Treatment of chronic frontal sinusitis with difficult anatomy: A hybrid balloon technique in four cases.

The presence of frontal cells poses unique challenges when using endoscopic approaches. This study describes the use of a balloon dilation system as an aid for functional endoscopic sinus surgery (FESS) to access the frontal sinus in cases that would traditionally require open approaches. We present a case series of four patients with chronic rhinosinusitis refractive to medical management who underwent FESS with the aid of a balloon dilation system at a tertiary referral center. All patients had variant forms of frontal sinus anatomy. Surgical techniques will be described and use of the ballo

The endoscopic modified Lothrop procedure for salvage of chronic frontal sinusitis after osteoplastic flap failure.

OBJECTIVE: While reported results utilizing the osteoplastic flap procedure are very good, some patients fail the surgery due to recurrent or persistent frontal sinus disease. This study was performed to evaluate the utility of the endoscopic modified Lothrop sinus surgery for osteoplastic flap failure. STUDY DESIGN AND SETTING: A retrospective chart analysis and telephone survey of 10 patients from outside our institution for whom an osteoplastic flap with fat obliteration failed were salvaged using a computerized endoscopic modified Lothrop procedure. RESULTS: The main complaints were head

Do abnormalities of the frontonasal duct cause frontal sinusitis? A CT study in 198 patients.

OBJECTIVE: The purpose of the study was to determine the correlation between frontonasal duct abnormalities (narrowing or obstruction caused by hypertrophic mucosa) and frontal sinusitis. This study was based on the hypothesis that abnormalities of the frontonasal duct cause frontal sinusitis by impairing normal drainage of the sinus. MATERIALS AND METHODS: CT studies of 198 consecutive patients with clinical diagnoses of chronic sinusitis were reviewed retrospectively. Criteria for inclusion were (1) no history of sinus surgery or facial trauma and (2) absence of polyps at rhinoscopy. As 37 …

Draf III Procedure Using Multiple Pedicled Mucosal Flaps for Refractory Frontal Sinusitis: A Case Report.

The Draf III procedure may serve as a surgical option for patients with frontal sinusitis that is refractory to standard treatment or prone to recurrence. However, postoperative restenosis caused by neo-osteogenesis and granulation tissue formation remains a major clinical problem. Coverage of exposed bone using mucosal grafts or pedicled flaps has been advocated to prevent restenosis, but adequate flap reconstruction may be difficult in revision cases with marked bony overgrowth. We report a case of a 62-year-old woman with refractory frontal sinusitis associated with IgG4-related disease and…

Management of frontal sinusitis-associated subperiosteal abscess in children less than 9 years of age.

PURPOSE: To investigate practice patterns for cases of subperiosteal abscess (SPA) with concomitant frontal sinusitis to identify factors favorable to medical management in children <9 years of age. METHODS: The medical records of all pediatric cases of orbital and periorbital cellulitis admitted at a tertiary care center from 1999-2014 were reviewed retrospectively. Cases were included if radiography demonstrated sinusitis-associated SPA in children <9 years of age. Cases of SPA with ipsilateral frontal sinusitis were compared to cases of SPA without ispilateral frontal sinus involvement. R…

Paranasal sinuses in children: size evaluation of maxillary, sphenoid, and frontal sinuses by magnetic resonance imaging and proposal of volume index percentile curves.

Our objective was to establish the age-related 3D size of maxillary, sphenoid, and frontal sinuses. A total of 179 magnetic resonance imaging (MRI) of children under 17 years (76 females, 103 males) were included and sinuses were measured in the three axes. Maxillary sinuses measured at birth (mean+/-standard deviation) 7.3+/-2.7 mm length (or antero-posterior)/4.0+/-0.9 mm height (or cranio-caudal)/2.7+/-0.8 mm width (or transverse). At 16 years old, maxillary sinus measured 38.8+/-3.5 mm/36.3+/-6.2 mm/27.5+/-4.2 mm. Sphenoid sinus pneumatization starts in the third year of life after convers

Computerized Tomography-Based Volumetric Analysis of The Maxillary, Sphenoid, and Frontal Sinuses: An Observational Study.

INTRODUCTION: Volumetric analysis of the paranasal sinuses can help evaluate disease extent, treatment, and sinus surgery outcome. This study aims to determine the baseline volumes of frontal, maxillary, and sphenoid sinuses. METHODS: An observational cross-sectional study was conducted in a tertiary care center after obtaining ethical approval (Reference number: 264/2021). Computerized Tomography Scan head or Computerized Tomography Scan nose/Paranasal sinus images of patients across all age groups from January to December 2022 were analyzed. Sinus volumes were calculated using Digital Imagi

The Development of Frontal and Sphenoid Sinuses After Full-House Endoscopic Sinus Surgery in a Child.

Although endoscopic sinus surgery (ESS) is a standard procedure for adults and is considered a safe and effective procedure in children as well, some uncertainty remains when it comes to children, especially with regard to the future facial and sinus development. Overall, limited ESS does not affect sinus development, and extended ESS does not affect facial features. However, whether or not full-house ESS affects the sinus development, especially the frontal and sphenoid sinuses, remains unclear. The authors herein report a 15-year-old child who received full-house ESS and later re-examined th

Comparison of Frontal Sinuses for Personal Identification in 3 Populations Using Cameriere's Code Number.

The identification of unknown individuals is of crucial importance to society, especially in the context of mass disasters involving individuals of different nationalities. The comparison of frontal sinus patterns is an important tool for personal identification. In Cameriere's method, a new personal code number (codC) containing 8 digits was elaborated, based on the measurements of frontal sinus patterns. The present study is an update of this previous work. Digital radiographic images of the skulls of 299 individuals of different nationalities (100 Italians, 99 Kosovars, and 100 Turks) were

Cephalometric association of mandibular size/length to the surface area and dimensions of the frontal and maxillary sinuses.

OBJECTIVE: This study aimed to determine the cephalometric association of mandibular size/length to the surface area and dimensions of the frontal and maxillary sinuses. MATERIALS AND METHODS: This descriptive study was conducted on 116 digital lateral cephalograms of 38 patients with skeletal Class I malocclusion (normal), 40 patients with skeletal Class II malocclusion with mandibular deficiency, and 38 patients with skeletal Class III malocclusion with mandibular excess. Both male and female patients were included. Using AutoCAD 2016 software, the anteroposterior dimension, height and surf
