Genital elephantiasis and sexually transmitted infections - revisited.

Genital elephantiasis is an important medical problem in the tropics. It usually affects young and productive age group, and is associated with physical disability and extreme mental anguish. The majority of cases are due to filariasis; however, a small but significant proportion of patients develop genital elephantiasis due to bacterial sexually transmitted infections (STIs), mainly lymphogranuloma venereum (LGV) and donovanosis. STI-related genital elephantiasis should be differentiated from elephantiasis due to other causes, including filariasis, tuberculosis, haematological malignancies, i

Lymphocyte subpopulations in donovanosis assessed by monoclonal antibodies and immunoglobulins.

The haematological profile and peripheral blood lymphocyte subpopulations of 36 patients with donovanosis were estimated by an indirect immunofluorescence technique using monoclonal antibodies. Concentrations of serum immunoglobulins (IgM, IgG, and IgA) were also estimated. The total leucocyte and lymphocyte counts were generally raised, as were total T lymphocyte, T4, and T8 counts. Patients with the hypertrophic variant, however, had decreased numbers of T lymphocytes and T8 cells and increased B cells and IgG. B lymphocyte counts and IgM and IgG concentrations were raised generally.

Thymidine labelling studies in a transmissible venereal tumour of the dog.

The cell population kinetics of the transmissible venereal tumour of the dog was studied at two different stages of tumour growth using the labelled mitoses technique. At the first stage the tumours were growing with a doubling time of about 4 days; at the second stage their growth rate was limited, probably by an immune reaction on the part of the host, to a doubling time greater than 20 days.Labelling of the tumour cells was found to be extremely heterogeneous throughout the tumour. Mitotic figures, however, were present in well labelled as well as in poorly labelled fields, suggesting that

An uncommon cause of a destructive vulval lesion.

We present a case of a 39-year-old woman with a vulval lesion, which macroscopically looked consistent with a fungating squamous cell carcinoma of the vulva. However, further investigations demonstrated a less common cause for this presentation.

Sexually transmitted diseases in adolescents: update 1991.

Clinicians caring for sexually active adolescents are likely to be called on to diagnose and treat many of the STDs discussed in this article. A variety of other STDs not covered here also may be observed, including lymphogranuloma venereum, granuloma inguinale, molluscum contagiosum, scabies, pediculosis, and hepatitis A, B, and C. Some of the special issues related to gay and lesbian youth are discussed in the article by Drs Bidwell and Deisher (see "Adolescent Sexuality: Current Issues," pp 293-302). Nonetheless, it should be mentioned that the same STDs occur in homosexual youths as in the…

Vulval elephantiasis as a result of tubercular lymphadenitis: two case reports and a review of the literature.

INTRODUCTION: Elephantiasis as a result of chronic lymphedema is characterized by gross enlargement of the arms, legs or genitalia, and occurs due to a variety of obstructive diseases of the lymphatic system. Genital elephantiasis usually follows common filariasis and lymphogranuloma venereum. It may follow granuloma inguinale, carcinomas, lymph node dissection or irradiation and tuberculosis but this happens rarely. Vulval elephantiasis as a consequence of extensive lymph node destruction by tuberculosis is very rare. We present two very unusual cases of vulval elephantiasis due to tuberculou…

Genital Herpes in an HIV-Positive Patient Mimicking Donovanosis: A Case Report.

Genital herpes is one of the most common sexually transmitted infections worldwide, primarily driven by herpes simplex virus type 2 (HSV-2). … nical appearance of the lesions, the initial differential diagnoses focused heavily on donovanosis (granuloma inguinale) and vulvar malignancy. To establish an accurate diagnosis, an extensive clinical investigation was performed. Polymerase chain reaction (PCR) testing definitively confirmed HSV-2 infection. Upon initial evaluation, the patient was found to have a n …

Sexually transmitted infections and human immunodeficiency virus coinfection: Scenario in Western India.

CONTEXT: Sexually transmitted infections (STIs) have a well-established synergistic relationship with human immunodeficiency virus (HIV) inf … 8.88%, molluscum contagiosum 7.43%, secondary syphilis 4.33%, gonorrhea 1.85%, chancroid 1.44%, and granuloma inguinale 0.41%. Of all the patients with herpes simplex virus infections, 45.6% (n = 57) had multiple recurrences (>6/year). The incidence of mixed STI was 17.29% in the present study. CONCLUSIONS: The study represents decreasing trends in bacterial STIs an …

Infantile donovanosis presenting as external auditory canal polyps: a diagnostic trap.

Two infants, 6 months and 4 months of age, presented with bilateral or unilateral external auditory canal polyps and otorrhea, respectively. Additional findings on examination included otitis media and mastoiditis. Tympanic membrane perforation was noted in one patient and a postauricular abscess in the other. Incisional biopsies of the polyps and abscess were reported as nonspecific mixed inflammation and abscess wall, respectively. There was a limited response to an empirical 5-day course of trimethoprim sulfamethoxazole. The children were referred to the academic hospital, and excision of t

Case report. MR appearance of cervical donovanosis mimicking carcinoma of the cervix.

Donovanosis is an acquired sexually transmitted disease predominantly affecting the external genitalia. The cervix is an uncommon site of involvement, and can clinically mimic carcinoma of the cervix. The imaging (MR) appearance of donovanosis has not been described previously in the literature. We report a case wherein the MR appearance of cervical donovanosis is indistinguishable from carcinoma of the cervix.

Extragenital donovanosis in a patient with AIDS.

A case of extragenital donovanosis in a patient with AIDS is reported from Zimbabwe. Despite the rarity of donovanosis in Zimbabwe it is important that health workers are familiar with this disease since donovanosis increases the risk of HIV transmission and appropriate treatment is often successful even in patients with severe immunodeficiency.

Pilot study of azithromycin in the treatment of genital donovanosis.

OBJECTIVES: To determine the effectiveness of azithromycin, an azalide antibiotic with long tissue half-life, in a pilot study of patients with genital donovanosis in the Northern Territory, Australia. DESIGN: Patients with histologically confirmed donovanosis were randomised to receive one of two open-label azithromycin dosage regimens: Regimen A--1.0 g once weekly for 4 weeks; or Regimen B--500 mg daily for 7 days. Patients were assessed at 6 weeks and classified as either "cured", "improved" or "Ошибка". RESULTS: Seven patients received regimen A and 4 received regimen B. Six weeks after
