Pelvis/acetabulum: management of geriatric injuries.

Geriatric pelvic and acetabular fractures pose significant challenges due to patient frailty, comorbidities, and the complexity of fracture patterns. This review examines current evidence and evolving strategies for managing these injuries. Treatment approaches range from nonoperative management to surgical interventions, including percutaneous fixation, open reduction and internal fixation (ORIF), and total hip arthroplasty, either as a standalone procedure or combined with ORIF. Decision making is guided by fracture morphology, patient functionality, and physiological reserve to optimize cli

[Management of ankle fractures in the geriatric population].

Ankle fractures in geriatric patients are common and challenging due to comorbidities and the risk of functional decline. Management must ensure fracture stability while promoting early mobilization to prevent complications and maintain independence. Open reduction and internal fixation remains the standard for unstable fractures; however, alternative techniques such as fibular intramedullary nail or tibiotalocalcaneal nail are suitable for fragile patients, as they reduce wound complications and permit early weight-bearing. Conservative treatment is appropriate for stable fractures or patient

Geriatric Assessment in Acute Myeloid Leukemia.

BACKGROUND: Acute myeloid leukemia (AML) is a heterogenous disease that affects mostly older adults with varying baseline health and functional status. Treatment options have expanded for older adults, ranging from less intensive chronic therapies to intensive induction strategies with curative intent. Despite this, outcomes remain poor with advancing age due to underlying disease biology and variability in treatment tolerance. Reliance on chronological age alone, however, increases risks of both over- and under-treatment. Strategies to better characterize fitness in the context of therapy are

Pneumatic Dilation in Geriatric Achalasia Patients.

BACKGROUND: The aim of the study was to share the effectiveness of pneumatic dilation in geriatric achalasia patients. METHODS: Achalasia patients over the age of 65 and those under the age of 65 as the control group who received pneumatic dilation as the first-line treatment were evaluated in the study. RESULTS: The average age of geriatric patients was 72.5 ± 55.92 years (65-90), with 50.3% of them being male. Follow-up was conducted for a mean of 64.52 ± 38.73 months. While pneumatic dilation was successful in 98.6% (141/143) of geriatric patients, it was also successful in 94% (141/150)

Missed Opportunities in Geriatric Oncology Research.

The field of geriatric oncology has made significant progress in recent decades, but there are still missed opportunities in important areas of research. One issue is the underrepresentation of older patients, especially those aged 75 years and older, in clinical trials. This has resulted in a lack of high-quality data for the care of this population, and the American Society of Clinical Oncology has called for an increase in the evidence base for older patients with cancer. The second missed opportunity is the chance to gather important knowledge from older patients participating in clinical

In 2014, the European undergraduate curriculum in Geriatric Medicine was published to cover the minimum requirements that a medical student … of their specialty-are inevitably involved in the care of older patients, but for most physicians, geriatrics is not part of their postgraduate generalist or specialty training. A key area for postgraduate education remains the provision of Geriatric Medicine competencies to all specialties outside geriatrics. There is also need for wider educational initia …

Clinical and hematological evaluation of geriatric anemia.

BACKGROUND: Anemia in the elderly is a cause of concern. It is not merely physiological due to aging and requires appropriate evaluation. Anemia has a significant negative impact on cardiac function, cognition, sleep, frequent hospitalization, mobility, morbidity, and mortality. Anemia in the elderly is attributable to many causes: nutrient deficiencies, chronic inflammatory diseases, thyroid disorders, diabetes mellitus, gastrointestinal (GI) tumors and bleeding, chemotherapy-induced anemia, and drug-induced hemolysis. OBJECTIVES: We aimed to evaluate the clinical and hematological profile o

[Sarcopenia: a geriatric syndrome that impacts urology].

With increasing age, people lose muscle strength and muscle mass. Acute illnesses, surgical interventions or chronic illnesses can quickly lead to immobility and even more muscle loss. For the elderly, there is a risk of reduced functionality and everyday skills. This syndrome, referred to sarcopenia, is characterized by a progressive and generalized loss of muscle mass and strength with negative consequences such as functional impairment and frailty. In the case of serious illnesses or surgical interventions, this is particularly noticeable in prolonged convalescence. Sarcopenia is associated

Comprehensive Geriatric Assessment in Cardiovascular Disease.

Frailty and cardiovascular disease (CVD) are both highly prevalent in older adults. Cardiovascular disease has been identified as the most frequent cause of death, while frailty has been identified as one of geriatric giants characterized by decreased physiological reserves and increased vulnerability. However, the exact pathobiological links between the two conditions have not been fully elucidated. Consequently, we observe a relevant difficulty not only in accurately defining cardiovascular risk in vulnerable elderly patients (and the other way around), but also a lack of consensus regarding

Impact of Hearing Loss on Geriatric Assessment.

BACKGROUND: Due to the aging society, the incidence of age-related hearing loss (ARHL) is strongly increasing. Hearing loss has a high impact on various aspects of life and may lead to social isolation, depression, loss of gain control, frailty and even mental decline. Comorbidity of cognitive and sensory impairment is not rare. This might have an impact on diagnostics and treatment in the geriatric setting. OBJECTIVE: The aim of the study was to evaluate the impact of hearing impairment on geriatric assessment and cognitive testing routinely done in geriatrics. MATERIAL AND METHODS: This re…

Osteoprotegerin as a biomarker of geriatric frailty syndrome.

The lack of a univocal definition of frailty, a condition frequently found in the elderly population which is correlated with an increased risk of mortality, has prompted the search for clinical and laboratory parameters associated with this condition. Whereas OPG is a protein involved in different pathophysiological conditions including bone, vascular, immune and tumor disease and studies found a positive linear correlation between OPG and age we hypothesized that it may represent a frailty marker in the elderly. We conducted an observational study of 172 elderly subjects, with and without hip

Paradigm shift in geriatric fracture treatment.

The number of geriatric patients is increasing. These patients exhibit specific characteristics, which influence the type of fracture care. Many patients have comorbidities, which make them more vulnerable to surgical procedures. The soft tissue envelope around the fracture often is compromised due to pre-existing diseases such as diabetes, chronic venous insufficiency or peripheral vascular disease. Bone mineral density has decreased, which enhances the risk of implant loosening. The goals of treatment differ from those, which are valid for younger adults. Primary goal is preserving independe
