Acute Respiratory Distress in a Hospitalized Patient Revealing Cervical Diffuse Idiopathic Skeletal Hyperostosis (DISH).

Diffuse idiopathic skeletal hyperostosis (DISH) is a clinical condition marked by calcification and ossification of primarily ligaments of the spine and peripheral entheses. It predominantly affects older adults, with a prevalence rate of up to 42% in individuals over 65 years. It commonly presents with dysphagia. Stridor secondary to osteophyte compression has rarely been documented. This report discusses a patient who was admitted for alcohol intoxication following binge drinking and subsequently developed respiratory distress and unexplained stridor during hospitalization. He was intubated …

[A clinical case of upper airway obstruction, dysphagia caused by diffuse idiopathic skeletal hyperostosis].

Diffuse idiopathic skeletal hyperostosis (DISH, Forestier's disease) is a rare non-inflammatory degenerative-dystrophic disease of the musculoskeletal system, characterized by calcification of ligaments and tendons, in particular the anterior longitudinal ligament. It is rare, more often among men over 60 years old. When the cervical spine is affected, DISH manifests itself as neck pain, stiffness of movement, dysphagia and, in some cases, difficulty breathing. The main goal of surgical treatment of Forestier's disease is the removal of the osteophyte complex, however, with severe compression …

Dysphagia with recurrent severe aspiration due to cervical diffuse idiopathic skeletal hyperostosis (Forestier's disease).

Dysphagia is a common symptom in patients presenting to the gastroenterologist. However, extraluminal causes of dysphagia may be missed by endoluminal diagnosis alone. This case report presents a 72-year-old man with slowly progressive dysphagia that occurred with the ingestion of certain solid foods and was intermittently associated with severe aspiration. Esophagogastroduodenoscopy and barium swallow study were both normal. However, a conventional x-ray and computed tomography (CT) scan of the neck revealed diffuse idiopathic skeletal hyperostosis (Forestier's disease) of the anterior cervic…

Long-term results after surgical treatment of diffuse idiopathic skeletal hyperostosis (DISH) causing dysphagia.

Diffuse idiopathic skeletal hyperostosis (DISH) is a non-inflammatory, systemic skeletal disease. The associated formation of anterior cervical osteophytes can cause severe dysphagia, so the osteophytes have to be surgically removed. Because the clinical syndrome is rare, long-term outcome after surgical therapy is likewise scarce. In this retrospective, single-center study, five consecutive patients with DISH causing dysphagia and following resection of osteophytes between 2005 and 2015 were included. Patient and surgical reports were evaluated regarding surgery-related and postoperative comp…

Diffuse idiopathic skeletal hyperostosis (DISH) - A common but less known cause of back pain.

Diffuse idiopathic skeletal hyperostosis (DISH) is a systemic non-inflammatory disease of an unknown aetiology. It is characterized by ossification of the enthesis and may significantly affect the quality of life of the affected individuals. Despite the fact that this disease was first described more than 60 years ago and is a relatively common condition in the society, its awareness among clinician is still limited. An awareness of this entity and its mimicking conditions is essential so as to reach to an early diagnosis and avoid unnecessary investigations and treatment.

Thoracic spondylolisthesis and spinal cord compression in diffuse idiopathic skeletal hyperostosis: a case report.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis has long been regarded as a benign asymptomatic clinical entity with an innocuous clinical course. Neurological complications are rare in diffuse idiopathic skeletal hyperostosis. However, if they do occur, the consequences are often significant enough to warrant major neurosurgical intervention. Neurological complications occur when the pathological process of ossification in diffuse idiopathic skeletal hyperostosis extends to other vertebral ligaments, causing ossification of the posterior longitudinal ligaments and/or ossification of the …

CT-based morphological analysis of spinal fractures in patients with diffuse idiopathic skeletal hyperostosis.

PURPOSE: To clarify correlations between spinal fracture and delayed paralysis in patients with diffuse idiopathic skeletal hyperostosis (DISH) using computed tomography (CT) with multiplanar reformatting (CT-MPR). DISH increases susceptibility to unstable spinal fractures, leading to neurological deterioration. The pathomechanism of the neurological injury is unclear. METHODS: This multicenter retrospective study included 42 DISH patients (32 male; 10 female) treated for 45 spinal fractures during a 5-year period. The mean age at the time of injury was 77.1 ± 10.1 years. The cause of injury,…

Unilateral compressive optic neuropathy due to skull hyperostosis secondary to nutritional vitamin A deficiency.

We report a 17-year-old boy who presented with a chronic left unilateral optic neuropathy. Computerized tomography and magnetic resonance imaging demonstrated compression of the left optic nerve due to skull hyperostosis. He was found to be profoundly vitamin A deficient secondary to an unusual diet consisting predominantly of potato chips and crisps. Skull hyperostosis with cranial neuropathies and other neurological abnormalities has been described in growing animals fed vitamin A deficient diets but has not been previously reported in humans.

Fluctuating mixed-type hearing loss associated with synovitis-acne-pustulosis-hyperostosis-osteomyelitis (SAPHO) syndrome.

Synovitis, acne, palmoplantar pustulosis (PPP), hyperostosis, and osteitis (SAPHO) are the characteristic features of SAPHO syndrome. A 53-year-old woman had been treated for PPP for 2 years. She complained of hearing loss in the right ear, and otitis externa was diagnosed. The pure-tone audiogram (PTA) indicated mild hearing loss in the right ear, and her hearing continued to deteriorate despite recovery from inflammation. Her tympanogram was of the As-type, and acoustic reflex was absent in the right ear. A computed tomography (CT) scan revealed bilateral normal ossicles and cochleas. Bone s…

Diffuse idiopathic skeletal hyperostosis (DISH) and situs viscerum inversus. Report of a single case.

An uncommon case of diffuse idiopathic skeletal hyperostosis (DISH) is described in a patient with situs viscerum inversus. The radiographic features typical of DISH are present only on the left side of the thoracic spine. This finding confirms the role of the descending thoracic aorta in preventing the location of DISH on the left side.

[New cases of familial generalized cortical hyperostosis with dominant transmission (Worth's type) (author's transl)].

A case of two families with a total of 5 cases of generalized cortical hyperostosis of the cortex of the long bones is clinically latent. It is not accompanied by any osseous fragility and is usually discovered by chance. Mandibular hypertrophy can occur but is not constant. It must be differentiated from the recessive form described by Van Buchen, which is more severe and involves extensive and progressive osteocondensation which can lead to paralysis of the cranial nerves. The small number of cases published (23) leads one to suppose that its diagnosis is frequently missed, undoubtedly becau…

Diffuse Idiopathic Skeletal Hyperostosis (DISH): Potential Links Between Gut Microbiota and Spinal Degenerative Diseases.

Diffuse Idiopathic Skeletal Hyperostosis (DISH) is a systemic, non-inflammatory skeletal disorder characterized by progressive calcification and ossification of spinal ligaments and entheses. Its strong associations with aging, obesity, type 2 diabetes, and metabolic syndrome raise the possibility that metabolic and inflammatory pathways may contribute to disease development. The gut microbiota participates in nutrient metabolism, short-chain fatty acid production, bile-acid transformation, intestinal-barrier maintenance, and immune regulation; these functions provide a plausible framework for…

Severe Hyperostosis Frontalis Interna in a 91-Year-Old Female Cadaver: A Case Report.

Hyperostosis frontalis interna (HFI) is characterized by benign thickening of the frontal bone and most commonly affects postmenopausal elderly women. While the exact cause remains unknown, its prevalence is thought to be multifactorial. This case report presents a discovery of extensive HFI during routine dissection of a 91-year-old female cadaver in the Clinical Anatomy Laboratory of Sam Houston State University College of Osteopathic Medicine. The patient's (donor) death was reported as systolic heart failure, and complete bilateral invasion of the frontal and temporal bones, with lesser in…

Spinal Cord Stimulator Implantation in a Patient With Diffuse Idiopathic Skeletal Hyperostosis: Case Report.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is a noninflammatory skeletal disorder causing calcifications and ossification of ligaments and tendons. We present a patient with undiagnosed DISH in whom a spinal cord stimulator (SCS) provided adequate pain relief while the disease progressed to become debilitating. CASE REPORT: The patient is a 66-year-old woman with an SCS placed for lower back pain due to degenerative changes. The patient presented to the emergency department with progressive right-sided lower extremity weakness and a 3-day history of urinary incontinence. Comp…
