"I'm just searching to get better": Constructions of treatment citizenship on injectable opioid agonist treatment.

BACKGROUND: As part of the response to Canada's worsening overdose crisis driven by a toxic, adulterated drug supply, there has been increas … in treatment and avoids pleasure. However, structural vulnerabilities, including, homelessness and housing instability, entrenched poverty, criminal-legal system engagement, and unmanaged pain, shaped the ability of participants to make claims to this normative model of citizenship. CONCLUSION: This study examined how structural vulnerabilities impact people's cons …

[Vertigoes in E.N.T].

Vertigoes represent 5% of GP consultations. We thus often deal with complaints from vestibular origin. The starting point of this text is the word "vertigo or instability" as well as a targeted anamnesis allowing a general approach leading to a precise clinical examination and thus to a diagnosis of most commonly encountered E.N.T. pathologies. The anamnesis, the oto-neurological test and the treatment(s) will be further developed. Willingly the complementary tests performed in the E.N.T. unit have been skipped and we will keep the focus on the anamnesis and the clinical examination that can b

Construct, concurrent, and real-world predictive validity of the Difficulties in Emotion Regulation (DERS-18) among young adults with history of homelessness.

Skills in emotional regulation are vitally important for enabling homeless young adults to navigate the complex and chaotic settings associated with homelessness. The current study seeks to test the construct, concurrent, and predictive validity of the Difficulties in Emotional Regulation Scale (DERS-18) in a sample of formerly and currently homeless young adults. Data come from a study of HIV risk among young adults who have experienced homelessness, including both those currently experiencing homelessness (n = 99) and in permanent housing (n = 120), collected between 2017 and 2019 in Los Ang

Eviction From Renter-occupied Households and Rates of Sexually Transmitted Infections: A County-level Ecological Analysis.

BACKGROUND: Housing instability is linked to numerous health outcomes, but the specific impact of eviction from renter-occupied households, an event that has immediate implications for residential stability for low-income individuals, on sexually transmitted infections (STI) rates has not been adequately studied. METHODS: We examined county-level associations between eviction rates in 2014 and rates of chlamydia and gonorrhea in the following year using publicly available data sources (Eviction Lab National Database and AtlasPlus, respectively). Descriptive statistics compared medians and non…

Functional genetic evaluation of DNA house-cleaning enzymes in the malaria parasite: dUTPase and Ap4AH are essential in Plasmodium berghei but ITPase and NDH are dispensable.

BACKGROUND: Cellular metabolism generates reactive oxygen species. The oxidation and deamination of the deoxynucleoside triphosphate (dNTP) pool results in the formation of non-canonical, toxic dNTPs that can cause mutations, genome instability, and cell death. House-cleaning or sanitation enzymes that break down and detoxify non-canonical nucleotides play major protective roles in nucleotide metabolism and constitute key drug targets for cancer and various pathogens. We hypothesized that owing to their protective roles in nucleotide metabolism, these house-cleaning enzymes are key drug target

Adverse childhood experiences and risk of suicide and substance-related mortality through middle adulthood.

BACKGROUND: Few studies have prospectively examined whether adverse childhood experiences contribute to suicide or substance-related mortality. Moreover, children are often exposed to multiple adversities making it critical to identify which clusters of adversities are most harmful for these outcomes. Accordingly, we investigated risk for suicide and substance-related mortality based on the number and clusters of adversities children were exposed to. METHODS: Identifying information from 49,853 offspring born between 1959 and 1966 to participants in the Collaborative Perinatal Project was lin

Ketamine addiction following a single sub-anaesthetic ketamine treatment for acute suicidality in a psychiatrically multimorbid patient: case report.

This case report presents the case of a 25-year-old woman who developed ketamine addiction following a single sub-anaesthetic dose of intran … ethcathinone (3-MMC). Within weeks she had escalated to daily use, which led to financial distress, housing instability and a suicide attempt when access was cut off. Although she initially ceased use, she later relapsed into ketamine and cocaine addiction. This case highlights the addictive risk of ketamine, even in controlled settings. Given ketamine's rising use i …

Addressing Health-Related Social Needs: Value-Based Care or Values-Based Care?

Health-related social needs, such as food insecurity, housing instability, and lack of transportation, are associated with worse health outcomes, and are increasingly the focus of health-related social needs interventions within healthcare. Adoption of health-related social needs interventions is often justified by the potential to reduce healthcare costs. However, this can present a conundrum to clinicians. Physicians are often more accustomed to justifying clinical innovation based on improvements in health, in accord with the fundamental values of the medical profession, which include using…

Local environment, socioeconomic status, and health literacy: key modifiers of adherence in pediatric nephrology.

Pediatric nephrology patients have high rates of treatment nonadherence, thus increasing their risk for poor health outcomes. Key modifiers of adherence among pediatric nephrology patients include local environment (place of residence, school setting, clinic setting), socioeconomic status, and health literacy. Where patients live affects access to health care, community, and school resources that support adherence and disease management. Socioeconomic challenges, such as lower income, food insecurity, and housing instability, may lead patients and families to prioritize immediate survival need…

Association between social determinants of health and periodontitis: a population-based study.

BACKGROUND: The association between socioeconomic status and periodontitis is intricate. This research examines how social determinants of health relate to periodontitis in U.S. adults, an area that has not been thoroughly studied. METHODS: This study utilized data from the 2009-2014 National Health and Nutrition Examination Survey (NHANES). During each survey cycle, data on self-reported Social Determinants of Health (SDoH) were collected, encompassing employment, family income, food security, education, healthcare access, health insurance, housing instability, and marital status (married or…

Neighbourhood risk factors and spatiotemporal trends for overdoses following cannabis legalization and pandemic restrictions in Toronto, Canada.

Drug & opioid overdoses in Toronto, Canada, have risen substantially in recent years. To explore possible causes, we spatially analyze associations between overdose incidence data over 2019-2022 and select socioeconomic & built-environment variables among Toronto neighbourhoods. Using spatiotemporal analysis, we also assess average area trends and local hotspots before/after two major events: Canada's 2018 legalization of cannabis (2017-2020) and COVID-19 pandemic lockdowns (2019-2022). Previous discussions frame cannabis as a possible alternative to more dangerous drugs, while pandemic lockdo

Psychosocial needs of incarcerated veterans with other than honorable discharge characterizations.

The Veterans Health Administration recently expanded eligibility to allow behavioral health care for veterans with "other than honorable" (OTH) discharge characterizations, a group with unique demographic and psychosocial needs, including overrepresentation in criminal-legal contexts. To guide the integration of these veterans into Veterans Health Administration behavioral health care settings, this study offers initial insight into the needs of veterans with OTH involved in the criminal-legal system. Using data from the 2016 Survey of Prison Inmates, analyses compared incarcerated veterans wi

Patterns of Exposure to Socio-structural Stressors and HIV Care Engagement Among Transgender Women of Color.

Transgender women are disproportionately affected by HIV and experiences of social adversity that may interfere with engagement in care and viral suppression. We used latent class analysis to examine patterns of social adversity and their impact on HIV care continuum outcomes in an urban sample of transgender women of color. Participants (n = 224) were median age 29 and 86% non-Hispanic Black. Lack of resources, unemployment, and housing instability were reported by over 50%, and 41% reported history of incarceration. Latent class analysis identified 2 distinct classes representing higher and …

Bilateral Blindness in a Patient With Vestibular Schwannoma and Empty Sella Syndrome: A Case Report.

We report a rare clinical presentation of a T4b vestibular schwannoma causing severe compression of the brainstem and fourth ventricle, associated with obstructive hydrocephalus and empty sella syndrome, in a patient presenting with profound neurological and ophthalmological deficits, most notably complete bilateral blindness. Radical microsurgical resection was achieved via a lateral suboccipital (retromastoid) approach, resulting in gross-total tumor removal with preservation of most cranial nerves. Postoperatively, the patient developed House-Brackmann grade III facial palsy. Hydrocephalus

Ageing processes in the bonobo gut microbiome mirror human patterns.

Ageing is associated with changes in the human gut bacterial microbiome: with age, it becomes more diverse, intra- and interindividual variability increases and it shows a decline in core bacterial genera and an expansion of rarer genera. While recent evidence in model organisms suggests these patterns may arise through stochastic processes rather than host-driven selection, the role of life-time exposures, or the exposome, remains poorly understood. Here, we characterise age-related gut microbiome dynamics in 165 bonobos (Pan paniscus) from wild and zoo-housed populations, aged 2 to 71 years,
