[Primary hyperparathyroidism associated with sarcoidosis].

Co-existence of primary hyperparathyroidism and sarcoidosis has been described in less than 50 cases since 1958. The mechanism of such an association is unknown. However, the steroid suppression test may be helpful in differentiating these two disorders. The hypercalcemia of sarcoidosis should be corrected with the steroid test. The measurement of parathyroid hormone (PTH) in serum is also helpful, for PTH levels are normal or reduced in sarcoidosis associated with hypercalcemia whereas in patients with primary hyperparathyroidism PTH levels are elevated.

[Persistent and recurrent primary hyperparathyroidism].

Aim of this study is to evaluate clinical, radiological and therapeutic aspects of patients who have persistent or recurrent primary hyperparathyroidism. First, it is essential to reconfirm the diagnosis. Then, the previous operative and pathology reports must be reviewed. In these patients, it is important to use preoperative localization studies. When the results of the various localization tests were combined, the site of the elusive remaining parathyroid tumor could be identified in more than 80%, enabling a focused surgical approach which not only saves time, but also improves the success…

Primary hyperparathyroidism--an Indian study.

Twenty eight patients of sporadic primary hyperparathyroidism seen over a period of 10 years were studied. There were 18 females and 10 males with a mean age of 35.9 years. Bone involvement was the commonest clinical presentation (90%) followed by renal involvement (65%) and more than half the patients (54%) had involvement of both the skeletal and renal systems. The tumor was clinically palapable in six patients. Thalliumtechnetium subtraction scan had a sensitivity of 87% followed by computerised tomography (70%), and ultrasound (65%) in diagnosing parathyroid pathology. All the patients und…

[Trabecular connectivity in primary hyperparathyroidism].

By bone histomorphometry, 10 cases of primary hyperparathyroidism (PHPT) were used to study the trabecular connectivity, which in a few literatures was mentioned to be reserved somehow. Our data suggest that in comparison with the normal control (10 subjects) the nodes (N.Nd), the number of terminus (N.Tm), the total strut length (TSL), the length between the nodes (Nd.Nd), trabecular separation (Tb.Sp) and trabecular thickness (Tb.Th) were significantly increased while no significance was found in N.Nd/N.Tm, Tm.Tm and TBV. The correlation analyses indicate that there is no correlation between…

Over a period of 3 months a 51-year-old woman developed a depressive syndrome with loss of memory, inability to find words, impaired fine mo … lustrates that even in seemingly isolated neuropsychiatric changes an organic disease, like primary hyperparathyroidism, needs to be thought of.

[Chronic pancreatitis and primary hyperparathyroidism].

The frequency of acute or chronic pancreatitis in primary hyperparathyroidism has decreased from the former 5-10% to 1-2% thanks to earlier diagnosis and operative treatment. Chronic pancreatitis, which occurs only in prolonged primary hyperparathyroidism, should therefore virtually disappear. We investigated this topic in a prospective long term study of chronic pancreatitis (1963-1992). Over the last three decades 336 patients with chronic pancreatitis have been studied at regular intervals. 245 suffered from alcohol-induced (84% with calcifications) and 91 from non-alcohol-induced chronic p…

[Primary hyperparathyroidism in 2 adolescents].

The authors report on two cases of primary hyperparathyroidism (PHP) in adolescents. In a 15-year-old boy, the clinical picture included skeletal, dental, renal and psychological manifestations. In the second case, the first symptom was urolithiasis at age 12; following a period without any important clinical event, the diagnosis was established when the patient was 18. In both cases, increased calcium and low phosphorus plasma levels associated to moderately elevated plasma parathyroid hormone levels were noted. CT scan and ultrasounds were normal but MRI showed an enlarged gland in the secon…

[Primary hyperparathyroidism. Pathogenesis--diagnosis--therapy].

A report is given on 33 patients with primary hyperparathyroidism who underwent surgical treatment in our institution in the period from 1980 to 1989. In 57 per cent of the cases a hypercalcaemic syndrome was present. The primary hyperparathyroidism manifested itself in 75 per cent at the kidneys in the form of nephrolithiasis. One third of the patients showed a renal manifestation form exclusively. Remarkable is the relatively high number of exclusively gastrointestinal manifestations (11 per cent). In all patients the diagnosis was established on the coincidence of hypercalcaemia and increas…

Primary hyperparathyroidism: problems in management.

Most hyperparathyroidism is subclinical, with no complaints of bone pain, constipation, mental confusion, or depression, no skeletal findings on x-ray, and no history of kidney stones. Routine hyperparathyroidectomy for asymptomatic hypercalcemia, with normal bone density and normal calciuria, particularly with moderate elevations of serum calcium, is now generally rejected.

Brown tumor and secondary hyperparathyroidism.

Brown tumor is a focal, bony lesion of hyperparathyroidism that results from parathyroid hormone on bone increasing osteoclastic activity with bone resorption and trabecular fibrosis. This leads to microfractures and hemorrhage and the appearance of brown tumors, which are seen most commonly in primary hyperparathyroidism and less frequently in secondary hyperparathyroidism. Rarely do these tumors involve the orbit. We report the sixth case, to our knowledge, of orbital involvement, in a patient with chronic renal failure (secondary hyperparathyroidism) and review the literature.

MRI in hyperparathyroidism requiring reoperation.

A 34-year-old woman presented with persistent hyperparathyroidism (HPT) following primary surgical removal of a hypertrophied parathyroid gland. Reevaluation ultrasound (US), computed tomography (CT), and parathyroid scintigraphy studies were negative for parathyroid adenoma. Magnetic resonance imaging (MRI) clearly identified an ectopic parathyroid adenoma within the lateral carotid sheath. Uncomplicated surgical extirpation of the adenoma was accomplished. We discuss the role of US, CT, parathyroid scintigraphy, and MRI in recurrent HPT.

[Primary hyperparathyroidism. Mechanisms of hypercalcemia].

In primary hyperparathyroidism, hypercalcaemia is due to inappropriate hypersecretion of parathormone (PTH). Yet, the intestinal or osseous origin of the excess in plasma calcium and the symptoms of the disease are largely conditioned by vitamin D reserve and metabolism. In cases with sufficient vitamin D reserve and normal metabolism, the primary disorder is hyperabsorption of calcium by the intestine, and there is a risk of renal stone formation. In patients with vitamin D deficiency, there is a significant increase of bone resorption accompanied by osteoarticular symptoms. In addition, othe…

Primary hyperparathyroidism and monoclonal gammopathy.

Coexistent primary hyperparathyroidism and monoclonal gammopathy, although rare, has been reported previously by a number of investigators. We report four patients with such an occurrence who were seen between 1976 and 1988. Another patient with primary hyperparathyroidism also had multiple myeloma and was in remission for 12 years. These patients represent approximately 1% of the 386 patients with primary hyperparathyroidism seen during the same 12-year period. Although several mechanisms have been proposed to explain this concurrence, we believe it is the result of a chance occurrence. A rev…

[Hyperparathyroidism simulating severe hypercalcemia syndrome].

The case history of a 65 year old female patient has been reported here by the authors. The patient was admitted to the Intensive Therapy Un … raphic examination and the elevated level of parathormone in blood established the diagnosis of the hyperparathyroidism. The surgical resection of parathyroidic adenoma yielded a complete recovery of the patient. The authors call the attention to the significance of the clinical signs in the diagnosis of the disease.
