DNA methylation signatures of treatment response in medication-overuse headache.

BACKGROUND: Medication overuse headache (MOH) causes substantial disability in suffering patients, significantly reducing the quality of life. It may lead to structural and functional brain changes detectable by neuroimaging. Successful and effective treatments can alleviate headache burden, reduce consumption of abused drugs and reverse the biological alterations. Therefore, improvement of therapeutic strategies and optimization of patient stratification to match the individuals who can benefit with certain approaches, is essential. This study investigates DNA methylation (DNAm) associated wi

Characterization of rare genomic structural variants across 2,981 genomes reveals significant involvements in recessive conditions.

Structural variants (SVs) underlie genomic diversity and human diseases; however, current short-read sequencing-based methods experience challenges in precisely identifying and determining SV composition, which often lead to incorrect interpretation and/or missing detection of clinically significant variants. Here, we introduce SCARD (split-read, chimeric read-pairs, and read-depth difference), an integrated approach optimized for the detection and characterization of rare SVs using short-read genome sequencing data. Firstly, we evaluated rare SV (with minor allele frequency <1%) detection per

Equity in genome sequencing for rare disease diagnosis: a cross-sectional analysis of data from the UK 100,000 Genomes Project.

BACKGROUND: Genome sequencing has improved rare disease diagnosis and is now part of routine clinical care in the National Health Service in England. Automated prioritisation pipelines narrow millions of variants per patient to a small subset for clinical review, a process that relies on allele frequency resources that do not fully represent human genetic diversity. We assessed ancestry-related differences in variant prioritisation and diagnostic outcomes in patients from the UK 100,000 Genomes Project. METHODS: We analysed 29,405 rare disease probands with genome sequencing and linked clinic

Whole-genome sequencing resolves biochemical misidentification of Neisseria species from urogenital specimens.

Neisseria meningitidis (Nm) and Neisseria gonorrhoeae (Ng) are human pathogens that sometimes occupy the same anatomical niche. Ng, the causative agent of gonorrhea, infects 87 million individuals annually worldwide and is an urgent threat due to increasing drug resistance. Ng is a pathogen of the urogenital tract and may infect the oropharyngeal or rectal site, often asymptomatically. Conversely, Nm is an opportunistic pathogen. While often a commensal in the oropharyngeal tract, it is also the leading cause of bacterial meningitis with 1.2 million cases globally, causing significant morbidit

Familial occurrence of multiple sclerosis - current state of knowledge.

Multiple sclerosis (MS) is an immune-mediated, inflammatory, and neurodegenerative disease of the central nervous system with a complex, multifactorial etiology. Familial aggregation, along with a higher concordance rate in monozygotic than dizygotic twins, supports a significant, albeit non-Mendelian, genetic contribution to MS susceptibility. The strongest genetic association involves the human leukocyte antigen (HLA) region, particularly HLA-DRB1*15:01, while genome-wide association studies have identified over 200 additional susceptibility signals, most of which implicate immune regulation

The clinical introduction of genetic testing for Alzheimer disease. An ethical perspective.

OBJECTIVE: Primary caregivers should be aware of recent progress in the genetics of Alzheimer disease (AD) and of the clinical and ethical considerations raised regarding the introduction of genetic testing for purposes of disease prediction and susceptibility (risk) analysis in asymptomatic individuals and diagnosis in patients who present clinically with dementia. This statement addresses arguments for and against clinical genetic testing. PARTICIPANTS: The 20 participants were selected by the investigators (S.G.P., T.H.M., A.B.Z., and P.J.W.) to achieve balance in the areas of genetics, co

Dysosmobacter: from a serendipitous discovery to a next-generation beneficial gut commensal linked to health.

Dysosmobacter is a genus of strictly anaerobic, butyrate-producing bacteria of the family Oscillospiraceae that was first described in 2020 with the human faecal type species Dysosmobacter welbionis J115T. The organism was recovered by chance during a targeted attempt to isolate new Subdoligranulum species, a project that failed in its original aim but yielded a bacterium that fulfilled the criteria for an entirely new genus. Since then, D. welbionis has emerged as one of the best-supported examples of a candidate next-generation beneficial microbe. It is carried by roughly two thirds to more

Plasmodium vivax transcriptomics: What is new?

Malaria is the leading human parasitosis and is transmitted through the bite of anopheline mosquitoes infected with parasites of the genus Plasmodium spp. Among the seven species that cause malaria in humans, Plasmodium vivax is the most prevalent species in Latin America. In recent years, there have been an increasing number of reports of clinical complications caused by P. vivax infections, which were previously neglected and underestimated. P. vivax biology remains with large gaps. The emergence of next-generation sequencing technology has ensured a breakthrough in species knowledge. Couple

A bipartite network approach to inferring interactions between environmental exposures and human diseases.

Environmental exposure is a key factor of understanding health and diseases. Beyond genetic propensities, many disorders are, in part, caused by human interaction with harmful substances in the water, the soil, or the air. Limited data is available on a disease or substance basis. However, we compile a global repository from literature surveys matching environmental chemical substances exposure with human disorders. We build a bipartite network linking 60 substances to over 150 disease phenotypes. We quantitatively and qualitatively analyze the network and its projections as simple networks. W

ALDH1A3 loss of function causes bilateral anophthalmia/microphthalmia and hypoplasia of the optic nerve and optic chiasm.

The major active retinoid, all-trans retinoic acid, has long been recognized as critical for the development of several organs, including the eye. Mutations in STRA6, the gene encoding the cellular receptor for vitamin A, in patients with Matthew-Wood syndrome and anophthalmia/microphthalmia (A/M), have previously demonstrated the importance of retinol metabolism in human eye disease. We used homozygosity mapping combined with next-generation sequencing to interrogate patients with anophthalmia and microphthalmia for new causative genes. We used whole-exome and whole-genome sequencing to study

Practical genomics. Out of the lab, into practice.

Mapping individuals' genes could transform health care. A person susceptible to a certain disease could take corrective action years before onset. A doctor could become a lifelong lifestyle coach. Comparing genetic maps across populations could reveal factors that combine to cause diseases. Some hospitals are pushing forward on the genomics front despite daunting obstacles, including cost, fear of misuse of data by insurers, and an inability to share data.

Hearing loss drug discovery and medicinal chemistry: Current status, challenges, and opportunities.

Hearing loss is a severe high unmet need condition affecting more than 1.5 billion people globally. There are no licensed medicines for the prevention, treatment or restoration of hearing. Prosthetic devices, such as hearing aids and cochlear implants, do not restore natural hearing and users struggle with speech in the presence of background noise. Hearing loss drug discovery is immature, and small molecule approaches include repurposing existing drugs, combination therapeutics, late-stage discovery optimisation of known chemotypes for identified molecular targets of interest, phenotypic tiss

Observ-OM and Observ-TAB: Universal syntax solutions for the integration, search, and exchange of phenotype and genotype information.

Genetic and epidemiological research increasingly employs large collections of phenotypic and molecular observation data from high quality human and model organism samples. Standardization efforts have produced a few simple formats for exchange of these various data, but a lightweight and convenient data representation scheme for all data modalities does not exist, hindering successful data integration, such as assignment of mouse models to orphan diseases and phenotypic clustering for pathways. We report a unified system to integrate and compare observation data across experimental projects,

Relationship between genomic risk scores (GRS) and coronary artery calcium (CAC) score: A pilot study.

BACKGROUND: Coronary artery disease (CAD) genomic risk scores (GRS), as FDR202, GRS46K, 1.7M, and MetaGRS, help in assessing cardiovascular related morbidity and mortality. Interventions to adhere to a healthy lifestyle as a means of prevention based on the GRS have a potential to greatly reduce incident CAD event rates. We performed a prospective observational study to see the relationship between GRS and coronary artery calcium (CAC) scoring in individuals who are at risk. METHODS: 104 subjects with mean age 55.1 ± 8.8 years were enrolled and consented and all the participants underwent CAC

Evaluation of Recipients of Positive and Negative Secondary Findings Evaluations in a Hybrid CLIA-Research Sequencing Pilot.

While consensus regarding the return of secondary genomic findings in the clinical setting has been reached, debate about such findings in the research setting remains. We developed a hybrid, research-clinical translational genomics process for research exome data coupled with a CLIA-validated secondary findings analysis. Eleven intramural investigators from ten institutes at the National Institutes of Health piloted this process. Nearly 1,200 individuals were sequenced and 14 secondary findings were identified in 18 participants. Positive secondary findings were returned by a genetic counselo

Isolation and Molecular Profiling of Nuclei of Specific Neuronal Types from Human Cerebral Cortex and Striatum.

Most pathological conditions of the central nervous system do not affect all cell types to the same extent. Delineation of molecular events underlying disease symptoms, including genetic, epigenetic, and transcriptional changes, thus relies on the ability to characterize a specific cell type separately from others. We have developed a methodology for the collection of nuclear RNA and genomic DNA of specific cell types from frozen post-mortem striatum and cerebral cortex. This allows deep transcriptomic profiling of specific cell populations and characterization of their genomes and epigenetic

Genetic factors in the pathogenesis of primary biliary cirrhosis.

Current knowledge of the genetic basis of PBC is at best incomplete and at worst poor. Studies so far may be used as a guide to the pitfalls that await unwary investigators and also in deciding where to look and which genes or systems are most likely to yield informative results. The Human Genome Project has revealed a vast array of polymorphism that is too much to contemplate even with the best of current techniques. The crucial processes are the selection of candidates and study design. The strong genetic associations so far in PBC are with chromosomes 6p21.3 and 2q and include; HLA DRBI*08 …

Long-distance transmission of tmexCD1-toprJ1 positive Klebsiella pneumoniae between humans and food animals.

OBJECTIVES: The tmexCD1-toprJ1 RND efflux pump gene cluster is predominantly identified among Klebsiella pneumoniae (K. pneumoniae) from humans and food animals. However, the mechanism of the long and rapid dissemination of such resistance determinant among strains from humans, food animals, and the environment is unclear. METHODS: K. pneumoniae isolates were collected from two surveillance projects among patients and food animals. Gut microbiome associations in male reproductive endocrine-skeletal physiology: a taxonomic-resolution Mendelian randomization study.

The gut microbiome is increasingly linked to endocrine physiology, but human evidence that individual microbial traits contribute to male reproductive and skeletal phenotypes remains largely observational. Integrated Proteogenomics and Single-Cell Transcriptomics Prioritize Putative Protective Plasma Proteins for Hidradenitis Suppurativa.

Translating hidradenitis suppurativa (HS) genetic susceptibility into actionable targets remains challenging, as most genome-wide association study loci lie in non-coding regions and tissue-level transcriptomics cannot easily distinguish causal drivers from secondary inflammation. In this study, we aimed to prioritize plasma proteins whose genetically predicted levels are causally associated with HS risk and to localize them within human skin at single-cell resolution. We performed two-sample Mendelian randomization (MR) using cis-pQTL instruments for 2923 plasma proteins from the UK Biobank P
