Evaluation of Reablement Home Care: Effects on Care Attendants, Care Recipients, and Family Caregivers.

BACKGROUND: The traditional home care model entails caring "for" people with disabilities, not "with" them. Reablement care has been applied to long-term care, but the evidence for care attendants, home care recipients, and family caregivers simultaneously is limited. METHODS: First, a survey was conducted to explore the needs of home care recipients and family caregivers to achieve independence at home to develop the reablement home care model for home care. Then, an intervention with two groups was implemented. The experimental group included a total of 86 people who participated in the rea

Supported housing programs for persons with serious mental illness in rural northern communities: a mixed method evaluation.

BACKGROUND: During the past two decades, consumers, providers and policy makers have recognized the role of supported housing intervention for persons diagnosed with serious mental illness (SMI) to be able to live independently in the community. Much of supported housing research to date, however, has been conducted in large urban centers rather than northern and rural communities. Northern conditional and contextual issues such as rural poverty, lack of accessible mental health services, small or non-existing housing markets, lack of a continuum of support or housing services, and in some com

Infection prevention and control in residential congregate settings for people with intellectual and developmental disabilities: A learning needs assessment.

Infection prevention and control (IPC) practices are important to prevent the spread of infectious diseases in congregate residential settings, including group homes for people with intellectual and developmental disabilities (IDD). These settings face unique challenges and often receive fewer IPC resources than other congregate residence types. The New York City Health Department conducted a learning needs assessment comprised of a survey among leadership (n=41) and focus groups with frontline staff (n=9). Results highlight the need for IPC guidance tailored specifically to IDD residential se…

Conducting clinical trials in a constantly changing health care environment.

Continuous change and cost containment characterize the current health care system, making conduct of clinical trials and other clinical research difficult. Identification, accrual, and follow-up of patients who move between health care environments such as hospitals, nursing homes, schools and the home is particularly challenging. This article describes a circuit rider approach to patient identification and follow-up that was established by the Communication Sciences and Disorders Clinical Trials Research Group. It also gives suggestions for design of clinical trials in a constantly changing

Statewide study of the direct support staff workforce.

Issues of recruitment and retention related to the direct support staff and how these issues affect the lives of people with developmental disabilities were investigated. Major findings included the following: (a) High direct support staff turnover and vacancy rates have negative consequences for many people who receive supports. (b) Direct support staff provide a vast array of services and have numerous job titles and job descriptions. (c) Direct support staff are inadequately compensated and often have to work more than one job. (d) Turnover rate and recruitment is a serious problem. The nee

Homelessness and substance use among alcohol abusers following participation in project H&ART.

Project H&ART was a randomized intervention trial for homeless alcohol abusers in Albuquerque, N.M. Interventions were four months in duration and included: a high intensity program (case management plus peer-supervised housing), a medium intensity group (peer-supervised housing only); a housed, and a nonhoused control group. Clients were interviewed at baseline and re-interviewed ten months following program entry to determine substance use, housing stability and employment status. Program graduation rates were about 25% for the three housed groups. The outcome evaluation revealed significant

Is the adult social care outcomes toolkit (ASCOT) relevant to health protection teams? Development of an outbreak support tool.

BACKGROUND: Outbreaks of infection in care homes can be mitigated by Public Health and Social Measures (PHSM) including isolation of cases, quarantine of exposed contacts, and shielding the most vulnerable. We aimed to assess the relevance and utility of Adult Social Care Outcomes Toolkit (ASCOT) in outbreak decision-making, with a view to reducing the adverse impact of control measures and informing pandemic planning. METHODS: We worked with health protection practitioners (HPPs) to determine the relevance of ASCOT domains to health protection. The domains were adapted into a 'Wellbeing Chec

Verbal Working Memory Predicts Twice the Growth Rate in Math Skills for Children from Low- Income Homes Relative to Their Higher-Income Peers.

Working memory (WM) and socioeconomic status (SES) associations with math achievement are well established, but little work has considered whether WM contributions are the same across SES strata. Here, we focus on the extent to which verbal WM predicts math differently for children in low vs. higher SES homes in the US to disentangle how these relations might play out across development in differently resourced contexts, as well as the magnitude of these relationships across SES. Using data from a nationally representative sample of 13,527 children, we estimated a parallel process model to ana

Domestic mould exposure and invasive aspergillosis-air sampling of Aspergillus spp. spores in homes of hematological patients, a pilot study.

UNLABELLED: Aspergillus spp.-related morbidity and mortality remains a major challenge in the management of neutropenic patients. Little is known about the impact of domestic Aspergillus spp. EXPOSURE: In this controlled prospective study, fungal spores were collected from homes of neutropenic patients. Cases were defined as patients with probable or proven controls as patients with no invasive pulmonary aspergillosis, while patients with possible disease were evaluated as a third group. Forty patients were enrolled and returned questionnaires on high-risk activities and mould exposure. A. fu

Accidents in the institutionalized elderly: injuries and consequences.

A total of 308 major and 519 minor accidents involving 588 patients occurred during one year at a somatic long-term care hospital and its associated nursing homes. In relation to the number of patients, the incidence of accidents was nearly three times higher in the nursing homes than in the hospital. The most prevalent injuries in female patients were fractures and hematomas, while open wounds were more frequent among male patients. Hip fractures were about as common in women as in men, while fractures of the extremities were three times more frequent in women. The majority of the accidents o

ENDEMIC BYSSINOSIS IN AN EGYPTIAN VILLAGE.

Previous epidemiological surveys of flax byssinosis may have underestimated the incidence of permanent pulmonary disability by failing to reach those who have had to leave work. In the present study a representative sample of the inhabitants of a village where flax is processed both in the homes and in small plants was examined for byssinosis. The one in five random sample included a total of 190 male family heads living in the village. Dust exposure was evaluated. The study showed that 48·4% of the sample had byssinosis, and this included 92·5% of those working with flax in their homes and 75

The Impact of Living Arrangements on the Prevalence of Falls after Total Joint Arthroplasty: A Comparison between Institutionalized and General Geriatric Population.

Due to population aging, there is an increasing need for orthopedic surgery, especially total knee arthroplasty (TKA) and total hip arthroplasty (THA). In geriatric patients, postoperative falls are common events which can compromise the success of these expensive procedures. The aim of our study was to assess the influence of living arrangements on the prevalence of postoperative falls following joint replacement. We included 441 patients after TKA or THA, living in nursing homes, alone or with family. The prevalence of falls in the first 2 years (15.2%) was significantly influenced by living

Does self-reported sleep quality predict poor cognitive performance among elderly living in elderly homes?

OBJECTIVES: Sleep complaints are common among elderly, especially institutionalized elderly, as they experience poorer sleep quality and higher use of sedative hypnotics, when compared to community-dwelling elderly. Recent findings suggest that there may be a relationship between poor quality of sleep and cognitive deficits. This study aimed at studying the relation between sleep quality and cognitive performance in older adults living in elderly homes. METHOD: 100 elderly living in an elderly home in El Mansoura, Egypt, were recruited in this study, 50 cases with subjective poor quality of s

[Depressive symptoms in physically frail elderly].

In 1988 a study aiming to evaluate the effectiveness of an outpatient geriatric clinic was started. Elderly persons (75-90 years) living at home or in the two old people's homes in the municipality Sittard (The Netherlands) were screened on physical vulnerability by means of oral interviews. It was possible to collect data about demographic variables and depressive complaints of 222 frail elderly. For this article the internal consistency of the Zung-depression scale (DSI) was tested for the study group. Relations between depression and the registered demographic variables are discussed. It sh

Aryl hydrocarbon receptor expression is associated with a family history of upper gastrointestinal tract cancer in a high-risk population exposed to aromatic hydrocarbons.

BACKGROUND: Polycyclic aromatic hydrocarbon (PAH) exposure is a risk factor for esophageal squamous cell carcinoma, and PAHs are ligands of the aryl hydrocarbon receptor (AhR). This study measured the expression of AhR and related genes in frozen esophageal cell samples from patients exposed to different levels of indoor air pollution, who did or did not have high-grade squamous dysplasia and who did or did not have a family history of upper gastrointestinal tract (UGI) cancer. METHODS: 147 samples were evaluated, including 23 (16%) from patients with high-grade dysplasia and 48 (33%) from pa

House dust allergens of children with newly diagnosed hazelnut or sesame allergy.

Background: Hazelnut and sesame are among the leading causes of immunoglobulin E mediated food allergies in the eastern Mediterranean region. Notably, initial allergic reactions often occur on the first known oral exposure, which suggests a potential role of non-oral routes of sensitization, including environmental exposure. Objectives: The study aimed to investigate the presence of environmental hazelnut and sesame allergens. Methods: House dust samples were collected from the homes of children newly diagnosed, previously diagnosed, and nonallergic (control) with or without hazelnut or sesame
