Periodontal and Restorative Treatment of Gingival Recession Associated with Non-Carious Cervical Lesions: Case Study.

The association between the presence of gingival recession and non-carious cervical lesions is a common finding in dentistry. These diseases have multifactorial etiology and the treatment should be multidisciplinary. Although traditionally the majority of professionals treat non-carious cervical lesions only with conventional restorative procedures, in most cases a combination of periodontal and restorative treatments provides the best functional and esthetic results. Thus, the objective of this case report was to present a new option for treatment, which consists of a subepithelial connective…

Vascularization after treatment of gingival recession defects with platelet-rich fibrin or connective tissue graft.

OBJECTIVE: The aim of this study was to evaluate histologically the following treatment of bilateral localized gingival recessions with coronally advanced flap (CAF) combined with platelet-rich fibrin (PRF) or subepithelial connective tissue graft (SCTG). MATERIALS AND METHODS: Tissue samples were harvested from 14 subjects either 1 or 6 months after the surgeries. The 2-mm punch biopsies were obtained from the mid-portion of the grafted sites. Neutral buffered formalin fixed, paraffin-embedded 5-μm thick tissue sections were stained with hematoxylin eosin and Masson's trichrome in order to a…

Comparison of coronally advanced and semilunar coronally repositioned flap for the treatment of gingival recession.

BACKGROUND: Gingival Recession (GR) occurs in population with low oral hygiene levels. Root coverage may be achieved by a number of surgical techniques, including pedicle gingival grafts, free grafts, connective tissue grafts, gtr may also be used. The objective of the present study is to compare the clinical outcomes of the Semilunar Coronally Repositioned Flap (SCRF) and Coronally Advanced Flap (CAF) procedure in the treatment of miller's class I gingival recession defects in maxillary teeth. MATERIALS AND METHODS: Twenty systemically healthy patients, with isolated miller's class 1 gingiva…

Subepithelial connective tissue graft with growth factor for the treatment of maxillary gingival recession defects.

Gingival recession is a challenging defect for clinicians to treat effectively. A number of recent systematic reviews have identified the connective tissue graft (CTG) in combination with coronally advanced flaps (CAFs) as the most consistently effective treatment for Miller Class I and II defects across all outcome measures. Recent advances in recombinant growth factor technology may provide opportunities for improved outcomes in conjunction with CAF + CTG procedures. In this prospective consecutive case series, recombinant human platelet-derived growth factor BB (rhPDGF-BB) was combined with…

Shallow localized gingival recession defects treated with modified coronally repositioned flap technique: A case series.

OBJECTIVES: Various coronally repositioned flap (CRF) techniques have been proposed for coverage of gingival recession defects. Although CRF has several modifications all of them needs vertical or oblique external releasing incisions for treatment of localized gingival recession defects. The aim of present article was to evaluate the effectiveness of a modification of the new CRF procedure without any releasing incision for treatment of shallow localized gingival recession defects. CONCLUSION: Shallow localized gingival recession defects can be treated with modified coronally repositioned fla…

Response of pulp sensibility test is strongly influenced by periodontal attachment loss and gingival recession.

INTRODUCTION: To assess, in vivo, the influence of periodontal attachment loss and gingival recession on responses to pulp sensibility tests (PSTs) with cold stimuli in mandibular incisors in adult patients. METHODS: This cross-sectional study included 45 patients aged 30 to 60 years treated at a university dental health service. In each patient, 1 mandibular incisor was randomly selected for analysis. One calibrated dentist performed all periodontal assessments. Periodontal attachment loss and gingival recession were measured at 6 sites of the selected tooth followed by application of the PS…

Subepithelial connective tissue graft in a localized, plaque-induced ulcerative gingival recession: a case report.

The transposition of connective tissue to increase the zone of keratinized tissue or cover exposed root surfaces has become an integral part of the surgical dental practice. An effort to expand the surgical protocol to gingival recession of ulcerative etiology is presented. Parameters such as pathogenesis, clinical characteristic, and histopathology are analytically discussed. Furthermore, different aspects of the mucogingival therapy phase of the grafting procedure are presented.

Long-term stability of the mucogingival complex following guided tissue regeneration in gingival recession defects.

The purpose of the present study was to evaluate the stability of soft tissue conditions in gingival recession defects treated with guided tissue regeneration (GTR). The study population was selected among those patients who had been treated with GTR procedures for Miller's class I or II, deep (> or =3 mm), buccal gingival recession defects. Defects were included only when they had revealed recession depth reduction > or =2 mm and root coverage > or =60% at 6 months following GTR treatment. These defects were regarded as successfully treated and scheduled for further monitoring. 20 patients, 1…

Controlled clinical evaluation of the subpedicle connective tissue graft for the coverage of gingival recession.

The purpose of this study was to clinically evaluate the 1-year coverage of gingival recessions by a subpedicle connective tissue graft according to the original technique compared to untreated recessions by measuring probing sulcus depth (PSD), height of keratinized tissue (HKT), and mucogingival junction location changes. Paired gingival recessions were selected in 15 patients. In each patient, one recession was randomly assigned for treatment (test group) and the other recession was left untreated (control group) for 1 year. Surgery consisted of a connective tissue graft covered by a double…

The natural history of periodontal disease in man: prevalence, severity, and extent of gingival recession.

This paper describes the occurrence and levels of gingival recession in 2 cohorts of individuals participating in parallel longitudinal studies in Norway (1969-1988) and Sri Lanka (1970-1990), covering the age range from 15 to 50 years. In the Norwegian cohort gingival recession had begun early in life. It occurred in greater than or equal to 60% of the 20 year-olds and was confined to the buccal surfaces. At 30, greater than or equal to 70% had recession, which still was found mainly on buccal surfaces. As the group approached 50 years of age, more than 90% had gingival recession; greater tha…

Grafted and ungrafted labial gingival recession in pediatric orthodontic patients: effects of retraction and inflammation.

The purpose of this retrospective study was to determine changes in recession and other periodontal factors after orthodontic treatment of m … ues and fair-to-poor oral hygiene were maintained by the patients. Statistically significantly less gingival recession was found in both groups after orthodontic treatment than was found pretreatment. The results of this study indicated that labial recession tends to decrease with retrusion of mandibular incisors even when moderate inflammation and fair-to-poor oral …

Prevalence of gingival recession and its correlation with gingival phenotype in mandibular incisors region of orthodontically treated female patients: A cross-sectional study.

BACKGROUND: Gingival recession is associated with dentin hypersensitivity, unesthetic appearance, and carious or noncarious cervical lesions. Orthodontic treatment, gingival thickness (GT), and keratinized tissue width (KTW) play roles in gingival recession etiology. The study is aimed to compare the prevalence of gingival recession in the mandibular incisor region of orthodontically-treated females with untreated controls, and to identify if there is any correlation among the GT, KTW, and gingival recession. SUBJECTS AND METHODS: A total of 150 Saudi females were enrolled in this study. They…

Unexpected resilience to experimental gingivitis of subepithelial connective tissue grafts in gingival recession defects: a clinical-molecular evaluation.

BACKGROUND AND OBJECTIVE: The subepithelial connective tissue graft (SCTG) is the most widely used procedure for the treatment of gingival recession defects. Little is known, however, as to whether the apposed gingival flaps are more predisposed to develop plaque-related inflammation compared to healthy sites. This has salient clinical implications, as the long-term results of root coverage will depend largely on the level of inflammation of the grafted tissue. METHODS: In the present split-mouth case-control study, clinical and biomolecular parameters were used to assess the level of inflamm…
