[Anti-MDA5 (melanoma differentiation-associated gene 5) antibody and dermatomyositis with rapidly progressive interstitial pneumonia].

Anti-MDA5 antibody is one of the dermatomyositis-specific autoantibodies and anti-MDA5-potsitive patients show characteristic clinical features, such as hypomyositis, high prevalence of acute/subacute interstitial pneumonia (A/SIP) with poor prognosis, hyperferritinemia and elevated hepatobiliary enzyme. We found that serum IL-6, IL-18, M-CSF and IL-10 were significantly higher and serum IL-12 and IL-22 were significantly lower in anti-MDA5-positive patients than in anti-MDA5-negative patients before treatment. Taking together these serological findings, we hypothesized that monocyte and macro…

Pretransplant iron overload may be associated with increased risk of invasive fungal pneumonia (IFP) in patients that underwent allogeneic hematopoietic stem cell transplantation (alloHSCT).

Invasive fungal pneumonia (IFP) has become increasingly common in patients that previously underwent alloHSCT. The aim of this study was to determine the role of hyperferritinemia, via iron overload in invasive fungal pneumonia in patients that underwent alloHSCT. Medical records of 73 patients with pneumonia that underwent alloHSCT were studied retrospectively, whereby a pre-transplantation serum ferritin level measured up to 100 days prior to transplantation of patients with invasive fungal pneumonia (IFP) and non-fungal pneumonia (non-IFP) was compared. Patient records revealed 35 and 38 ca…

[Anemia in malignant tumor diseases. VI. Secondary siderosis caused by the shift of iron, unused for hemoglobin synthesis, into iron stores].

Impaired utilization of iron by the heme synthesizing red cell precursors was investigated in 258 patients with malignant tumors of differen … ted with the malignancy and the stage of the tumor. It could be demonstrated that the degree of the hyperferritinemia paralleled very closely the severity of the anemia becoming more pronounced with increasing tumor mass. This parallelism indicates that the siderosis is pathophysiologically related to the defect of erythropoiesis observed in malignant disease. Sinc …

Navigating the Diagnosis of Atypical Still's Disease in the Absence of the Characteristic Rash.

Although adult-onset Still's disease (AOSD) typically presents with a triad of fever, rash, and arthritis, atypical manifestations can significantly hinder the diagnostic process of fever of unknown origin (FUO). We describe the case of a 57-year-old woman who presented with prolonged fever, serositis, and extreme hyperferritinemia (ferritin levels 117,904 ng/mL), further complicated by aseptic encephalitis. Following a comprehensive diagnostic workup to exclude infectious, neoplastic, and other autoimmune etiologies, the patient was evaluated using the Yamaguchi criteria. Despite the absence …

Which Factors May Contribute to a Successful Outcome in Hemophagocytic Lymphohistiocytosis: A Case Report.

Hemophagocytic lymphohistiocytosis (HLH) is a rare and life-threatening hyperinflammatory syndrome characterized by immune dysregulation and excessive cytokine activation. In adults, HLH is most commonly secondary to infections, malignancies, autoimmune diseases, or drug exposure. We report the case of a 50-year-old woman undergoing adjuvant treatment for human epidermal growth factor receptor 2 (HER2)-positive breast cancer who developed acute hypoxemic respiratory failure, neurological impairment, bicytopenia, marked hyperferritinemia, hypertriglyceridemia, and hepatomegaly. Extensive infect…

Hemophagocytic lymphohistiocytosis is characterized by tissue factor-driven coagulopathy, endothelial injury, and immunothrombosis.

Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome marked by defective cytotoxic lymphocyte function. While its immunologic features are recognized, the contribution of immunothrombosis is less defined. We analyzed plasma from 12 HLH patients with viral, lymphomatous, and autoimmune etiologies. Hallmark features included hyperferritinemia, cytopenias, hypertriglyceridemia, elevated sIL-2Rα, and acute-phase reactants. Cytokine profiling showed high IL-6, IL-8, IL-10, TNF-α, CRP, and IL-18, with variable IFN-γ. Coagulation studies revealed compensated DIC, …

Secondary Hemophagocytic Lymphohistiocytosis in a Patient With Crohn's Disease Receiving Infliximab: A Diagnostic Challenge.

Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome caused by uncontrolled immune activation. While pediatric HLH is well-recognized, adult cases often mimic sepsis or autoimmune flares, delaying diagnosis. Inflammatory bowel disease (IBD) and its immunosuppressive treatments, including infliximab, have been increasingly linked to HLH. We report a 65-year-old man with Crohn's disease on infliximab who presented with persistent fever, chills, night sweats, headaches, and bradycardia. Laboratory testing revealed pancytopenia, hyperferritinemia (32,531 …

Parvovirus B19-Induced Aplastic Crisis and Hemophagocytic Lymphohistiocytosis in a Child With Hereditary Spherocytosis.

Parvovirus B19 (PVB19) is a common virus that usually causes a mild and self-limiting illness. However, in patients with hereditary spherocytosis (HS), it can lead to severe complications such as aplastic crisis and, rarely, hemophagocytic lymphohistiocytosis (HLH). We report a six-year-old boy with HS who presented with persistent fever and severe anemia. Laboratory tests revealed a progressive pancytopenia, including a sudden drop in hemoglobin from 10 to 6.8 g/dL, hyperferritinemia (5,129 ng/mL), elevated soluble IL-2 receptor (1,179 U/mL), mild hypofibrinogenemia, and positivity for PVB19 …

Effects of Tocilizumab on Inflammation and Iron Metabolism in Critically Ill Patients with COVID-19.

COVID-19 produces cytokine-mediated persistent inflammation and is associated with elevated iron stores and low circulating iron. It is believed that central to the pathophysiological mechanism is interleukin 6 and hepcidin. A state of iron overload, termed hyperferritinemia, and inflammatory anemia take place. Both conditions are linked to a worse result in critically ill patients. Blocking the interleukin 6-hepcidin pathway with Tocilizumab could present favorable outcomes. The aim of this study was to evaluate if Tocilizumab influences survival, the occurrence of sepsis, anemia and transfus…

Type 4B hereditary hemochromatosis due to heterozygous p.D157A mutation in SLC40A1 complicated with hypopituitarism.

Hemochromatosis is a clinical syndrome characterized by iron overload in various organs. We present here a case of type 4 hereditary hemochromatosis due to heterozygous mutation in SLC40A1 gene (p.D157A). SLC40A1 encodes ferroportin, a macromolecule only known as iron exporter from mammalian cells. He first presented symptoms correlated with hypopituitarism. Furthermore, marked hyperferritinemia and high transferrin saturation were revealed in combination with the findings of iron overload in the liver, spleen and pituitary gland by computed tomography and magnetic resonance imaging. Liver bio…

Hemophagocytosis in a case with Crimean-Congo hemorrhagic fever and an overview of possible pathogenesis with current evidence.

Hemophagocytic lymphohistiocytosis (HLH) is a clinicopathologic condition characterized by high fever, hepatosplenomegaly, cytopenia, hyperferritinemia, and increased hemophagocytic macrophage proliferation and activation in the reticuloendothelial system. Primary HLH is familial and is a fatal disease that begins during early childhood. Secondary HLH may be acquired after intense activation of the immune system due to infection. Clinical and biologic symptoms result from cytokines secreted by T-lymphocytes and macrophages. Subtypes of primary HLH are caused by genetic defects in several cell …
