HELLP (hemolysis, elevated liver enzymes, low platelets) syndrome is a rare but serious complication of pregnancy characterized by hemolytic anemia, elevated liver enzymes, and thrombocytopenia. It occurs in <1% of all pregnancies with 70% of cases developing before delivery, the majority occurring between the 27th and 37th weeks of gestation. Respiratory failure seen in HELLP syndrome clinically and radiographically appears similar to acute respiratory distress syndrome (ARDS), with presence of bilateral pulmonary opacities on imaging as well as persistent hypoxemia requiring elevated ventila…

INTRODUCTION: Hemolysis, elevated liver enzyme levels, and low platelet count (HELLP) syndrome is a pregnancy-related clinical condition characterized by hemolysis, elevated liver enzymes, and thrombocytopenia, posing significant risks to maternal and fetal safety. Hepatic hematoma with spontaneous rupture and bleeding is a rare but severe complication of HELLP syndrome, with limited reports of hepatic artery intervention and embolization therapy.
PRESENTATION OF CASE: We present the case of a 35-year-old pregnant woman who developed worsening pain under the xiphoid process the night followin…

BACKGROUND: Postpartum hemolysis, elevated liver enzymes, and low platelet count (HELLP) syndrome is more difficult to treat than HELLP syndrome during pregnancy. We describe a case of postpartum HELLP syndrome that responded to plasma exchange (PE) therapy.
CASE PRESENTATION: A 30-year-old primipara woman was hospitalized for gestational hypertension at 33 weeks of gestation and underwent an emergent cesarean section at 36 weeks and 6 days of gestation due to rapidly progressing pulmonary edema. After delivery, liver dysfunction and a rapid decrease in platelet count were observed, and the p…

Gestational diabetes insipidus is a rare, but well recognized, complication of pregnancy. It is related to excess vasopressinase enzyme activity which is metabolized in the liver. A high index of suspicion of gestational diabetes insipidus is required in a correct clinical setting especially in the presence of other risk factors such as preeclampsia, HELLP syndrome, and twin pregnancies. We are presenting a case of gestational diabetes insipidus in a patient with HELLP syndrome. The newborn in this case also had hypernatremia thereby raising possibilities of vasopressinase crossing the placent…

AIM: To determine the recurrence risk for hypertensive diseases in pregnancy after HELLP (Hemolysis Elevated Liver enzymes and Low Platelets) syndrome in a first pregnancy.
METHODS: The study was designed as a cohort study investigating 148 Caucasian primiparae with a diagnosis of HELLP syndrome in a first pregnancy and at least one subsequent pregnancy conducted beyond the 24th gestational week. Diagnoses were verified by reviewing medical records and classified according to ISSHP (International Society for the Study of Hypertension in Pregnancy) criteria. The recurrence risk of HELLP syndro…

OBJECTIVE: This study was performed to evaluate and compare the placental pathology in patients with severe pre-eclampsia (PE) and HELLP syndrome. Moreover, neonatal birth weight was compared between the two groups.
MATERIALS AND METHODS: This cross-sectional prospective study was conducted in Alzahra and Beheshti Hospitals (Isfahan, Iran) between 2007 and 2009. Placentas from 32 patients having severe pre-eclampsia without HELLP (referred to as preeclampsia group) and 25 patients having severe preeclampsia with HELLP syndrome (referred to as HELLP group) were evaluated. The studied parameter…

INTRODUCTION: the aim of this study was to evaluate the management of HELLP syndrome at Josephine-Bongo Maternity Hospital.
STUDY POPULATION AND METHODS: the retrospective study included all women admitted to the intensive care unit of our hospital from 1 January 2004 through 30 December 2007 with HELLP syndrome. The characteristics studied included: age, parity, number of previous pregnancies, antenatal monitoring, term at onset, clinical and laboratory signs, treatment, complications, perinatal and neonatal mortality and duration of intensive care unit admission.
RESULTS: nine patients, ma…

CASE REPORT: We herein present the first reported case of a pregnant patient with underlying pregnancy-induced hypertension and HELLP (hemolysis, elevated liver enzymes, and low platelet count) syndrome who presented with hepatic rupture following a minor motor vehicle accident.
DISCUSSION: The diagnostic dilemma between spontaneous HELLP syndrome-associated versus trauma-related hepatic rupture is discussed.

OBJECTIVE: To compare neonatal outcome after preterm delivery of infants where pregnancy had been complicated by the HELLP syndrome.
STUDY DESIGN: The maternal and neonatal charts of 475 consecutive pregnancies complicated by hypertensive disorders at our perinatal unit were reviewed. The HELLP syndrome was defined by previously published laboratory criteria. 93 women fulfilled the criteria and constituted our HELLP syndrome study group. 188 normotensive patients who were delivered because of preterm labor comprised the control group. Results were compared by means of chi2 analysis and Studen…

Severe folate deficiency in pregnancy may mimick HELLP syndrome as there are similar features in both. Proper diagnosis is important as far as the prognosis and management are concerned. Two cases of severe folate deficiency occuring in multigravid women mimicking HELLP syndrome are reported below with review of literature.

OBJECTIVE: To determine whether severe pre-eclampsia complicated by hemolysis, elevated liver enzymes, low platelets (HELLP) syndrome alters neutrophil oxygen radical production.
MATERIALS AND METHODS: Neutrophils were obtained from 10 healthy non-pregnant, 9 normal pregnant and 9 women with severe pre-eclampsia with concurrently HELLP syndrome. Oxygen radical production was evaluated using luminol-enhanced chemiluminescence and measured by cytochrome C reduction. Furthermore we incubated sera from cases and controls with isolated healthy neutrophils and measured their capacity to generate ox…

Two patients with HELLP syndrome are presented and hepatic subcapsular haematoma and infarction were documented. The first clinical manifestation in both cases was hypotension. A surgical pregnancy interruption was required. These patients also presented leukocytosis, renal insufficiency, severe hepatic failure and the presence of hepatic encephalopathy (grade III-IV). One patient died in spite of medical intervention with multiple organ failure. The other patient is still alive but with minor liver dysfunction.

A total of 52 patients were referred to our center from gynecology and obstetric units in our area with acute renal failure during the last two years. Seven patients were found to have so called syndrome of hemolysis (H), elevated liver enzymes (EL) and low platelets (LP) associated with acute renal failure. The syndrome can easily be confused with other diagnoses like hemolytic uremic syndrome, idiopathic thrombotic thrombocytopenic purpura and disseminated intravascular hemolysis. Six patients had renal biopsies to confirm the diagnosis, while one did not consent for biopsy. Four patients we

OBJECTIVE: To determine the incidence of Hellp syndrome (HS) and the maternal fetal outcome associated with its occurrence.
DESIGN: A prospective study during a 6-month period.
SETTING: The department of obstetrics and gynecology and the Intensive Care Unit of a 700 bed teaching hospital.
PATIENTS: Sixty-two consecutive preeclamptic and eclamptic women.
MEASUREMENTS AND RESULTS: All patients were systematically investigated for the biological markers of HS. The effects of the occurrence of HS on maternal and fetal prognosis were evaluated by comparing for prognosis indicators usually asses…
