OBJECTIVE: Sucroferric oxyhydroxide, a novel iron-based phosphate-binder, has been shown to have beneficial effects in lowering serum phosph … roferric oxyhydroxide in hemodialysis patients.
RESULTS: Hemodialysis patients, receiving existing hyperphosphatemia drugs with insufficient benefit, were administered sucroferric oxyhydroxide with/without calcium carbonate for 16 weeks. Serum phosphorus level declined rapidly in Week 8 (p < 0.0001) and this decrease persisted until Week 16 (p < 0.0001). FGF23 dec …

Vascular calcification resulting from hyperphosphatemia is a major determinant of mortality in chronic kidney disease (CKD). Vascular calcification is driven by aldosterone-sensitive osteogenic transformation of vascular smooth muscle cells (VSMCs). We show that even in absence of exogenous aldosterone, silencing and pharmacological inhibition (spironolactone, eplerenone) of the mineralocorticoid receptor (MR) ameliorated phosphate-induced osteo-/chondrogenic transformation of primary human aortic smooth muscle cells (HAoSMCs). High phosphate concentrations up-regulated aldosterone synthase (C…

Hyperphosphatemia is a driving force in the pathogenesis of vascular calcification (VC) and secondary hyperparathyroidism associated with renal failure. To test for the possible contribution of parathyroid hormone (PTH) to cardiovascular calcification, we removed the parathyroid glands from rats but infused synthetic hormone at a supraphysiologic rate. All rats were pair-fed low, normal, or high phosphorus diets and subjected to a sham or 5/6 nephrectomy (remnant kidney). Control rats were given a normal diet and underwent both sham parathyroidectomy and 5/6 nephrectomy. Heart weight/body weig…

In hemodialysis patients, it remains unclear whether patient characteristics influence the clinical impacts of changes in serum mineral meta … ACVD or DN. These findings indicate that PS should be considered in treating mild hypercalcemia or hyperphosphatemia in hemodialysis patients. Moreover, stringent management of hypercalcemia and hyperphosphatemia in patients with a history of ACVD or DN might be associated with a better prognosis.

PURPOSE OF REVIEW: Renal bone disease has significant detrimental effects on both cardiovascular and bone health. It is important to understand that the pathophysiologic mechanisms are different from traditional causes of bone disease; and thus, the monitoring and treatment of this disease process requires special attention. Although new guidelines are overdue, progress has been made in the treatment of hyperphosphatemia as well as investigational therapies for renal osteodystrophy.
RECENT FINDINGS: At a time when the treatment of hyperphosphatemia in chronic kidney disease was once diet and …

BACKGROUND: Treating hyperphosphatemia is a tenet of dialysis care. This trial assessed the safety and efficacy of tenapanor for the management of hyperphosphatemia.
METHODS: In this 52-week phase 3 study (NCT03427125), participants receiving maintenance dialysis with both hyperphosphatemia (serum phosphate 6.0-10.0 mg/dl) and a 1.5 mg/dl increase after phosphate binder washout were randomized (3:1) to tenapanor 30 mg twice daily for 26 weeks (randomized treatment period) or sevelamer carbonate (52-week safety control). Participants completing 26 weeks of treatment with tenapanor were rerando…

Sucroferric oxyhydroxide (SFOH) is a non-calcium, iron-based phosphate binder indicated for the treatment of hyperphosphatemia in adult dialysis patients. Studies in Japan about the side effects of SFOH treatment indicate that the incidence of diarrhea (25%) is greater while that of constipation (2.9%) is lesser in comparison to that observed upon treatment with an existing phosphate binder. In the present study, the effect of treatment with a combination of the existing phosphate binders and SFOH on the serum phosphorus level and digestive symptoms was observed in hemodialysis patients with h…

Therapeutic apheresis and dialysis : official peer-reviewed journal of the International Society for Apheresis, the Japanese Society for Apheresis, the Japanese Society for Dialysis Therapy2006eng
Secondary hyperparathyroidism (SHPT) leads not only to bone disorders, but also to cardiovascular complications in long-term dialysis patients. Conventional treatment with calcium (Ca) supplement, phosphate (P) binders and active vitamin D analogs lead in part to amelioration of SHPT, but are simultaneously associated with unacceptable side-effects, including hypercalcemia, hyperphosphatemia, and increased Ca x P products, which are the risk factors for cardiovascular disease in dialysis patients. Conventional treatment has been unable to facilitate the attainment of optimal management of SHPT…

Cardiovascular disease is a major cause of morbidity and mortality in adult patients with end-stage renal disease receiving maintenance dialysis. Coronary artery calcifications (CAC) contribute to the high prevalence of cardiac disease and are associated with hyperphosphatemia, an elevated calcium-phosphorus product (CaxP), and prolonged time on dialysis. Chronic inflammation and malnutrition are also associated with an increased risk for development of cardiac calcifications. Young adults receiving maintenance dialysis develop cardiac calcifications at a degree out of proportion to healthy ad…

A case of Burkitt's lymphoma cell leukemia with recurrent hypophosphatemia that coincided with tumor growth, and hyperphosphatemia during tumor lysis is presented. Redistribution of body phosphorus with increased uptake by the rapidly replicating tumor cells is suggested. Tumor genesis syndrome seems to us to be a proper term to include metabolic abnormalities that result directly from tumor growth.

Immediate and persistent hypophosphatemia following subtotal parathyroidectomy, despite discontinuation of phosphate binders, developed in three chronic hemodialysis patients. Although the serum phosphorous level is regularly reduced by parathyroidectomy in such patients, prolonged hypophosphatemia has not previously been reported. This observation supports the concept that parathyroid overactivity in end-stage renal disease is a major determinant of hyperphosphatemia.

We describe a case of calciphylaxis in a 47-year-old man with alcohol-induced end-stage liver disease and acute renal failure secondary to hepatorenal syndrome. Possible contributing factors included transiently impaired renal function, protein C and S deficiencies, elevated calcium-phosphate product, hyperphosphatemia, low serum albumin, repeated albumin infusions, and elevated alkaline phosphatase level.

We report three patients presenting symptoms of tetany at eleven months, five years and seven years. They had hypocalcemia hypomagnesemia, hyperphosphatemia and elevated phosphate tubular reabsorption. PTH serum levels were decreased and cAMP urinary excretion was very low. Clinical and biological findings suggest hypoparathyroidism, but due to peculiar phenotype of one patient differential diagnosis is established.

INTRODUCTION: Pseudohypoparathyroidism (PHP) is an uncommon metabolic bone disorder characterized by biochemical hypocalcemia, hyperphosphatemia and raised parathyroid hormone (PTH), and target tissue unresponsiveness to the biological actions of PTH. In addition, many patients with PHP exhibit a distinctive constellation of developmental and skeletal defects.

Fibroblast growth factor 23 (FGF23) is a circulating protein that regulates the renal reabsorption of phosphate and also inhibits 1-alpha-hydroxylase production. In adults FGF23 is increased in chronic kidney disease (CKD) and is an important prognostic factor for cardiovascular morbidity. In order to gain insight into the role of FGF23 and other biochemical variables of bone metabolism in children we studied 69 patients at different stages of CKD. FGF23 was found to be significantly elevated in stage 3 compared with stages 1 and 2 of CKD, preceding significant hyperphosphatemia in stage 4 dis…

In maintenance hemodialysis patients, efficient removal of phosphate during the hemodialysis session is crucial for the control of hyperphosphatemia and other frequent abnormalities of mineral metabolism. A mathematical model that integrates different distribution compartments and forms of phosphate may provide deeper insight and elucidation of observed kinetics of phosphate levels in plasma. Three mathematical models were constructed to describe the kinetics of phosphate during a one-week cycle of three dialysis sessions. The models included the extracellular and intracellular compartments fo…
