Porous CaP/silk composite scaffolds to repair femur defects in an osteoporotic model.

The most common complication for patients with postmenopausal osteoporosis is bone-related defects and fractures. While routine medication has a high probability of undesirable side effects, new approaches have aimed to develop regeneration procedures that stimulate new bone formation while reversing bone loss. Recently, we have synthesized a new hybrid CaP/silk scaffold with a CaP-phase distribution and pore architecture better suited to facilitate cell differentiation and bone formation. The aim of the present study was to compare the involved remodeling process and therapeutic effect of por

Schwann cells, acutely dissociated from a predegenerated nerve trunk, can be applied into a matrix used to bridge nerve defects in rats.

BACKGROUND: The gold standard to reconstruct a nerve defect is a conventional autologous nerve graft. There may be a lack of such grafts in severe nerve injuries. Alternatives to autologous nerve grafts are needed. METHODS: We have developed a technique where mainly Schwann cells are acutely dissociated from the ends of the severed nerve trunk after nerve injury. The technique does not require long-term cell culture procedures. The obtained cells, which can be dissociated within a few hours, are applied to a silicone tube or a tendon autograft used to bridge a nerve defect. FINDINGS: Dissoci

Recent advances in nerve tissue engineering.

Nerve injury secondary to trauma, neurological disease or tumor excision presents a challenge for surgical reconstruction. Current practice for nerve repair involves autologous nerve transplantation, which is associated with significant donor-site morbidity and other complications. Previously artificial nerve conduits made from polycaprolactone, polyglycolic acid and collagen were approved by the FDA (USA) for nerve repair. More recently, there have been significant advances in nerve conduit design that better address the requirements of nerve regrowth. Innovations in materials science, nanote

Regenerative medicine for the respiratory system: distant future or tomorrow's treatment?

Regenerative medicine (RM) is a new field of biomedical science that focuses on the regeneration of tissues and organs and the restoration of organ function. Although regeneration of organ systems such as bone, cartilage, and heart has attracted intense scientific research over recent decades, RM research regarding the respiratory system, including the trachea, the lung proper, and the diaphragm, has lagged behind. However, the last 5 years have witnessed novel approaches and initial clinical applications of tissue-engineered constructs to restore organ structure and function. In this regard,

The effect of a collagen membrane in regenerative therapy of two-wall intrabony defects in dogs.

Guided tissue regeneration that supports the periodontal ligament and bone cells in achieving healthy attachment between teeth and alveolar bone following periodontal therapy has been repeatedly described in the literature. The aim of the present study was to assess the effect of an absorbable collagen membrane used in guided tissue regeneration procedures in two-wall intrabony defects. For this purpose, periodontal defects were surgically created around mandibular teeth in nine dogs. In a randomly chosen quadrant in each dog, a collagen membrane was shaped to cover the interproximal bone defe…

Biologics-based regenerative technologies for periodontal soft tissue engineering.

This manuscript provides a state-of-the-art review on the efficacy of biologics in root coverage procedures, including enamel matrix derivative, platelet-derived growth factor, platelet concentrates, and fibroblast-growth factor-2. The mechanism of action and the rationale for using biologics in periodontal plastic surgery, as well as their anticipated benefits when compared with conventional approaches are discussed. Although the clinical significance is still under investigation, preclinical data and histologic evidence demonstrate that biologic-based techniques are able to promote periodont

Use of a buccinator musculomucosal flap in implant surgery: a case report.

Primary wound closure is difficult in the posterior mandible when insufficient soft tissue is available. Primary wound closure using a buccinator musculomucosal flap after implant placement and guided bone regeneration was performed in a patient with an atrophic alveolar ridge in the posterior mandible. Implants were placed at the bone level of neighboring teeth to protect the inferior alveolar nerve, and exposed threads were covered using guided bone regeneration. A posterior-based buccinator musculomucosal flap was raised and placed over the alveolar crest, which resulted in a satisfactory c

Bacterial Fluorescence Signals Predict Dermal Regeneration Template Integration Outcomes.

Acute trauma wounds with exposed bone or tendon present significant reconstructive challenges, with high rates of dermal regeneration template (DRT) failure, often due to missed infections. Standard-of-care wound assessment, which relies on subjective clinical signs and traditional swabbing methods, frequently fails to detect clinically significant bacterial loads (> 104 CFU/g). This prospective observational study evaluated the ability of intraoperative bacterial fluorescence imaging to predict DRT outcomes in patients with complex wounds. A subset of 13 patients underwent a blinded fluoresce

[Application of pouch technique in the reconstruction of severe alveolar bone defects: a case report].

Guided bone regeneration (GBR) has been widely used in the repair and reconstruction of alveolar bone defects. However, conventional GBR techniques often fail to achieve the desired bone augmentation for severe bone defects (diameter≥5 mm). To address this limitation, several innovative GBR-based approaches, such as the tenting and sausage techniques have been developed, achieving varying degrees of clinical success. Nonetheless, these methods still face considerable challenges, including secondary trauma from autogenous bone harvesting, high technical sensitivity, and limited scalability. In

Healing of intrabony defects using a novel human recombinant amelogenin: a preclinical study.

OBJECTIVE: To histologically evaluate the effects of a novel human recombinant amelogenin (rAmelX) on periodontal wound healing/regeneration in intrabony defects. METHOD AND MATERIALS: Intrabony defects were surgically created in the mandible of three minipigs. Twelve defects were randomly treated with either rAmelX and carrier (test group) or with the carrier only (control group). At 3 months following reconstructive surgery, the animals were euthanized, and the tissues histologically processed. Thereafter, descriptive histology, histometry, and statistical analyses were performed. RESULTS:

A nouveau collagen scaffold to simplify lateral augmentation of deficient ridges between natural teeth.

The volume of the bone in a site past an extraction degrades significantly and thus it is imperative to evaluate the situation for implant placement. Besides the need for sufficient bone, the amount and quality of the soft tissue covering the bone in the missing tooth area and nature of the adjacent teeth must be carefully assessed. In anterior sites, reconstructive surgery is usually performed to restore these hard and soft tissues, mainly for esthetic reasons, but it is equally essential in posterior sites to ensure adequate functional support. In guided bone regeneration procedures, barrier

Single-Flap Approach in Periodontal Regeneration for Intraosseous Defects: Case Series.

INTRODUCTION: A single-flap approach (SFA) is the elevation of a periodontal flap to access the defect only from one side. Several studies have reported that the SFA is at least as clinically effective as the elevation of a flap at both buccal and palatal/lingual aspects. However, studies regarding the SFA have reported only 6 to 10 months follow-up clinical outcomes. The purpose of this case series was to investigate the outcomes of the SFA for periodontal regeneration with a collagen membrane and bone grafts in regard to linear bone defect fill and clinical parameters such as gingival recess

Novel spiral structured nerve guidance conduits with multichannels and inner longitudinally aligned nanofibers for peripheral nerve regeneration.

Nerve guidance conduits (NGCs) are artificial substitutes for autografts, which serve as the gold standard in treating peripheral nerve injury. A recurring challenge in tissue engineered NGCs is optimizing the cross-sectional surface area to achieve a balance between allowing nerve infiltration while supporting maximum axonal extension from the proximal to distal stump. In this study, we address this issue by investigating the efficacy of an NGC with a higher cross-sectional surface composed of spiral structures and multi-channels, coupled with inner longitudinally aligned nanofibers and prote

ESR investigations of gamma irradiated medical devices.

Guided tissue regeneration (GTR) and guided bone regeneration (GBR) biomaterials have been employed in recent years for periodontal procedures. In the present study, widely used dental GTR/GBR biomaterials (grafts: G1, G2, G3 and membranes: M1, M2, M3, M4) were exposed to gamma irradiation at an absorbed dose range of 0-50kGy and the radiolytic intermediates that have been created in the samples upon irradiation were characterized in detail by Electron Spin Resonance (ESR) spectroscopy. We aimed to standardize the measurement conditions for practical applications of gamma radiation sterilizati…

Design and optimization of a biodegradable porous zein conduit using microtubes as a guide for rat sciatic nerve defect repair.

Various degradable biomaterials have been used to bridge injured peripheral nerve defect; however, drawbacks such as poor mechanical properties, inappropriate degradation rate, and toxic degradation products continue to limit the application of them in nerve repair. Considering the unique properties of zein, such as its biocompatibility, biodegradability and ease of fabrication, we report the use of zein conduits to repair injured rat sciatic nerves with a 10-mm defect. Three-dimensional zein conduits were designed with/without pores, and with/without microtubes including in the lumen of condu

The multiple silicone tube device, "tubes within a tube," for multiplication in nerve reconstruction.

Multiple nerve branches were created during the regeneration procedure after a nerve injury and such multiple branches are suggested to be used to control, for example, prosthesis with many degrees of freedom. Transected rat sciatic nerve stumps were inserted into a nine mm long silicone tube, which contained four, five mm long, smaller tubes, thus leaving a five mm gap for regenerating nerve fibers. Six weeks later, several new nerve structures were formed not only in the four smaller tubes, but also in the spaces in-between. The 7-9 new continuous nerve structures, which were isolated as ind

In vivo intervertebral disc regeneration using stem cell-derived chondroprogenitors.

OBJECT: There is currently no biologic therapy to repair or restore a degenerated intervertebral disc. A potential solution may rest with embryonic stem cells (ESCs), which have a potential to grow indefinitely and differentiate into a variety of cell types in vitro. Prior studies have shown that ESCs can be encouraged to differentiate toward specific cell lineages by culture in selective media and specific growth environment. Among these lineages, there are cells capable of potentially producing nucleus pulposus (NP) in vivo. In this investigation, the authors studied ESCderived chondroprogen
