ANTT: an essential tool for effective blood culture collection.

Blood culture collection is an important and topical intravenous procedure for the management of suspected infection. Contaminated samples can lead to 'false positive' results, and inappropriate clinical interventions, which can compromise patient outcome and incur significant expense to healthcare organizations. The contamination of blood culture samples by ineffective aseptic technique has been estimated to be as high as 10% (Department of Health (DH), 2007a). Aseptic Non Touch Technique (ANTT) is a ground-breaking global initiative designed to improve outcomes of aseptic technique through t

Case report: Fluocinonide-induced perioral dermatitis in a patient with psoriasis.

Topical corticosteroids are the primary treatment for psoriasis. A patient with psoriasis being treated with topical fluocinonide for lesions on the extremities developed an erythematous facial eruption consistent with perioral dermatitis. When topical agents are applied, they often end up in unintended areas. The potential for drug-induced perioral dermatitis should be considered in psoriasis patients treated with potent topical corticosteroids.

Frequency and duration of handwashing in an intensive care unit.

The effects on hand decontamination of the introduction of an antiseptic handrub lotion in an intensive care unit was studied. To obtain baseline data, nursing, medical, physiotherapy, radiology, and orderly staff members were observed. Further observations were carried out after the introduction of the handrub lotion. Patient care activities were classified as high or low, according to the degree of contact with the patients or their equipment. A total of 884 patient contacts and 341 hand decontamination episodes was observed: 440 contacts and 140 handwashes in stage one (32%) and 444 contact

Bacterial colonization of intravascular catheters and anergy to recall skin antigens.

The skin reactivity of 46 ICU patients was tested against recall antigens: mumps, candidin, streptokinase-streptodornase, and purified protein derivative of tuberculin. At the same time, the intravascular catheters used in these patients were cultured with a semiquantitative method. Twelve patients did not react to any of the antigens. There was a statistically significant association between anergy in skin testing and lethal outcome of the current illness. Catheter cultures from 11 patients yielded over 15 colony-forming units of bacteria. No association was found between positive catheter cu

Water, sanitation, handwashing, and nutritional interventions can reduce child antibiotic use: evidence from Bangladesh and Kenya.

Antibiotics can trigger antimicrobial resistance and microbiome alterations. Reducing pathogen exposure and undernutrition can reduce infections and antibiotic use. We assess effects of water, sanitation, handwashing (WSH) and nutrition interventions on caregiver-reported antibiotic use in Bangladesh and Kenya, longitudinally measured at three timepoints among birth cohorts (ages 3-28 months) in a cluster-randomized trial. Over 50% of children used antibiotics at least once in the 90 days preceding data collection. In Bangladesh, the prevalence of antibiotic use was 10-14% lower in groups rece

Evaluation of an on-site sanitation intervention against childhood diarrhea and acute respiratory infection 1 to 3.5 years after implementation: Extended follow-up of a cluster-randomized controlled trial in rural Bangladesh.

BACKGROUND: Diarrhea and acute respiratory infection (ARI) are leading causes of death in children. The WASH Benefits Bangladesh trial implemented a multicomponent sanitation intervention that led to a 39% reduction in the prevalence of diarrhea among children and a 25% reduction for ARI, measured 1 to 2 years after intervention implementation. We measured longer-term intervention effects on these outcomes between 1 to 3.5 years after intervention implementation, including periods with differing intensity of behavioral promotion. METHODS AND FINDINGS: WASH Benefits Bangladesh was a cluster-ra

SARS-CoV-2 (COVID-19) and the Teaching of Ignaz Semmelweis and Florence Nightingale: a Lesson of Public Health from History, after the "Introduction of Handwashing" (1847).

Ignác Fülöp Semmelweis (1818-1865) and Florence Nightingale (1820-1910) were two important personalities in the history of medicine and public health. They dealt with the problem of handwashing. Semmelweis is also known as the "father of hand hygiene"; just in 1847 he discovered the etiology and prophylaxis of puerperal sepsis and imposed a new rule mandating handwashing with chlorine for doctors. He also tried to persuade European scientific community of the advantages of handwashing. During the Crimean War, in Scutary (Turkey), Florence Nightingale strengthened handwashing and other hygiene

Handwashing with soap after potential faecal contact: global, regional and country estimates.

BACKGROUND: Limited data have been available on the global practice of handwashing with soap (HWWS). To better appreciate global HWWS frequency, which plays a role in disease transmission, our objectives were to: (i) quantify the presence of designated handwashing facilities; (ii) assess the association between handwashing facility presence and observed HWWS; and (iii) derive country, regional and global HWWS estimates after potential faecal contact. METHODS: First, using data from national surveys, we applied multilevel linear modelling to estimate national handwashing facility presence. Sec

Effect of topical treatments on irritant hand dermatitis in health care workers.

BACKGROUND: Irritant contact dermatitis (ICD) from repetitive hand hygiene is the primary reason for compliance failure among health care workers (HCWs). Chronic ICD has implications for infection control because higher bacterial counts are associated with increased skin compromise. Guidelines recommend lotions/creams to lessen irritation. We evaluated the effects of 5 to 10 daily applications of a test cream (A, glove and chlorhexidine gluconate compatible) and current lotions/creams (B) compared with a control of normal skin care. METHODS: Outcomes were visual skin erythema and dryness, exc

Dermal absorption of isopropyl alcohol from a commercial hand rub: implications for its use in hand decontamination.

Isopropyl alcohol-containing hand rubs are widely used in healthcare for hand decontamination. Ten healthy adult volunteers applied a commercially available isopropyl alcohol-containing hand rub to their hands every 10 min over a 4 h period. Blood isopropyl alcohol levels were measured at the beginning and end of the study. At the end of the study, measurable blood isopropyl alcohol levels (range 0.5-1.8 mg/l) were recorded in nine subjects. We confirmed that isopropyl alcohol could be absorbed through the intact skin of adult humans. The social and medical implications are discussed.

Practical implementation of true on-site water recycling systems for hand washing and toilet flushing.

On-site wastewater reuse can improve global access to clean water, sanitation and hygiene. We developed a treatment system (aerated bioreactor, ultrafiltration membrane, granular activated carbon and electrolysis for chlorine disinfection) that recycles hand washing and toilet flush water. Three prototypes were field-tested in non-sewered areas, one in Switzerland (hand washing) and two in South Africa (hand washing, toilet flushing), over periods of 63, 74 and 94 days, respectively. We demonstrated that the system is able to recycle sufficient quantities of safe and appealing hand washing and

Colonization of a hand washing sink in a veterinary hospital by an Enterobacter hormaechei strain carrying multiple resistances to high importance antimicrobials.

BACKGROUND: Hospital intensive care units (ICUs) are known reservoirs of multidrug resistant nosocomial bacteria. Targeted environmental monitoring of these organisms in health care facilities can strengthen infection control procedures. A routine surveillance of extended spectrum beta-lactamase (ESBL) producers in a large Australian veterinary teaching hospital detected the opportunistic pathogen Enterobacter hormaechei in a hand washing sink of the ICU. The organism persisted for several weeks, despite two disinfection attempts. Four isolates were characterized in this study. METHODS: Brill

Coronavirus outbreaks and infection prevention in dentistry: a narrative review.

OBJECTIVE: This narrative review aims to compile and analyse infection prevention and control (IPAC) practices followed by dental clinics during 3 coronavirus outbreaks: SARS (2002-2004), MERS (2012-2014), and COVID-19 (2019-); and to draw parallels from them for future epidemics. METHODS: Data were collected from 3 databases: Google Scholar, PubMed, and Embase using search terms "SARS," "MERS," "COVID-19," "infection control," "disinfection," and "sterilization". RESULTS: Careful examination of 108 peer-reviewed articles on the 3 outbreaks revealed the following commonalities in the IPAC pr

[Prevention and treatment of necrotizing enterocolitis in the newborn infant from the pediatric point of view].

1. Enteral feeding and factors promoting hypoxia and ischaemia of the gut are thought to cause necrotising enterocolitis of the newborn but … disease should be isolated. The use of a bactericidal and virucidal disinfectant is imperative for hand disinfection. 4. Early diagnosis and early treatment are necessary to prevent progression to advanced stages. 5. Antibiotic treatment should be selected to cover the entire bacterial spectrum of the ward. 6. Frequent physical, radiological and ultrasound examina …

Capillary blood sampling: national recommendations on behalf of the Croatian Society of Medical Biochemistry and Laboratory Medicine.

Capillary blood sampling is a medical procedure aimed at assisting in patient diagnosis, management and treatment, and is increasingly used worldwide, in part because of the increasing availability of point-of-care testing. It is also frequently used to obtain small blood volumes for laboratory testing because it minimizes pain. The capillary blood sampling procedure can influence the quality of the sample as well as the accuracy of test results, highlighting the need for immediate, widespread standardization. A recent nationwide survey of policies and practices related to capillary blood samp
