Functional Outcome Of Proximal Humeral Fractures Treated With Philos Plate In Adults.

BACKGROUND: Proximal humeral fractures account for 4-5% of all fractures. Most fractures are minimally displaced and can be managed non-operatively in adults. Displaced and unstable fractures are difficult to manage and should be treated to achieve painless shoulder and full function. Our aim was to evaluate the functional outcome of proximal humerus fractures (Neers classification 2 part, 3 part and 4 part) treated with PHILOS (Proximal humeral internal locking system). METHODS: We retrospectively reviewed 22 patients who had proximal humerus fractures treated with PHILOS plate from Jan 2012

NON-UNIONS AFTER FIXATION OF HUMERAL FRACTURES USING HACKETHAL'S BUNDLE NAILING TECHNIQUE.

OBJECTIVE: The aim of the study was to identify factors contributing to the development of non-union after fixation of diaphyseal humeral fractures using Hackethal's intramedullary nailing technique. METHODS: In the time period from 2001 to 2010 156 patients with diaphyseal humeral fractures were treated surgically using Hackethal's technique. Six of them (3.8%) developed non-union. This group included three women and two men aged 63-69 years and one woman aged 37 years. The following parameters of the patients were recorded: age, gender, comorbidities, substance abuse, mechanism of injury, f

Incidence and functional outcomes of malunion of nonoperatively treated humeral shaft fractures.

We conducted a study to determine the overall incidence and long-term clinical and functional outcomes of patients with malunion after nonoperative management of humeral shaft fractures. Fifteen patients with radiographic malunion (>20° angulation or shortening of :ge;2.5 cm) were identified, and their medical records retrospectively reviewed for information about their injuries and treatment. Long-term outcomes were assessed with a self-reported questionnaire, the DASH (Disabilities of the Arm, Shoulder, and Hand) form, and physical examination. Of 91 study-eligible patients, 15 (16%) had mal

Plate osteosynthesis of humeral diaphyseal fractures associated with radial palsy: twenty cases.

BACKGROUND: Surgical management of humeral fractures associated with radial nerve palsy remains controversial. Some advocate surgical treatment in emergency while others recommend nonoperative treatment. PURPOSE: The aim of the study was to assess nerve recovery in a series of patients treated with open reduction and internal fixation (ORIF) with plate for humeral fracture associated with radial nerve palsy. We hypothesised that surgical treatment with ORIF in emergency allows good nerve recovery. METHODS: We conducted a retrospective study on 20 patients out of 225 humeral shaft fractures o

Complications and outcomes of minimally invasive percutaneous plating for proximal humeral fractures.

BACKGROUND: The minimally invasive plate osteosynthesis (MIPO) technique using periarticular locking plates may be a good option for the repair of displaced proximal humeral fractures. However, axillary nerve complications related to this technique may be underestimated. The purpose of this study is to evaluate the outcomes of the minimally invasive plating, focusing on the complications. METHODS: The records of 21 consecutive patients treated for proximal humerus fractures using the MIPO technique with locking plates were retrospectively reviewed. These patients were treated between March 20

Treatment of proximal humeral fractures with reverse shoulder arthroplasty in elderly patients.

BACKGROUND: Proximal humeral fractures in four or even only three parts, with metaphyseal hinge distances of <8 mm, represent a serious and widely debated problem. Reduction is complex and plating is often instable, especially in elderly patients. Failures, sometimes involving necrosis of the head, are frequent. Hemiarthroplasty has long been used for 3- or 4-part complex fractures, even in young patients, although often with sub-optimal results, due to reabsorption of tuberosities. This complication has partly been overcome with reverse shoulder prostheses which, although more invasive than p

[Osteosynthesis in distal intra-articular humeral fractures. External fixation with motion capacity].

The rising incidence of distal intra-articular humeral fracture in elderly patients associated with osteoporosis frequently leads to difficulties in acute reconstruction and may result in early failure of internal fixation. The biomechanics of the elbow joint and especially the flexor group as well as the brachioradialis muscle as part of the extensor group is analyzed. The complex motion pattern of the transarticular muscle groups increases the load on internal fixation and may affect the interface between implant and osteoporotic bone. An external fixator with motion capacity serves to prote

Functional outcome and complications following PHILOS plate fixation in proximal humeral fractures.

OBJECTIVES: Proximal humeral fractures account for approximately 5% of all fractures. New plating techniques have been developed to improve stability. The purpose of this study was to evaluate functional outcome following plate fixation with the Proximal Humeral Internal Locking System (PHILOS) and to analyze potential implant-related complications. METHODS: The PHILOS plate was used for internal fixation of displaced proximal humeral fractures in 28 patients (20 females, 8 males; mean age 60.7+/-12.9 years). Fractures were caused by low-energy trauma (fall from standing height) in 21 patient

[Follow-up our patients with proximal humeral fractures between 2000 and 2005].

Authors introduced a new method in 2000 for the operative treatment of proximal end of the humerus. Because of the anatomical properties of the shoulder, great emphasis is placed on the protection of the soft tissues of the joint, the minimal invasive operative technique. On the other hand, the osteoporosis, which presents itself in the old age, makes the osteosynthesis more difficult; the proportion of complication is high in spite of the most careful treatment. Authors collected the materials of these 6 years and they have ordered their operated patients back for examination. After a physica

Humeral fractures in the elderly: treatment with a reamed intramedullary locking nail.

In the elderly person, comorbidities combined with lower functional demands tend to indicate non-operative treatment; however, fast functional recovery is mandatory to prevent loss of independency and social ability. This retrospective follow-up study included 40 people with a mean age of 75 (range 60-87) years. They had all received treatment with a reamed intramedullary nail for humeral fracture; 21 were operated after a mean of 6 (range 0-16) days, and 19 after a mean of 28 (range 9-63) days after a primary decision for non-operative treatment. In all, 5 cases (12.5%) were lost to follow-up

[Proximal humeral fractures with angle-stable plate osteosynthesis--is everything better now?].

BACKGROUND: Proximal humeral fractures are common in the elderly as distribution peaks in the 6th and 7th decade. Optimal operative strategy regarding complex proximal humeral fractures is still being discussed controversely. Aim of the study was to evaluate implant associated problems of angle-stable implants in comparison to other established osteosynthetic methods. METHODS: 198 patients with proximal humeral fractures were treated operatively from 2000 to 2004 in our department with a primary angle-stable plate osteosynthesis. 166 patients (98 females and 68 males) were followed up. Supracondylar humeral fractures in children: ten years' experience in a teaching hospital.

Although supracondylar fracture is a very common elbow injury in childhood, there is no consensus on the timing of surgery, approach for open reduction and positioning of fixation wires. We report our ten-year experience between 1993 and 2003 in 291 children. Most fractures (285; 98%) were extension injuries, mainly Gartland types II (73; 25%) and III (163; 56%). Six (2%) were open fractures and a neurovascular deficit was seen in 12 (4%) patients. Of the 236 children (81%) who required an operation, 181 (77%) were taken to theatre on the day of admission. Most (177; 75%) of the operations wer
