Background/Objective: Intraoperative frozen section (FS) has long been used in thyroid surgery; however, its routine usefulness has shrunk with high-resolution ultrasound, standardized cytology, and molecular diagnostics. This narrative review synthesizes >20 years of evidence to clarify where FS still adds clinically meaningful value and where it does not. Methods: This study constitutes a narrative review of the contemporary literature spanning more than two decades, integrating prospective and retrospective evidence on FS performance in indeterminate/suspicious cytology (Bethesda III-V), NI

OBJECTIVE: To determine diagnostic agreement between frozen sectioning (FS) and formalin-fixed, paraffin-embedded (FFPE) tissue-processing methods for veterinary oral tumors and to chronicle the method and time required to perform FS sample processing in a private practice setting.
METHODS: 27 canine and feline oral tumor samples were collected between October 2024 and February 2025 and processed in 2 ways: (1) in-house using FS and (2) conventional FFPE. All biopsies were interpreted by 2 veterinary pathologists, one who read the FFPE samples for normal diagnostic turnaround time, who then b

Background: A primary pull-through for Hirschsprung's disease (HD) requires confirmation of normal ganglionic bowel by intraoperative biopsi … histology (mean clear margin 45.55 mm, range 11-72 mm). Conclusions: In conclusion, intraoperative frozen sections taken from doughnuts of bowel retrieved using 5 mm laparoscopic stapling devices are safe and have resulted in a 0% rate of transition zone pull-throughs while reducing the potential of spillage of enteric contents. We would recommend this protocol …

Frozen sections of uterine smooth muscle tumors are infrequently required, and related diagnostic difficulties are seldom discussed. We analyzed the clinicopathologic features of 112 frozen sections of uterine smooth muscle tumors and determined the accuracy, reasons for deferrals, and causes of interpretational errors. Most patients (median age, 45 y) presented with pelvic mass symptoms (53%). The main reasons for a frozen section examination were an abnormal gross appearance including loss of the usual whorled pattern of leiomyoma (36 cases, 32.1%), and intraoperative discovery of an abnorma…

BACKGROUND: Intraoperative frozen sections (FS) of sentinel lymph nodes (SLN) were evaluated to avoid the need for deferred axillary lymph node dissection (ALND) in patients with early breast cancer (EBC). However, FS has low sensitivity for detecting micro-metastases (<2 mm), resulting in patients who later undergo deferred ALND. The aim of the study was to determine the best clinical approach for selecting patients who would derive real benefit from ALND, as well as to minimize the functional and psychological damage caused by delayed surgery, and the risk of undertreating EBC patients.

BACKGROUND The aim of this study was to compare the clinical outcome in patients with pancreatic ductal adenocarcinoma who underwent frozen section and paraffin section histology of the surgical resection margins during pancreaticoduodenectomy. MATERIAL AND METHODS Frozen section and routine paraffin section histopathology were performed using the following categories: R0 (no tumor cells at the surgical resection margin), R1 (tumor cells at, or within 1 mm, of the surgical resection margin), and R2 (tumor seen macroscopically at the surgical resection margin). R1 and R2 patients underwent addi

Ectopic cervical thymoma (ECT) is a rare tumor that is frequently misdiagnosed on fine needle aspiration cytology and frozen section. Using conventional light microscopy and immunohistochemistry, we characterized the only two cases of ECT found in our institutional files over a period of 20 years. Both tumors were classified as type AB-thymoma. Neoplastic cells expressed cytokeratins but not CD5. Non-neoplastic T-lymphocytes were positive for CD3 and CD5. Lymphocytes expressed CD1a in only one case. One tumor breached the capsule and had positive surgical margins. For this patient, adjuvant ra

OBJECTIVE: Performing targeted biopsy (TB) first, followed by intraoperative frozen section examination (IFSE), and omitting systematic biopsy (SB) when prostate cancer is detected may reduce the number of unnecessary biopsy cores from SB while maintaining an equivalent prostate cancer detection rate. METHODS: Patients scheduled for mpMRI/TRUS fusion biopsy due to a PIRADS score ≥ 4 were prospectively enrolled between October 2023 and November 2025. Patients were randomly assigned to a control group (TB + SB) or an observation group using a random number table. In the observation group, TB was

OBJECTIVES: Salivary gland aspirates classified as salivary gland neoplasm of uncertain malignant potential (SUMP) under the Milan System for Reporting Salivary Gland Cytopathology pose a diagnostic challenge due to their indeterminate cytologic features. We evaluate the utility of intraoperative frozen section in risk stratification and in guiding surgical management in SUMP cases.
METHODS: We retrospectively reviewed 75 SUMP cases from 2019 to 2024 with paired frozen section and final pathology. Frozen section diagnoses, final pathology, and intraoperative decision-making processes were ana

Intraoperative frozen-section (FS) margin assessment is essential for guiding precise resection in perihilar cholangiocarcinoma (pCCA). However, freezing artifacts, superficial sampling, drainage-related reactive epithelial changes, and subtle cytologic atypia in well-differentiated adenocarcinoma can make intraoperative interpretation difficult. We report a 70-year-old woman with Bismuth-Corlette type II pCCA who underwent radical extrahepatic bile duct resection after PTBD-mediated biliary decompression. Intraoperative FS of the distal bile duct margin showed atypical glands of undetermined

INTRODUCTION: While the impact of physical impairments on disability is widely acknowledged, recent literature suggests an association between health-related, social, and quality of life aspects, mental distress and unfavorable outcomes in subjects with Frozen Shoulder (FS).
OBJECTIVE: To identify the main contributors to disability among external rotation (ER0°) and flexion (FL) passive Range of Motion (ROM) restrictions, pain and mental health-related quality of life domains in subjects with FS.
METHODS: The association of the Disability of the Arm, Hand, and Shoulder (DASH) scale (depende

BACKGROUND/AIM: Intraoperative frozen section (FS) pathologic evaluation remains an important part of neurosurgical procedure to ensure proper resection. Follicular thyroid carcinoma (FTC) is a common type of thyroid carcinoma, but metastasis to sellar turcica/pituitary fossa is rare and can be misdiagnosed as pituitary adenoma on FS diagnosis.
CASE REPORT: A 72-year-old female with a history of FTC treated 11 years ago presented to critical care with a pituitary gland lesion. Magnetic resonance imaging (MRI) revealed a 2.8 cm pituitary macroadenoma with sellar expansion and upward displaceme

BACKGROUND: Crystal-storing histiocytosis (CSH) is a rare disorder which most commonly occurs in the setting of concurrent lymphoproliferative disease. Morphologically, it consists of aggregates of histiocytes containing eosinophilic crystalline material, which in most cases is composed of aggregated abnormal light chains.
METHODS: Using histomorphology, immunohistochemistry and in situ hybridization, the authors characterize a rare case of orbital CSH associated with extranodal marginal zone (MALT) lymphoma and report for the first time the frozen section features of CSH.
RESULTS: The froze

BACKGROUND: The growing use of postmastectomy radiation/regional nodal irradiation (PMRT) has resulted in many women receiving both axillary dissection (ALND) and PMRT, increasing lymphedema rates. After developing standardized PMRT criteria, we adopted a policy of ALND omission among cN0 patients with 1-2 positive sentinel nodes (+SLNs) requiring PMRT. We evaluated how often overtreatment with ALND+PMRT was avoided with this approach.
METHODS: A retrospective review of a prospectively maintained database was performed beginning 1 year before policy adoption. Intraoperative SLN evaluation was

INTRODUCTION: Histological examination during surgery (FS) has a place in the surgical management of differentiated thyroid carcinoma (DTC). Extending the indication for limited surgery to 4 cm tumor size (ATA guidelines 2015) cytologically verified DTCs, increases the emphasis on accurate patient selection. In our work, we reflected on the effectiveness of FS and its relationship to optimal patient management.
METHODS: In a single-center retrospective study, we evaluated the documentation of patients indicated for primary surgery for DTC from January 1, 2016 to December 31, 2020 - there was

BACKGROUND: The assessment of myometrial invasion is a pivotal step in the preoperative staging of endometrial cancer. Intraoperative frozen section (FS) represents a reliable tool in directing surgeon's choices. Preoperative transvaginal ultrasound (US) showed high accuracy in evaluating myometrial invasion. This study aimed to understand if the application of a standardized ultrasonographic protocol for the pre-operative evaluation of myometrial invasion can help pathologists in improving the accuracy of FS. Furthermore, the agreement between US and FS in the assessment of myometrial invasio

BACKGROUND: Surgeons sometimes have difficulty determining which result to favor when preoperative results (MRI + preoperative endometrial biopsy [pre-op EB]) differ from intraoperative frozen section histology (FS) results. Investigation of how FS can complement ordinary preoperative examinations like MRI and pre-op EB in identification of patients at high risk of lymph node metastasis (high-risk patients) could provide clarity on this issue. Therefore, the aim of this study is to assess the utility of pre-op EB, MRI and FS results and determine how to combine these results in identification

The purpose is to report the United Kingdom's largest single-centre experience of robotically assisted laparoscopic radical prostatectomies (RALP), using the neurovascular structure-adjacent frozen-section (NeuroSAFE) technique. We describe the utilisation and outcomes of this technique. This is a retrospective study from 2012 to 2019 on 520 patients undergoing NeuroSAFE RALP at our Institution. Our Institution's database was analysed for false-positive frozen-section (FS) margins as confirmed on paraffin histopathological analysis: functional outcomes of potency, continence, and biochemical r
