The incidence of occult dysplasia of the contralateral hip in children with unilateral hip dysplasia.

BACKGROUND: The purpose of this study is to determine the incidence of contralateral hip dysplasia in children with unilateral hip dysplasia. METHODS: We reviewed all patients with developmental dysplasia of the hip (DDH) who were evaluated at our institution until skeletal maturity (at least age 13 years). In the contralateral ("normal") hip, radiographic measurements of the acetabular angle >46 degrees, center edge angle <20 degrees, and migration index >20% were considered abnormal. RESULTS: Of the 43 patients in the study, 18 (42%) were diagnosed with bilateral hip dysplasia as an infant…

Hip arthrography in the assessment of children with developmental dysplasia of the hip and Perthes' disease.

In our unit, children with developmental dysplasia of the hip or Perthes' disease, for whom an operation is considered, undergo examination under anaesthetic and a hip arthrogram. This prospective study assessed whether the arthrogram modified treatment and analysed the reliability of its interpretation. All children undergoing a hip arthrogram for developmental dysplasia of the hip and Perthes' disease over a 12-month period were included. Treatment plans before and after the arthrogram were compared for each of the three children's consultants. The preoperative and arthrographic appearances …

Results of medial open reduction of the hip in infants with developmental dislocation of the hip.

Thirty-two patients who had medial open reduction of 40 hips were reviewed at an average of 10.3 years follow-up (range 2.5-18.6 y). Center-edge angles and acetabular indices were measured on preoperative, postoperative, and final radiographs. The presence of AVN was noted. The final radiograph was assigned a Severin grade. One hip re-dislocated. Eight (20%) have required subsequent pelvic osteotomies. AVN developed in 11 hips (27.5%). Of the 11, five were classified as type I, defined as temporary irregular ossification of the femoral head. Bilateral dislocations and age older than 1 year at …

Treatment of dysplastic and congenitally dislocated hips with the Zweymueller total hip prosthesis.

Uncemented Zweymueller total hip prostheses were implanted in 35 dysplastic or dislocated hips in 33 patients. Sixteen hips were dislocated and 19 hips were dysplastic; in 12 hips, an intertrochanteric or pelvic osteotomy was performed in early childhood. In all cases, the titanium screw socket was implanted at the level of the original cotyloid cavity. Osteotomy of the greater trochanter, shortening osteotomy, or roof acetabuloplasty were not performed. In cases in which the femoral cavity was too narrow for the Zweymueller stem, an anterolateral longitudinal window-shaped osteotomy was perfo…

Modified technique for fluid aspiration from the hip in patients with prosthetic hips.

A modified technique was used for diagnostic aspiration of fluid from the hip in 185 patients who had previously undergone total hip arthroplasty. The aspiration needle was advanced past the lateral aspect of the shaft of the prosthesis and into the dependent portion of the joint. Fluid was successfully aspirated in 181 of 185 patients; thus the dry-tap rate was 2.2% (four of 185 patients). The modified technique was simple, could be performed quickly, and was effective for sampling joint fluid in patients with hip prostheses.

Intracapsular pressure and loosening of hip prostheses. Preoperative measurements in 18 hips.

We measured the intracapsular pressure preoperatively in 18 hips (17 patients) before revision of a total hip arthroplasty because of aseptic loosening. Distension of the joint capsule was measured with sonography in 13 cases. In extension, the mean intracapsular pressure was 26 (0-60) mmHg, in extension and inward rotation it was 159 (24-280) mmHg, in extension and outward rotation it was 30 (3-67) mmHg and in 45 degrees of flexion it was 12 (0-28) mmHg. A mean of 6 (0.5-20) mL of joint fluid was aspirated after the pressure measurements. Sonography showed increased joint fluid/synovial edema…

Recurrent traumatic dislocation of the hip in a child: significance of early hip arthrography.

The authors describe a case of a recurrent posterior traumatic dislocation of the hip in a child in whom early arthrography demonstrated a large defect of the posterior capsule. The authors emphasize the necessity of prolonged immobilization and long-term follow-up. The operation of the patient was performed on the 7th day after the third recurrence of the dislocation. Revision and suture of the capsular defect from Gibson's approach were done.

Safety and Functional Outcomes Following Total Hip Arthroplasty Using the Latitud™ Total Hip Replacement System in Patients With Hip Fracture.

Background Hip fractures, particularly among the aging population, present a significant burden due to their association with morbidity, mortality, and long-term disability. Total hip arthroplasty (THA) is increasingly being adopted for fracture management in active patients, but implant performance data in real-world Indian settings remain sparse. This study assesses the early clinical efficacy, functional outcomes, and implant survivorship of the Latitud™ total hip replacement system (HRS) (Meril Healthcare Pvt. Ltd., Vapi, India), a novel prosthesis designed and manufactured in India, in pa…

Anterior Inferior Iliac Spine Bone Morphology in Hip Dysplasia and Its Effect on Hip Range of Motion in Total Hip Arthroplasty.

BACKGOUND: Despite the fact that femoral impingement against the anterior inferior iliac spine (AIIS) is increasingly recognized, there is no description of morphologic features of the AIIS in hip dysplasia and their effect on hip range of motion (ROM) in total hip arthroplasty (THA). The purpose is to evaluate the bone morphology of the AIIS in hip dysplasia and whether its morphology affects hip ROM in THA. METHODS: Computed tomography-based simulation software was used to create 3-dimensional bone models and perform virtual simulations. Using the computed tomographic data of 85 patients (m…

Does Cup Position at the High Hip Center or Anatomic Hip Center in THA for Developmental Dysplasia of the Hip Result in Better Harris Hip Scores and Revision Incidence? A Systematic Review.

BACKGROUND: One goal of THA is to restore the anatomic hip center, which can be achieved in hips with developmental dysplasia by placing cups at the level of the native acetabulum. However, this might require adjuvant procedures such as femoral shortening osteotomy. Another option is to place the cup at the high hip center, potentially reducing surgical complexity. Currently, no clear consensus exists regarding which of these cup positions might offer better functional outcomes or long-term survival. QUESTION/PURPOSE: We performed a systematic review to determine whether (1) functional outcom…

Adaptive walking performance is related to the hip joint position sense during active hip flexion rather than during passive hip flexion.

The purpose of this study was to investigate the relationship between hip joint position sense during active or passive hip flexion and adaptive walking performance across obstacles. After screening, 30 young men with the right dominant leg (age, 21 ± 2 years) participated in the experiment. To measure adaptive walking performance on the first day, the participants stepped over an obstacle underfoot with the left leg just high enough to avoid touching the obstacle. The difference between the height of the knee joint at the moment of crossing the obstacle and the height of the obstacle was norm…

Iliopsoas Tenotomy Does Not Negatively Affect Hip Flexion Strength in Crowe 3 and 4 Hips Undergoing Total Hip Arthroplasty.

BACKGROUND: In total hip arthroplasty (THA), even if there is a dislocated hip, restoring the anatomical center of rotation increases long-term survival and hip function. Even with a shortening osteotomy, the procedure is still challenging due to soft-tissue tension. Therefore, soft tissue releases such as iliopsoas tenotomy may be necessary. In our study, we aimed to examine the effects of iliopsoas tenotomy on hip flexion and abduction strength in patients who underwent THA with shortening osteotomy for Crowe 3 and 4 hip dysplasia by using a hand dynamometer for measurement. METHODS: The pr…
