The history of polymyalgia rheumatica/giant cell arteritis.

Only two years separate the initial descriptions of polymyalgia rheumatica (PMR) by Bruce in 1888 and giant cell arteritis (GCA) by Hutchinson in 1890. However, the existence of an association between these two conditions was definitely accepted only in 1964. Even if PMR is generally accepted as a different disease from GCA, some authors deem PMR to be a manifestation of a generalised arteritis and use the term GCA to define PMR/GCA as a whole. The time has come to clarify this issue.

Giant-cell arteritis presenting as an orbital pseudotumor.

In this report a woman with giant-cell arteritis presenting as an orbital pseudotumor is described. Other causes of orbital pseudotumor were excluded in this patient. The pathogenesis of orbital pseudotumor caused by giant-cell arteritis is discussed. The concurrent arteritis of the superficial temporal artery and the rapid resolution of signs and symptoms after prednisone therapy supports the causal relationship between orbital pseudotumor and giant-cell arteritis in this patient.

[Intestinal infarct caused by giant cell arteritis].

Arteritis of giant cells compromising extracranial and particularly intestinal tissues is not frequent. Therefore, it is common practice to make the diagnosis retrospectively after analyzing the surgical sample. A case is presented of an 83 year old woman admitted to the Clinical Department with a clinical course of 3 days of evolution characterized by fever and pain in the left hemiabdomen. Her personal medical history included multiple diverticulosis of colon, collecistectomy and appendicectomy. Laboratory tests showed that uremia was 0.75 g/L (N.L to 0.45 g/L), V.E.S. 90 mm at the first hou…

Supratentorial giant cell ependymoma: a case report.

Ependymomas are neoplasms of the central nervous system that are capable of demonstrating remarkably heterogeneous histologic features. Thes … onsistent with an anaplastic ependymoma. Unique to this neoplasm was the presence of multiple tumor giant cells. The presence of pleomorphic tumor giant cells is a characteristic feature of the subependymal giant cell astrocytoma, and it is also commonly seen in pleomorphic xanthoastrocytoma and glioblastoma multiforme. Bizarre giant cells were recently descr …

Excision curettage and allografting of giant cell tumor.

Between 1987 and 1994 we followed 22 patients with giant cell tumors involving the long bones. Their average age was 31 years (range 17-50 years). Five patients had grade II tumors and the other 17 grade III lesions. The average volume of lesions after curettage was 231 ml (range 56-450 ml). All of the patients underwent a modified excisional curettage, and the cavity was filled with deep-frozen allogenic corticocancellous bone graft with supplementary fixation. Two patients developed postoperative complications including a superficial wound infection in one case and a traumatic tibial plateau

Postural vision loss in giant cell arteritis.

Two patients with biopsy-proven giant cell arteritis experienced bilateral transient vision loss after bending over and after getting up from a supine position. One patient had no demonstrable signs of carotid, ophthalmic, or anterior ciliary vascular disease, suggesting that his episodes of transient vision loss were due to vertebrobasilar insufficiency. The other patient experienced bilateral postural vision loss in the context of impending bilateral anterior ischemic optic neuropathy. Bilateral transient postural vision loss is an uncommon manifestation of giant cell arteritis that could re

Occult giant cell arteritis: ocular manifestations.

PURPOSE: To report the incidence, visual symptoms, and ocular signs of occult giant cell arteritis in patients who initially presented with visual symptoms and ocular signs of giant cell arteritis. Occult giant cell arteritis was defined as ocular involvement by giant cell arteritis without any systemic symptoms and signs of giant cell arteritis. METHODS: In a prospective study from 1973 to 1995, we investigated 85 patients who had ocular involvement caused by giant cell arteritis and whose diagnosis of giant cell arteritis was confirmed on temporal artery biopsy. At the initial visit, patien

Giant cell tumor of the spine. Surgical findings.

The authors report 5 cases of giant cell tumor surgically treated. After a mean follow-up of 7 years posterior stabilization alone in the latent form, and wide intralesional curettage in the four aggressive forms allowed for suitable treatment of the lesion, with remission of symptoms leaving stability of the spine intact. Radiation therapy was used successfully in only 1 case of local recurrence occurring 4 months after surgical treatment. Unlike localizations of giant cell tumor in other skeletal segments, whether or not margins are left intact does not seem to influence clinical results.

[Differential diagnosis of giant cell tumor of bone].

Osteoclast-like giant cells (GC) may dominate the histologic pattern not only in conventional giant-cell tumor (GCT)--originating as a radiologically pure lytic, possibly trabeculated lesion especially within the epiphyses of long tubular bones (LB) and pelvic bones of adults--but also in many tumor-like lesions as well as in various benign and malignant bone tumors which may simulate each other. Although the mononuclear cells as well as the amount of collagene fibres don't differ significantly, these lesions can be distinguished by substantial differences concerning their site, radiomorpholog…

Giant cell tumor of the temporal bone (osteoclastoma).

A 49-year-old white female patient was referred to the ENT Department, Zentralkrankenhaus, Bremen, because of conductive hearing loss. Widespread tumor formation was found in the left temporal bone and was identified by biopsy to be an osteoclastoma. The tumor was extirpated surgically and reconstruction was performed. Periodic clinical follow-ups during a 36-month period after surgery have included control computed tomographic scans and have shown no evidence for recurrent disease. The specific pathology of the osteoclastoma is presented and different ways for treatment are discussed.

Orbital myositis, vitiligo, and giant cell myocarditis.

Giant cell myocarditis is a rare idiopathic inflammatory heart disease characterized histologically by multinucleated giant cells, and clinically by rapid progressive heart failure, arrhythmias, or sudden death, often within hours to days of initial symptoms. There are two previously reported cases of giant cell myocarditis with idiopathic orbital myositis. We report a similar case in a patient who also had vitiligo, a diagnostic endomyocardial biopsy, and survival because of a cardiac transplant. Giant cell myocarditis should be monitored for in the course of inflammatory orbital myopathy bec…

Giant cell tumor of bone in children and adolescents.

Giant cell tumor of bone rarely affects children, in whom it is usually located in a metaphysis in contrast to the predominantly epiphyseal localization in adults. Five cases are reported, two at the femur, two at the fibula and one at the tibia. Plain film radiography and computed tomography are the most informative imaging studies. The differential diagnosis is with aneurysmal bone cyst and, in metaphyseal-epiphyseal forms, with chondroblastoma. Treatment usually consists in curettage of the tumor followed by filling of the cavity; however, more extensive resection is required in some cases.

[Giant cell leiomyosarcoma of the spermatic cord: a report].

Presentation of the case of one patient with a leiomyosarcoma of the spermatic cord with presence of numerous giant cells, a remarkably unusual finding. Presence of this type of cells, as well as their positivity for histiocytic markers in immunohistochemical staining, raise considerable problems for the differential diagnosis of this neoplasia with malignant fibrous histiocytomas, which probably requires a different, more aggressive therapeutical approach.

[Chromosome study on giant cell tumor of bone].

Chromosome study was made in 30 specimens of giant cell tumor of bone (GCT) and osteosarcoma (OS). In 15 tumors, karyotypes were satisfactorily made for chromosome analysis, including 13 GCTs, one OS and one cell-line of same. The karyotypes of all cells were recognized and described according to the international system for human cytogenetic nomenclature. Karyotype analysis showed that GCT differs from OS both in numerical chromosomal aberrations and in structural chromosomal aberrations. The number of chromosome of GCT concentrated on diploid or subdiploid, with the exception of some superdi

Treatment of corticosteroid-resistant giant cell arteritis.

The literature about the treatment of giant cell arteritis (GCA) is diverse and often includes patients with polymyalgia rheumatica (PMR) who do not have concurrent features of GCA. Consequent heterogeneity has contributed to controversy in the analysis of clinical data. Nevertheless, we have critically reviewed this literature to derive a rational approach to initial and maintenance corticosteroid (CS) therapy and thus define "CS-resistant GCA." In this article, the authors review what has been written about the treatment of presumed CS-resistant disease. Although firm recommendations are lac
