Accura balloon rupture during percutaneous trans-septal mitral commissurotomy: a rare and potentially fatal complication.

Percutaneous transseptal mitral commissurotomy (PTMC) is the treatment of choice in rheumatic mitral stenosis. The reuse of sterilised PTMC balloon catheters is widely practised to bring down the procedure cost and have proven safety and efficacy. The reused balloons may deform and are prone to rupture causing fatal complications like embolism either of the torn balloon material or air. We report a first case of Accura balloon rupture during PTMC to the best of our knowledge. Fortunately, there was no complication in our patient. Thus, during the balloon preparation it should be examined for a

Retrospective Cohort Study Comparing Rates of Symptomatic Implants Requiring Removal between Standard versus Low-Profile Tibial Nail Interlocking Screws.

This study aimed to compare the rates of symptomatic implants requiring removal between intramedullary tibia nails using standard (SD) versus low-profile (LP) screws. We hypothesize that LP screws will demonstrate lower rates of symptomatic screw removal compared with SD interlocking screws with no difference in secondary outcomes. We performed a retrospective cohort study of all patients 18-89 years old who underwent IMN of tibia fractures from November 2018 to February 2023 at a single, academic level one trauma center. The primary outcome analyzed was symptomatic screws requiring removal. S

Burying flexible intramedullary nails improves implant survival and reduces tip-related complications in nonambulatory children with cerebral palsy.

Closed diaphyseal femoral fractures in children with cerebral palsy classified as Gross Motor Function Classification System (GMFCS) levels IV/V are frequently managed with flexible intramedullary nails (FIN). This study aimed to evaluate the survival of FIN with distal ends buried within the distal femoral metaphyseal cortex. Secondary outcomes included the occurrence of complications, especially those requiring reoperation. Children were divided into two groups based on the location of the distal nail. In group A, the nail ends were buried within the distal femoral metaphyseal cortex. In gro

Management of an 11-cm Femoral Diaphyseal Defect Secondary to Chronic Osteomyelitis Using the Induced Membrane Technique in a 14-Year-Old Girl: A Case Report.

BACKGROUND Management of infected segmental defects of the femoral diaphysis is challenging because infection control, restoration of stability, and reconstruction of bone loss must all be achieved. The induced membrane technique, also known as the Masquelet technique, is a 2-stage reconstructive method for large post-infectious bone defects. This report describes reconstruction of an 11-cm femoral diaphyseal defect secondary to chronic osteomyelitis in a 14-year-old girl. CASE REPORT A 14-year-old girl sustained a Gustilo-Anderson type II open fracture of the right femoral shaft after a motor

A rare case report: MRSA osteomyelitis after CRPP for pediatric distal both-bone forearm fracture.

BACKGROUND: Distal forearm fractures are common pediatric injuries. While Closed Reduction and Percutaneous Pinning (CRPP) is a standard treatment with excellent outcomes, subsequent poliomyelitis is a less commonly reported but serious complication. This case highlights the diagnostic and therapeutic challenges of managing Methicillin-resistant Staphylococcus aureus (MRSA) fracture-related infection (FRI), particularly in the presence of subtle initial signs. CASE PRESENTATION: A 9-year-old male underwent CRPP for a right distal forearm fracture (Fig. 1). Intraoperative radiographs confirmed

Adolescent Clavicle Fractures: A Management Dilemma?

Clavicle fractures are among the most common orthopaedic injuries in adolescents, particularly male athletes, arising primarily from sports-related trauma and vehicular accidents. While non-operative treatment remains the standard approach due to favourable recovery outcomes and lower complication rates, the trend toward surgical fixation has gained traction, driven by emerging studies suggesting potential benefits in certain cases. This review critically examines the indications, outcomes, and complications associated with both conservative and operative management of adolescent clavicle frac

Case Report: Rescue of Relapsed Pain in a Patient with Complex Regional Pain Syndrome Type II by Adding Another Dorsal Root Ganglion Lead.

We present on a patient with complex regional pain syndrome (CRPS) following ankle surgery. Pain was refractory to both conservative and surgical measures including neurotomies, ankle fusion, hardware removal, and spinal cord stimulation (SCS) trial. A dorsal root ganglion (DRG) stimulation trial with lead placements at L4, L5, and S1 provided significant pain and functional improvement. However, during the implantation, we were able to place only two DRG leads at L4 and L5 and not S1 due to difficulties with advancing the lead to the desired location. Nonetheless, the two DRG leads provided 9…

A Case of Carpal Tunnel Syndrome Resulting from Interference Screw Malposition after LRTI.

Many approaches to CMC arthroplasty have been described for treatment of advanced arthritis, yet there is no consensus on the "best" operation. Implantable hardware is increasingly utilized for metacarpal suspension, but few hardware-mediated complications have been documented. Here we present the case of a 69-year-old man with insidious-onset median neuropathy following ligament reconstruction and tendon interposition for CMC arthritis, utilizing interference screw fixation. After surgery, the patient developed median neuropathy, and his physical examination and MRI demonstrated a mass in the

Management of cervical esophageal injury after spinal surgery.

BACKGROUND: Esophageal injury is a rare but catastrophic complication of anterior cervical spine surgery. Cases of esophageal perforation may be discovered intraoperatively, or as late as 10 years after surgery. In the current study we aim to review the principles of care and provide an algorithm that can be employed for successful management of this complex problem. METHODS: We performed a retrospective, Institutional Review Board-approved review of esophageal injuries resulting from anterior cervical spine surgery that were managed at our institution between January 1, 2007 and July 31, 200
