Saline Displacement of Rectal Wall for Improved Margin During MRI-Guided Transurethral Ultrasound Ablation (TULSA).

MRI-guided transurethral ultrasound ablation (TULSA) is an effective treatment for prostate cancer, but proximity to the rectum presents a challenge for ablation margin. We report the first case using saline infusion to displace the rectal wall during TULSA, allowing for improved treatment margins while minimizing rectal risk. A 67-year-old male with a Gleason score 3 + 4 = 7 lesion underwent right hemiablation with saline infusion achieving 5.3 mm displacement. Post-procedure imaging showed no rectal injury, and follow-up revealed no complications or recurrence. This novel technique demonstra

Improving necrosis estimation in MR-guided prostate focused ultrasound ablations with a probabilistic thermal dose model.

False positive necrosis classification close to ablation zones in magnetic resonance (MR)-guided transurethral ultrasound ablation of the prostate poses an issue for real-time assessment of the treatment. Typically, apriori knowledge about transducers is utilized to filter temperature maps, thereby reducing false positives. Our innovation, however, lies in the ability of our probabilistic CEM43 thermal dose model (pCEM43) to decrease false positives without the need for prior knowledge beyond the noise statistics estimated from baseline images. The original pCEM43 was modified improving statis

Precision Targeted Ablation of Fine Neurovascular Structures In Vivo Using Dual-mode Ultrasound Arrays.

Carotid bodies (CBs) are chemoreceptors that monitor and register changes in the blood, including the levels of oxygen, carbon dioxide, and pH, and regulate breathing. Enhanced activity of CBs was shown to correlate with a significant elevation in the blood pressure of patients with hypertension. CB removal or denervation were previously shown to reduce hypertension. Here we demonstrate the feasibility of a dual-mode ultrasound array (DMUA) system to safely ablate the CB in vivo in a spontaneously hypertensive rat (SHR) model of hypertension. DMUA imaging was used for guiding and monitoring fo

Effects of Thermal Preconditioning on Tissue Susceptibility to Histotripsy.

Histotripsy is a non-invasive ablation method that mechanically fractionates tissue by controlling acoustic cavitation. Previous work has revealed that tissue mechanical properties play a significant role in the histotripsy process, with stiffer tissues being more resistant to histotripsy-induced tissue damage. In this study, we propose a thermal pretreatment strategy to precondition tissues before histotripsy. We hypothesize that a thermal pretreatment can be used to alter tissue stiffness by modulating collagen composition, thus changing tissue susceptibility to histotripsy. More specificall

MRgFUS treatment of uterine fibroid in a nulliparous woman with acute retention of urine.

Uterine fibroids are the most common tumors of the female reproductive tract. Although most fibroids are asymptomatic, about 25% are associated with symptoms that can have a significant impact on patient's quality of life, including prolonged or excessive menstrual bleeding, pelvic pain or bulkiness, dyspareunia, increased urinary frequency, and infertility. Various treatment options available for symptomatic uterine fibroids include hysterectomy, myomectomy (abdominal or laparoscopic), uterine artery embolization, MR-guided Focused Ultrasound (MRgFUS), and hormonal therapy, which also is some

Targeted long-term venous occlusion using pulsed high-intensity focused ultrasound combined with a pro-inflammatory agent.

Esophageal and gastric varices are associated with significant morbidity and mortality for cirrhotic patients. The current modalities available for treating bleeding esophageal and gastric varices, namely endoscopic band ligation and sclerotherapy, require frequent sessions to obtain effective thrombosis and are associated with significant adverse effects. A more effective therapy that results in long-term vascular occlusion has the potential to improve patient outcomes. In this study, we investigated a new potential method for inducing long-term vascular occlusion by targeting segments of a r
