Casualties of the Global War on Terror and their future impact on health care and society: a looming public health crisis.

This article is a primer to understand the medical advances and the future health care consequences of the current conflicts in the Middle East and Southwest Asia, known as the Global War on Terror. There have been significant advances in health care learned in caring for those injured by the conflict--often a polytrauma blast victim, but there are also very high incidence rates of the hidden injuries of war--traumatic brain injury, post-traumatic stress disorder, suicide, and depression. These lead to disruptive behaviors, homelessness, and family violence. Global War on Terror returnees are

One Health, multiple gaps: Comparative analysis of global policies for zoonotic surveillance.

Understanding potential spillover risk remains critical given that an estimated 75% of human pathogens have zoonotic origins. However, despite calls for action, little work has been conducted to establish a global baseline on policies related to One Health, particularly dealing with the transmission and risks of zoonoses. Understanding the current regulatory and monitoring landscape provides the means to identify both gaps, and areas in greatest need of increased attention. Here we explore the national and international policy linked to One Health, related to both domestic animal husbandry and

Beyond dependency: Ethical health sovereignty and global justice after the pandemic treaty.

The inequities observed during the COVID-19 pandemic were not an accident. They reflect deeper patterns of injustice already embedded in rules, financing models and power arrangements in global health governance. This paper argues that the paradigm of ethical health sovereignty offers a moral countermodel to the dependency shaping global health relations. It examines how sovereignty, solidarity and justice are being reinterpreted after the pandemic by drawing on the WHO Pandemic Agreement (A78/10, 2025) and Africa's regional responses through the African Union and Africa Centres for Disease Co…

Global Health Hazards From Human-Derived Volatile Organic Compounds Amid Emission Variability.

Health risks from high air pollution exposure remain challenging despite pollution control strategies. Health hazards caused by particulate matter and ozone have been reported for the past three decades. Here, we report the hazard quotient for Volatile Organic Compounds (VOCs) causing non-carcinogenic risk globally using chemistry-climate model simulations. We report hazard quotient (HQ) for three scenarios: (a) baseline scenario, (b) continued increasing emissions leading to doubling of anthropogenic VOCs (DVOCS), and (c) mitigation scenarios where emissions are reduced to half (HalfVOCS). In

Exploring Bidirectionality in Global Health Partnerships: Perspectives From U.S. Pediatric Residency Programs.

Introduction Despite growing interest, little guidance exists to re-evaluate global health partnerships (GHPs) through promoting bidirectionality. This study aims to understand the perspectives of United States (U.S.) pediatric global health (GH) faculty on institutional bidirectional practices to inform future discussions with international collaborators. Methods An internally, iteratively developed REDCap-based online survey (REDCap, Vanderbilt University, Nashville, TN), by our diverse author group, collected qualitative data from January 18 to February 20, 2023. Free-text responses were re…

Strengthening One Health: Global Applications of the Joint Risk Assessment Operational Tool.

Risk assessment is critical for managing health threats at the human-animal-environment interface, yet sector-specific approaches can result in fragmented actions. To address this gap, the Joint Risk Assessment Operational Tool (JRA OT), an operational tool of the Tripartite Zoonoses Guide, was developed by the Food and Agriculture Organization of the United Nations, the World Health Organization, and the World Organization for Animal Health. The JRA OT provides a structured framework for joint qualitative risk assessments that integrate multisectoral expertise to identify risk pathways, asses

Toxic crossroads: environmental pesticides and their endocrine footprints in global metabolic health.

ABSTRACT: Endocrine-disrupting pesticides (EDPs) exert deleterious effects on the endocrine system, with documented evidence implicating specific pesticides in endocrine disruption, resulting in developmental delays during puberty and thyroid gland dysfunction, thereby increasing susceptibility to metabolic diseases. The disruption of intracellular insulin signaling, which is characterized by redox imbalance, toxicological effects, and pro-inflammatory activity, facilitates cellular adaptation to stress. However, this adaptive response can aberrantly induce a dysfunctional feedback loop charac

How Should Ongoing Surgical Care Needs be Met During Global Health Engagements?

The Department of Defense has a long history of Global Health Engagement, beginning with identifying and developing vaccines for yellow fever, influenzae, and malaria. Contemporary examples of global health engagement include the worldwide deployment of military assets to aid in natural disasters, and deployment of ship and land based medical teams for short-term medical and surgical engagements. In 2017, the Department of Defense Instruction on Global Health Engagement was published to help guide these efforts. Surgery is an indispensable part of global health and presents unique ethical chal…

Rift Valley fever: An emerging zoonotic threat to global health and agriculture.

Rift Valley fever (RVF) is an acute, mosquito-borne zoonotic disease caused by the RVF virus (RVFV), which belongs to the Phlebovirus genus of the Bunyaviridae family. The infection mainly targets domestic ruminants such as sheep, goats, cattle, and camels, with sheep showing the highest vulnerability. The virus is spread predominantly through the bites of Aedes and Culex mosquitoes, although humans may also become infected by direct exposure to blood, organs, or secretions of diseased animals. In livestock, the illness is marked by sudden fever, ocular and nasal discharges, vomiting, and swol

The Second Silent Pandemic: Why Arboviruses Demand an Orchestrated Global Health Response.

Infections caused by arboviruses, a diverse group of viral pathogens transmitted by biting arthropod vectors, mainly mosquitoes, ticks, and midges, can cause a range of illnesses in humans, from mild, influenza-like symptoms to severe neurological complications including encephalitis and viral hemorrhagic fever. According to 2024 World Health Organization statistics, vector-borne diseases collectively account for over 700,000 human deaths annually, with mosquito-borne infections such as dengue, chikungunya, Zika, and yellow fever constituting a growing and significant proportion of this burden

Emergence, evolution, and global dissemination of antimicrobial resistance: A One Health review.

Antimicrobial resistance (AMR) is a critical global health threat that undermines the treatment of infections and compromises medical interventions. AMR develops when microorganisms evolve mechanisms to survive antimicrobial exposure, a process accelerated by misuse and overuse of antibiotics in human medicine, agriculture, and veterinary settings. Bacterial resistance poses the greatest immediate concern, contributing to an estimated 1.27 million deaths in 2019 and nearly 5 million deaths associated with resistant infections worldwide. Without urgent intervention, Projections suggest that AMR…
