Intermittent complete vascular exclusion of the liver during hepatectomy: technique and indications.

BACKGROUND/AIMS: Complete intermittent vascular exclusion of the liver (IVEL) combines clamping of the hepatic pedicle with clamping of the hepatic veins without interruption of the caval flow. The major advantages of this technique are that patient preclamping fluid overload is avoided, major haemodynamic changes due to caval clamping are escaped, and it allows a very long clamping time. Disadvantage of this technique is the necessity of looping the terminal part of the hepatic veins. METHODOLOGY: In this prospective study, 41 cases of IVEL (Representing 19% of the hepatectomies carried out

Parenteral monoacetoacetin and liver regeneration interaction after partial hepatectomy in the rat.

Parenteral nutrients can be used to manipulate cell proliferation after partial hepatectomy. The relationship among macronutrients--glucose, monoacetoacetin, amino acids--and liver regeneration after partial hepatectomy was investigated. Male rats were anesthetized, received a 70% hepatectomy, and received a low-dose infusion of (1) glucose or (2) monoacetoacetin and a high-dose infusion of (3) glucose, (4) glycerol-glucose, or (5) monoacetoacetin-glucose beginning 6 hours after surgery. The five nonprotein nutrient combinations were infused with and without amino acids. Rats were killed 48 ho…

A new technique of hepatectomy using an occlusion balloon catheter for the hepatic vein.

To minimize intraoperative bleeding and allow more accurate resection, we have devised a new technique for hepatectomy. In addition to occlusion of the afferent vessels, we occlude the hepatic vein at the hepatocaval junction using a balloon catheter inserted transhepatically under intraoperative ultrasonic guidance. We have performed eight hepatectomies using this method. A sequence of 15 minutes of vascular occlusion followed by 5 minutes reperfusion was repeated throughout the operation, and the total ischemic time ranged from 45 to 90 minutes. This method greatly decreased intraoperative b…

The sex difference in the regulation of liver regeneration after partial hepatectomy in the rat.

The increases in the activity of hepatic thymidylate synthetase and thymidine kinase, which catalyzes the formation of thymidylate via the de novo and salvage pathways, respectively, were significantly suppressed 24 h after 70% partial hepatectomy in female rats administered either alpha- or beta-adrenoreceptor antagonists. The injection of beta-antagonist to male or ovariectomized female rats had no effect on the activities of these enzymes. Only alpha-adrenoceptor antagonist depressed these enzymatic activities of 24-h-regenerating liver in male and ovariectomized female rats. The decrease o…

Postoperative liver function trajectories and risk factors after hepatectomy for hepatocellular carcinoma.

PURPOSE: Hepatectomy is a curative treatment for hepatocellular carcinoma (HCC) but deteriorating postoperative liver function can limit its benefits, especially for patients with borderline resectable (BR) HCC. We evaluated postoperative liver function trajectories and their impact on long-term survival. METHODS: The subjects of this retrospective analysis were 790 patients who underwent initial curative hepatectomy, stratified by resectability (R, BR1, BR2) and modified albumin-bilirubin (mALBI) grade. Liver function was assessed via ALBI scores pre- and postoperatively. Deterioration was d…

Robotic Hepatectomy Using the Hugo RAS System: Initial Experience and Technical Considerations.

Robotic hepatectomy using the Hugo robotic-assisted surgery (RAS) system has recently been introduced; however, clinical experience remains limited. We report our initial experience in five patients, focusing on surgical techniques and short-term outcomes. All procedures were completed robotically without conversion to open or laparoscopic surgery. The median operative time was 175 min (range, 162-209), and the median console time was 100 min (range, 79-139). The median inflow occlusion time was 31 min (range, 14-62). Intraoperative blood loss was minimal, with a median of 0 mL (range, 0-10). …

Robotic Hepatoduodenal and Cardinal Lymphadenectomy during Hepatectomy for Left-Sided Intrahepatic Cholangiocarcinoma.

BACKGROUND: Surgery is currently the only curative treatment for resectable intrahepatic cholangiocarcinoma. Guidelines recommend lymphadene … rcinoma in the left liver. The multidisciplinary team chose upfront resection, including a left hemihepatectomy with lymphadenectomy along both the hepatoduodenal and cardinal routes. A robotic approach was proposed, and consent was obtained. RESULTS: The total operative time was 195 minutes, with minimal blood loss. The Pringle maneuver was used intermitten …

Left hepatectomy for chronic biopsy-induced intrahepatic arterioportal fistula with aneurysmal degeneration.

Arterioportal fistula (APF) is a rare complication of percutaneous liver biopsy. While most biopsy-induced shunts are asymptomatic and resol … perfusion, endovascular embolization was not pursued. Instead, the patient underwent a formal left hepatectomy, which effectively controlled the fistulous area. Histopathological examination confirmed chronic arterialization of portal venous structures, accompanied by fibromyxoid intimal thickening, consistent with long-standing high-flow shunting. This case …

Intermittent Inflow Occlusion in Robotic Right Donor Hepatectomy: A Randomized Controlled Trial.

OBJECTIVE: To assess the safety and efficacy of intermittent inflow occlusion (IIO) during robotic right donor hepatectomy (RDH) for adult living donor liver transplantation. BACKGROUND: Although evidence supports minimally invasive donor hepatectomy, its adoption by surgeons remains limited. Key challenges include bleeding during parenchymal transection and prolonged warm ischemia during graft extraction, resulting in morbidity in both the donor and recipient. METHODS: Between April 2022 and June 2023, out of 123 donors suitable for RDH, 113 were randomized to undergo robotic RDH with (n=56…

Metabolic changes after hepatectomy: Implications for perioperative management and long-term outcomes.

BACKGROUND: Hepatectomy triggers complex metabolic changes as the liver adapts to the loss of functional mass and initiates regeneration. These changes impact carbohydrate, lipid, and protein metabolism, as well as energy and oxygen dynamics. A deeper understanding of these shifts is essential for improving perioperative management and enhancing long-term outcomes. AIM: To review and synthesise the current understanding of metabolic changes following hepatectomy and explore their implications for perioperative care, postoperative recovery, and long-term patient outcomes. METHODS: A narrative…

Three-dimensional location approach with silk thread guided hepatectomy for liver tumor.

BACKGROUND: Intraoperative determination of resection margin and adequate residual liver parenchyma are the key points of hepatectomy for the treatment of liver tumors. Intraoperative ultrasound and indocyanine green fluorescence navigation are the most commonly used methods at present, but the technical barriers limit their promotion. AIM: To evaluate the value of the three-dimensional location approach with silk thread (3D-LAST) in precise resection of liver tumors. METHODS: From September 2020 to January 2022, 8 patients with liver tumors including hepatocellular carcinoma, intrahepatic c…

Vitamin D Receptor Regulates Liver Regeneration After Partial Hepatectomy in Male Mice.

Vitamin D signals through the vitamin D receptor (VDR) to induce its end-organ effects. Hepatic stellate cells control development of liver … cking hepatocyte VDR had significant increases in expression of conjugated bile acids after partial hepatectomy, consistent with failure to normalize hepatic function by the 14-day time point tested. Real-time PCR of hVDR and control livers showed significant changes in expression of cell-cycle genes including cyclins D1 and E1 and cyclin-dependent kinase 2. …

Outpatient and Ambulatory Extended Recovery Robotic Hepatectomy: Multinational Study of 307 Cases.

BACKGROUND: For open minor hepatectomy, morbidity and recovery are dominated by the incision. The robotic approach may transform this "incision dominant procedure" into a safe outpatient procedure. STUDY DESIGN: We audited outpatient (less than 2 midnights) robotic hepatectomy at 6 hepatobiliary centers in 2 nations to test the hypothesis that the robotic approach can be a safe and effective short-stay procedure. Establishing early recovery after surgery programs were active at all sites, and home digital monitoring was available at 1 of the institutions. RESULTS: A total of 307 outpatient (…

Retroperitoneal laparoscopic hepatectomy of recurrent hepatocellular carcinoma: case report and literature review.

BACKGROUND: Almost all liver tumours can be removed laparoscopically, but some difficult tumour locations complicate laparoscopic surgery. Recurrent liver tumours often pose great difficulties to laparoscopic surgery due to adhesions caused by previous operations. Referring to laparoscopic adrenalectomy, a retroperitoneal approach is proposed to remove liver tumours near the adrenal gland, which will provide a new method for liver surgery. CASE PRESENTATION: Our case involves a patient with recurrent hepatocellular carcinoma (HCC) whose last operation was laparoscopic hepatectomy in our depar…
