Spatiotemporal dynamics of leptospirosis in Europe: a retrospective observational study with prospective projections.

BACKGROUND: Leptospirosis is a widespread zoonotic disease caused by Leptospira spp., with a substantial burden on both human and animal health. Transmission occurs through contact with the urine of infected animals, with rodents serving as the main disease reservoir. Climate and environmental changes, including higher temperatures, heavy rainfall, urbanization, forest encroachment, and biodiversity loss are expected to alter rodent populations and disease dynamics, raising concerns about the increasing risk of leptospirosis in Europe. Despite this, no comprehensive risk map or modelling study

A systematic review of the cost-effectiveness of anti-VEGF drugs for the treatment of diabetic retinopathy.

BACKGROUND: Non-proliferative and proliferative diabetic retinopathy are common complications of diabetes and a major cause of sight loss. Anti-vascular endothelial growth factor drugs represent a treatment option for people with diabetic retinopathy and are routinely used to treat various other eye conditions. However, anti-vascular endothelial growth factor drugs are expensive relative to current care options, and it is unclear whether this additional cost is justified when the immediate risk of vision loss is lower compared to patients with more aggressive ophthalmological conditions. Household costs in the United States for accommodating functional impairments associated with Duchenne muscular dystrophy: results from a caregiver survey.

BACKGROUND: Individuals with Duchenne muscular dystrophy (DMD) often require mobility aids and accessible homes and vehicles. Because these accommodations are not typically reimbursed by insurance, costs largely fall upon patients' families. Additionally, families often incur high out-of-pocket costs for DMD-related health services and treatments. Research describing these expenses is important to understand the full financial cost associated with DMD. The objective of this study was to quantify household costs associated with DMD-related accommodations. METHODS: A cross-sectional, retrospect

Resident experience of new Low Traffic Neighbourhoods in London: qualitative insights from a mixed methods study.

BACKGROUND: Reducing private car use and increasing active travel is essential for transport decarbonisation and addressing public health crises of road traffic injuries, physical inactivity and air pollution. Low Traffic Neighbourhoods have emerged as a key intervention, particularly in London, United Kingdom, to create better environments for walking, wheeling and cycling by restricting through traffic on residential streets. While evidence suggests that Low Traffic Neighbourhoods reduce car use, increase walking and cycling, and improve road safety, their implementation has been politically

Relationship between anxiety, post-traumatic stress, insomnia and fibromyalgia among female refugees in jordan: A cross-sectional study.

UNLABELLED: WHAT IS KNOWN ON THE SUBJECT?: Fibromyalgia (FM) is a syndrome of chronic widespread pain, typically associated with fatigue, sleep, cognitive dysfunction and disordered mood. FM may limit an individual's ability to participate in everyday work and social activities, thereby making it difficult to maintain normal relationships with other individuals. While it has been studied in different populations and settings, the impact of FM and associated psychological factors has not been previously studied among female war refugees. WHAT DOES THE PAPER ADD TO EXISTING KNOWLEDGE?: The study

Survey of Clinical Translation of Cancer Nanomedicines-Lessons Learned from Successes and Failures.

In 1995, the year the first cancer nanomedicine, Doxil, was approved by the Food and Drug Administration (FDA), only 23 manuscripts appeared in a PubMed search for "nanoparticles for cancer" keywords. Now, over 25 000 manuscripts can be found using those same keywords, yet only 15 nanoparticle-based cancer nanomedicines are approved globally. Based on the clinicaltrials.gov database, a total of 75 cancer nanomedicines are under clinical investigation involving 190 clinical trials summarized here. In this Account, we focus on cancer nanomedicines that have been approved or reached clinical tria

A multi-country study to co-design and evaluate digital educational resources to support conversations about ending fertility treatment.

STUDY QUESTION: How can educational resources be feasibly co-designed and used to support conversations between staff and patients about ending fertility treatment? SUMMARY ANSWER: Co-design workshops allow for the development of educational resources that account for all stakeholders' perspectives and are considered sensitive, informative, and helpful to support end-of-treatment conversations, but staff and patients have different views about how these can be used within the treatment pathway. WHAT IS KNOWN ALREADY: Ending treatment without children is a common outcome but seldom discussed

Post-hoc analysis of vitamin D status and reduced risk of preterm birth in two vitamin D pregnancy cohorts compared with South Carolina March of Dimes 2009-2011 rates.

BACKGROUND: Two vitamin D pregnancy supplementation trials were recently undertaken in South Carolina: The NICHD (n=346) and Thrasher Research Fund (TRF, n=163) studies. The findings suggest increased dosages of supplemental vitamin D were associated with improved health outcomes of both mother and newborn, including risk of preterm birth (<37 weeks gestation). How that risk was associated with 25(OH)D serum concentration, a better indicator of vitamin D status than dosage, by race/ethnic group and the potential impact in the community was not previously explored. While a recent IOM report sug

Professionalisation or Credentialism? Fifteen-Year Enrolment Trends in the UK Postgraduate Certificates in Medical Education.

BACKGROUND: Postgraduate qualifications for clinical educators have proliferated internationally, raising questions about whether this reflects professionalisation or a credentialist response to competitive specialty selection. Postgraduate Certificates in Medical Education (PGCMEs) are established in the United Kingdom (UK), yet no published data describe enrolment over time. This research determined PGCME enrolment trends in the UK universities from 2010 to 2024 and interpreted them against this framing. METHODS: Freedom of Information requests were sent to all the UK public universities of

Nasal RNA-scaffold-protein vaccine protects mice from human H5N1 clade 2.3.4.4b virus lethal infection and safeguards against vaccine-unmatched viruses.

BACKGROUND: Recent cases of H5N1 avian influenza spilling over into human populations have raised concerns once again about the unpredictable emergence of zoonotic viral diseases. This phenomenon highlights the persistent threat posed by viruses that jump from animals to humans, underscoring the need for preparedness for antigens that humans have not encountered. Nasal vaccines, which can effectively block transmission at the site of viral entry, represent a promising strategy and should be prioritised for stockpiling to halt zoonotic spillover. METHODS: Here, we immunised a preclinical anima

Digital technologies for health financing in low-income and middle-income countries: a scoping review protocol.

INTRODUCTION: Universal health coverage (UHC) is a global priority, ensuring equitable access to quality healthcare services without financial hardship. Many countries face challenges in progressing towards UHC. Health financing is pivotal for advancing UHC by raising revenues, enabling risk-sharing through pooling of funds and allocating resources. Digital technologies in the healthcare sector offer promising opportunities for health systems. In low-income and middle-income countries (LMICs), digital technologies for health financing (DTHF) have gained traction, supporting these three main fu

An integrated action plan to fund and support drug development for Dup15q syndrome: a patient organization perspective.

Maternal 15q11.2-13.1 duplication syndrome, or Dup15q syndrome (Dup15q), is a rare neurodevelopmental disorder affecting as many as 1 in 5000 to 1 in 20,000 children worldwide. Autism and seizures are two of the most commonly observed phenotypes in Dup15q, with intellectual disability, hypotonia, gastrointestinal distress, and substantial fine and gross motor deficits also commonly reported. The community that is now known as the Dup15q Alliance started in 1994 as a small group of families raising children with chromosome 15q duplications. Originally named IsoDicentric 15 Exchange, Advocacy an

Tackling challenges in rare diseases: The ISGACA approach on non-pancreatic cancers in the periampullary region.

OBJECTIVE: Rare but aggressive cancer types like non-pancreatic periampullary cancers pose unique challenges for cancer research due to their low incidence rates and lack of consensus on optimal treatment strategies, therefore necessitating a collaborative approach. The International Study Group on non-pancreatic peri-Ampullary CAncer (ISGACA) aimed to build a collaborative initiative to pool expertise, funding opportunities, and data from over 60 medical centers, in order to improve outcomes for underrepresented patients with rare cancers. METHODS: The ISGACA approach predefined a stepwise a

Importance of occupational support for NHS patients with mental illness.

BACKGROUND: Unemployment is a structural inequality which raises the risk of premature deaths among people with mental illness. AIMS: This study examined whether UK National Health Service (NHS) patients with mental illness get support to find or keep a job because reducing unemployment rates can reduce the risk of premature mortality. METHODS: This study analysed recently released data from 54 NHS trusts which randomly sampled patients for a Care Quality Commission survey. This study assessed 11 001 working-age patients with mental illness, of whom 50% are long-term service users (6+ years)

Autologous Blood Injections in Temporomandibular Hypermobility: A Systematic Review.

The injection of autologous blood (AB) is one of the methods of treatment of recurrent dislocations in the temporomandibular joints (TMJs). Due to the low invasiveness of this technique, it is reasonable to evaluate it in accordance with the standards of evidence-based medicine. The purpose of this systematic review is to identify primary studies on AB injection for the treatment of TMJ hypermobility and assess the therapy for effectiveness. This systematic review was conducted in accordance with the current "Preferred Reporting Items for Systematic Reviews and Meta-Analyses" guidelines. Contr

HIV Digital Vaccine Strategy: Proposal for Applying Blockchain in Preventing the Spread of HIV.

BACKGROUND: The HIV epidemic imposes a heavy burden on societal development. Protection of susceptible populations is the most feasible method for eliminating the spread of HIV. In the absence of a biological vaccine, the definitive solution is enabling susceptible populations to recognize and avoid high-risk sexual behavior. OBJECTIVE: The objective of this study is to use specific technologies and strategies to establish a system by which high-HIV-risk individuals can determine the HIV infection status of one another anonymously, conveniently, and credibly. METHODS: This study proposes an
