed the incidence of SUDEP in a cohort reconstructed from a dedicated database of consecutive patients ref…
