Leading infectious diseases problems in Turkey.

Turkey has significant geographical and socio-economic differences throughout a vast area of the country. These characteristics affect the epidemiology of infectious diseases, some of which are rarely seen in western Europe. However, effectively implemented control measures have resulted in decreased rates of many community-acquired infections, including tuberculosis and malaria, that were major health problems only a few decades ago. There are high rates of antimicrobial resistance in various nosocomial isolates of Gram-positive and Gram-negative bacteria. A recently implemented, nationwide,

Tracking Cholera through Surveillance of Oral Rehydration Solution Sales at Pharmacies: Insights from Urban Bangladesh.

BACKGROUND: In Bangladesh, pharmacy-purchased oral rehydration solution (ORS) is often used to treat diarrhea, including cholera. Over-the-counter sales have been used for epidemiologic surveillance in the past, but rarely, if ever, in low-income countries. With few early indicators for cholera outbreaks in endemic areas, diarrhea-related product sales may serve as a useful surveillance tool. METHODOLOGY/PRINCIPAL FINDINGS: We tracked daily ORS sales at 50 pharmacies and drug-sellers in an urban Bangladesh community of 129,000 for 6-months while simultaneously conducting surveillance for diar

Survey of medication history of patients with stroke after discharge from an acute hospital ward: a case series study.

BACKGROUND: Stroke is a leading cause of death, reducing disability-free life expectancy. After acute treatment, patients require rehabilitation to prevent recurrence. Continued use of medication is crucial for recurrence prevention and risk management, even after the transition from acute-care institutions to other medical institutions. Although "discharge summaries on medications" are shared between hospitals and community pharmacists, no reports have addressed medication continuity for patients with stroke transferred to other institutions after discharge. This study aimed to clarify medica

Detection of viral respiratory pathogens in mild and severe acute respiratory infections in Singapore.

To investigate the performance of laboratory methods and clinical case definitions in detecting the viral pathogens for acute respiratory infections (ARIs) from a prospective community cohort and hospital inpatients, nasopharyngeal swabs from cohort members reporting ARIs (community-ARI) and inpatients admitted with ARIs (inpatient-ARI) were tested by Singleplex Real Time-Polymerase Chain Reaction (SRT-PCR), multiplex RT-PCR (MRT-PCR) and pathogen-chip system (PathChip) between April 2012 and December 2013. Community-ARI and inpatient-ARI was also combined with mild and severe cases of influen

Effective implementation strategies and drivers of culture change for improving time to treatment for severe maternal hypertension.

OBJECTIVE: To assess effective implementation strategies to reduce time to treatment for severe maternal hypertension and drivers of culture change among high-performing sites in a statewide quality improvement (QI) initiative. METHODS: Using a mixed-methods sequential explanatory design, mixed effect linear regression models with a logit link were used to assess the association between achievement of system changes at each hospital and the proportion of cases in which time to treatment was achieved, including fixed effects for system changes and time (quarter of implementation year) and rand

Value of radiotelemetry in a community hospital.

The use of radiotelemetry for continuous electrocardiographic monitoring of patients outside of special care units, although common practice in many community hospitals, has not been adequately studied. Two-hundred five consecutive patients placed on telemetry over a 3-month period were prospectively studied to determine the benefits of radiotelemetry. Fourteen episodes of significant arrhythmias in 12 patients who required specific intervention were detected over 608 patient-days of monitoring. Significant arrhythmias occurred only in patients with known or suspected coronary artery disease o

Rehabilitation after myocardial infarction.

The concept that many patients with asymptomatic coronary atherosclerotic heart disease, including myocardial infarction, can and should return toward normal living is the basis of the rehabilitation effort. The primary care physician is increasingly employing this approach in the care of patients with myocardial infarction. Much of the capacity to effect rehabilitation should be available in the community hospital, in the office of the primary physician, or through community, governmental, public, and voluntary health care agencies and facilities. Although some patients with complex diagnosti

Considerations for Multiteam Systems in Emergency Medical Services.

OBJECTIVE: Despite good intentions, mishaps in teamwork continue to affect patient's lives and plague the medical community at large and Emergency Medical Services (EMS) in particular. Effective and efficient management of patient care necessitates that sets of multiple teams (i.e., multiteam systems [MTSs] - EMS ground crews, EMS air crews, dispatch, and receiving hospital teams) seamlessly work together. Although advances have been made to improve teams, little research has been dedicated to enhancing MTSs especially in the critical yet often under studied domain of EMS. The purpose of this

Structural awareness and knowledge of social determinants among pharmacists by practice settings and training eras.

BACKGROUND: Pharmacists' ability to recognize structural factors - including socioeconomic conditions and systemic barriers - that influence patient health outcomes is critical for addressing health disparities. To build this capacity, social determinants of health (SDOH) and population health have become integral aspects of the pharmacy curriculum. However, whether their inclusion enhances structural awareness remains unknown. The study objectives were to determine whether structural awareness differs between pharmacists in hospital, ambulatory, and community settings; and between those train

Reasons for Optimism About Academic Medicine's Actions Against Climate Change.

Since the first mention of climate change in Academic Medicine in 2009, the pace of the climate crisis has accelerated, its impacts on every facet of planetary health have grown more severe, and the urgency for humans to act has become more dire. Medical schools, teaching hospitals and health systems, universities, affiliated organizations, and the millions of people who traverse the halls of these institutions as leaders, physicians, scientists, educators, learners, patients and families, and community members have an obligation to respond. In this commentary, the authors describe 3 reasons t

Risk factors of community-onset extended-spectrum β-lactamase-producing Klebsiella pneumoniae bacteraemia in South Korea using national health insurance claims data.

BACKGROUND: Although it is essential to know the particular causes of antibiotic-resistant infections in the community, there is lack of evidence regarding risk factors for community-onset extended-spectrum β-lactamase-producing Klebsiella pneumoniae (ESBL-KP) bacteraemia in South Korea. As such, this study aimed to identify risk factors for community-onset ESBL-KP bacteraemia. METHODS: From May 2016 to April 2017, patients with community-onset KP bloodstream infection (BSI) (n = 408) from six sentinel hospitals participating in the Global Antimicrobial Surveillance System in South Korea were

Family Voices in Digital Patient Navigation for Cervical Cancer Care in Indonesia.

Background: Cervical cancer remains a significant health issue in Indonesia, where structural barriers, fragmented information, and sociocultural norms continue to hinder timely diagnosis and treatment. Families play a central role throughout the illness journey, yet their perspectives are often overlooked in the development of digital patient navigation systems. This study explored family experiences, caregiving challenges, and expectations for a family-centered digital navigation model, DIVA.ID, by integrating Digital Health frameworks and Family Systems Theory. Methods: A qualitative descri

Health-system drivers influencing the continuum of care linkages for low-birth-weight infants at the different care levels in Ghana.

BACKGROUND: Low birth weight (LBW) is associated with short and long-term consequences including neonatal mortality and disability. Effective linkages in the continuum of care (CoC) for newborns at the health facility, community (primary care) and home care levels have a high tendency of minimizing adverse events associated with LBW. But it is unclear how these linkages work and what factors influence the CoC process in Ghana as literature is scarce on the views of health professionals and families of LBW infants regarding the CoC. Therefore, this study elicited the drivers influencing the CoC

Innovations in plans of safe care implementation for infants with prenatal substance exposure.

INTRODUCTION: Federal legislation requires states to notify child welfare agencies of all newborns affected by prenatal substance exposure and to develop a plan of safe care (POSC). However, there is substantial variation in how states implement POSC and no best-practice standards. This study explored innovative state practices to understand how POSC policies can best support affected children and families across policy, health system, and child welfare contexts. METHODS: We conducted qualitative interviews with national experts on perinatal substance use policy, as well as state officials an
