Researchers generally agree that Geriatric Evaluation and Management (GEM) Units are effective only when they are targeted at a specific group of frail, elderly patients who are most likely to benefit. Such patients are those who are neither too sick (eg, severely demented or moribund) nor too well. Various strategies for identifying such patients have been employed by investigators with little consensus on the most efficient targeting criteria. Criteria most often use for inclusion in GEM programs are various combinations of patient age, degree of functional impairment, presence of geriatric

BACKGROUND: An increasing number of individuals aged 60 and older will rely on healthcare services, particularly due to increased physical limitations and mobility restrictions. This implies an increase in permanently immobile, often bedridden people who require targeted care. Mobility impairments progress gradually, leading to loss of autonomy and self-care skills, physical decline, functional deterioration, disability, etc. This review synthesises research findings on the implications of becoming and being bedridden for nursing care of older people in long-term care.
OBJECTIVES: The aim of

Irritant diaper dermatitis (IDD) is frequently observed in young children but can affect individuals throughout their lifespan. The broader term incontinence-associated dermatitis (IAD) is used to describe inflammatory skin reactions from prolonged exposure to urine and/or feces. This review compares IAD and IDD in pediatric and geriatric populations, focusing on their pathogenesis, clinical considerations, and management. Both conditions involve prolonged bodily fluid exposure disrupting the skin barrier, making infants and older adults particularly susceptible due to their age-related skin c

The coronavirus disease 2019 (COVID-19) pandemic placed new strains on informal caregivers, who are already vulnerable to negative psychosocial effects due to demands of the caregiving role. The current study aimed to explore the early impact of COVID-19 on caregivers living with and apart from care recipients. Semi-structured qualitative interviews with seven cohabitating and 10 distanced caregivers of patients in a home-based primary care program were conducted from April to November 2020. A framework matrix was used to identify patterns in caregiver experiences. Cohabitating and distanced c

Postoperative delirium is a serious complication, particularly in older adults with hip fractures. Using the 2019 American College of Surgeons National Surgical Quality Improvement Program data file, we performed multiple regression analyses to compare risk factors for postoperative delirium in hip fracture patients with and without dementia. Preoperative delirium and mobility aid use were common risk factors in both groups. However, differential effects were observed for other factors. Pathological fracture increased delirium risk in patients with dementia but had a protective effect in those

Risk factors that contribute to pressure ulcer formation include immobility and inactivity, age, malnutrition, fecal and urinary incontinence, and a decreased level of consciousness. Gerontological nurses should collaborate with other health disciplines (eg, the physician, physical therapist, and dietitian) to plan and provide preventive treatment measures for the elderly person with potential for impaired skin integrity: pressure ulcers. Gerontological nurses must continue the development and refinement of tools with which to predict the degree of risk for impaired skin integrity, as well as

The incidence of gambling among older adults has risen significantly over the last decade. Pathological and problem gambling disorders among older adults have devastating consequences including stress, alcohol abuse, loss of income and assets, treatment nonadherence, malnutrition, safety risks related to associated alcohol usage and financial losses, and increased psychiatric problems such as depression. This article reviews the theoretical frameworks and literature concerning gambling incidence, prevalence, behavior, diagnostic tools and interventional strategies for older adults, as well as

This cross-sectional study investigated the relationship between apathy and quality of life (QOL) in nursing home residents (n = 227). In all, 92 residents could be assessed with the Mini Mental State Examination (MMSE), the Geriatric Depression Scale (GDS) and the Philadelphia Geriatric Centre Morale Scale (PGCMS), and were able to answer a question about overall subjective QOL. Apathetic behaviour and consciousness disorders were measured with the Behaviour Rating Scale for Psychogeriatric Inpatients (GIP). Linear regression analysis was first applied to study the association of cognition, d

INTRODUCTION: The life expectancy has significantly increased in the last 100 years. Therefore it has to be taken into account that many patients who come to the dentist are over 60. In this group of patients, you should pay attention to geriatric aspects of medical history and treatment plan. Nowadays as well as in the past, teeth loss correlates with the age especially thus with the branch of dentistry which is prosthetics. The aim of this study was to define: 1) the condition of the masticatory system of people over 60 who live with their families or at the nursing home; 2) the assessment o

Background/Objectives: Despite its benefits for both nurses and patients, entrepreneurship among nurses remains uncommon. Specifically, impact entrepreneurship has rarely been applied in nursing. Impact entrepreneurship promotes solutions to health, social, and environmental challenges using business models committed to measurable social impact and return on investment. Beginning in 2020, an impact entrepreneurship course was introduced as a mandatory component of the master's and nurse practitioner programs in geriatrics and palliative care at an Israeli college. This article examines the per

BACKGROUND: Aging populations, multimorbidity, and rapid medical progress have expanded treatment options in old age, but also intensified ethical and organizational challenges in everyday care. Little is known about how nurses, as key frontline professionals, experience these structural developments specifically in the context of medical and nursing care for older adults in Germany. Therefore, this study aimed to examine how nursing staff perceive structural conditions in the German healthcare system and their effects on medical and nursing care for older patients.
METHODS: This study employ

BACKGROUND: Delirium is a frequent and serious neuropsychiatric syndrome in older adults. People living with dementia are at particularly high risk of developing delirium. In nursing homes, where dementia prevalence is high, delirium superimposed on dementia (DSD) represents a major clinical challenge and research gap. Aim of the study is to produce reliable data on prevalence and incidence of DSD in nursing homes as well as to investigate risk factors, clinical course, physical activity patterns, and care processes.
METHODS: This prospective multicenter cohort study will be conducted between

The objective of this study was to determine patterns among geriatric practitioners in prescribing agents that protect the gastrointestinal tract when nonsteroidal anti-inflammatory drug (NSAID) treatment is started for elderly patients. A questionnaire describing five scenarios of elderly patients requiring NSAID therapy asked respondents to choose gastrointestinal-protective agents for each scenario. Respondents were then asked to what extent four established risk factors for NSAID gastropathy (age, previous peptic ulcer, previous gastrointestinal bleeding, and heart disease) affected their
