Drug hypersensitivity syndrome caused by minocycline.

BACKGROUND: Minocycline is a commonly prescribed drug for the treatment of acne. Its use is generally not associated with systemic side effects. OBJECTIVE: To describe a case of minocycline-induced drug hypersensitivity syndrome in a 20-year-old Japanese woman. METHODS AND RESULTS: Following 2 months of minocycline treatment, the patient developed skin lesions composed of exudative maculopapules, purpuratous macules, and target-like, erythema multiforme-like plaques over most of her body. In addition, she had fever, abnormal liver function tests, eosinophilia, and atypical lymphocytosis. Lab…

Severe hypersensitivity pneumonitis associated with anagrelide.

OBJECTIVE: To report a case of severe life-threatening hypersensitivity pneumonitis temporally associated with the use of anagrelide in a patient with myeloproliferative disorder. CASE SUMMARY: A 60-year-old white woman with chronic myeloid leukemia who had been treated with hydroxyurea for 7 years was offered anagrelide to control thrombocytosis. She developed severe hypersensitivity pneumonitis soon after the drug was initiated and required intubation and mechanical ventilation. A high-resolution computed tomography scan of the chest demonstrated extensive multifocal ground glass attenuatio…

Anticonvulsant hypersensitivity syndrome with fatal outcome.

Anticonvulsant hypersensitivity syndrome is a disease characterized by multisystemic involvement, fever, lymphadenopathy, mucocutaneous rash, hypertransaminasemia and peripheral eosinophilia. This rare syndrome seems to be related to arene oxide metabolites of aromatic anticonvulsants (phenytoin, phenobarbital and carbamazepine). Anticonvulsant hypersensitivity seems to be much more aggressive in patients undergoing concomitant radiotherapy. We report a case of anticonvulsant hypersensitivity syndrome developing toxic epidermal necrolysis with fatal outcome in a patient receiving cranial irrad…

Type IV hypersensitivity to vitamin K.

The day after intramuscular injection of vitamin K1 (phytomenadione) into her thigh, a 27-year-old-woman with normal liver function developed a relapsing and remitting eczematous reaction localized to the injection site, and later a further eczematous reaction under an adhesive dressing (Duoderm). On patch testing, she was positive to vitamin K1 and cross-reacted to vitamin K4; she was also positive to colophonium and to ester gum rosin, the dressing adhesive. Recurrent angioedema persisted for several months and, 2 years later, symptoms were still occurring over the injection sites. Structure

Carbamazepine hypersensitivity syndrome mimicking mycosis fungoides.

We report a case of a patient who experienced a generalized skin rash with systemic involvement 2 months after beginning carbamazepine treatment for trigeminal neuralgia. Skin biopsy specimens suggest mycosis fungoides. Complete remission of the clinical and pathologic changes after drug discontinuation and the positive result of carbamazepine from an epicutaneous test suggest a diagnosis of pseudolymphoma due to carbamazepine.

Chronic beryllium disease: a hypersensitivity disorder.

Chronic beryllium disease (CBD) is one of two pulmonary syndromes caused by environmental exposure to beryllium. Acute beryllium disease was … ssociated with a clear-cut dose-response curve, and was associated with a granulomatous reaction, a hypersensitivity to beryllium was suspected as the cause. The hypothesis that CBD was due to hypersensitivity was not proven until the 1980s when samples of bronchoalveolar cells obtained by bronchoscopy demonstrated that not only did every individual with CBD have …

Dental gold alloys and contact hypersensitivity.

A total of 172 patients referred to the Norwegian National Adverse Reaction Group were patch-tested with a dental series. Of these, 25% showed a positive reaction to gold sodium thiosulfate or potassium dicyanoaurate. There was a statistically significant correlation (p=0.0019) between the presence of dental gold and a positive patch test to gold. There was a statistically significant correlation between ear piercing and a positive patch test to gold (p=0.04). In most cases, we did not find clinical correlates to positive patch tests to gold. 2 patients with objective and subjective oral/perio

Hypersensitivity myocarditis associated with ephedra use.

BACKGROUND: Ephedrine has previously been described as a causative factor of vasculitis but myocarditis has not yet been associated with eit … l (EFN 50%) and the patient asymptomatic. CONCLUSION: Ephedra (Ma Huang) is the suspected cause of hypersensitivity myocarditis in this patient due to the temporal course of disease and its propensity to induce vasculitis.

[Fatal outcome in allopurinol hypersensitivity syndrome].

A 76 year-old otherwise healthy man was treated with allopurinol after a single episode of gout. He developed allopurinol hypersensitivity syndrome with epidermal necrolysis, dermal vasculitis, impaired renal function, fever, gastrointestinal bleeding, and possibly pulmonary vasculitis. The outcome was lethal. Controlling allopurinol therapy according to renal function is emphasized.

Mercurochrome allergy. Immediate and delayed hypersensitivity.

We describe eight patients suffering from Mercurochrome allergy. Patch and prick tests were carried out with the following organic and inorganic mercury compounds: thimerosal, Mercurochrome, phenylmercuric acetate, phenylmercuric nitrate, metallic mercury, and mercuric chloride, and with sodium fluorescein. Two patients had an anaphylactic reaction a few minutes after application of Mercurochrome. The prick tests with Mercurochrome were positive and they were negative with the other tested products. All patch tests were negative. In the other six patients, the clinical picture was local eczema

Nasal hypersensitivity and recurrent adenoids disease.

Authors studied nasal hypersensitivity (NH) behaviour on 41 subjects suffering from recurrent adenoids disease during their paediatric age. All subjects underwent clinical history, ENT examination, skin-test, RAST and non-specific nasal provocation test with histamine and assessment of the amount of sneezes. The results showed a familial transmission of the NH in 53.6% of cases, a hypogammaglobulinemia in 14.6% of cases. NH was present in 57% of subjects, much more with higher number of phlogosis, with higher possibility of familial transmission and with premature socialisation. These data sho…

Phenytoin hypersensitivity syndrome: a case report.

We report a life-threatening case of phenytoin hypersensitivity syndrome, a rare reaction primarily characterized by fever, skin eruptions, lymphadenopathy, eosinophilia, and hepatotoxicity. The reaction developed in a 27-year-old black man with surgically corrected tetralogy of Fallot who was treated with phenytoin for one month for exercise-induced sustained ventricular tachycardia. Phenytoin therapy was discontinued and the patient was treated with intravenous methylprednisolone; the therapy was later converted to oral prednisone. Clinical improvement was noted five days after admission and…

Topical zinc and delayed hypersensitivity responsiveness.

The ability of topical zinc to enhance the DHR to skin test antigens may be a simple method of evaluating functional zinc status. Additional testing is required to determine the sensitivity and specificity of this technique. In addition to zinc, other nutrient deficiencies such as protein deficiency may suppress DHR. Because of this and because of the variability among individuals in degree of reactiveness, each patient should serve as his/her own control, with zinc-treated and nontreated skin test sites being evaluated simultaneously.

Acute eosinophilic pneumonia: a hypersensitivity phenomenon?

A previously healthy young man presented with acute respiratory distress and diffuse bilateral infiltrates on chest radiograph. Eosinophilic pneumonia was diagnosed by bronchoalveolar lavage and confirmed by transbronchial lung biopsy. There was no evidence of an infectious etiology, and the patient rapidly improved with corticosteroid therapy. Most cases of eosinophilic pneumonia reported previously have followed a chronic course. The case presented here was acute in onset, suggesting a hypersensitivity reaction. High levels of bronchoalveolar lavage eosinophils indicate the diagnosis but not…

Drug hypersensitivity causing granulomatous interstitial nephritis.

In association with treatment for pharyngitis, a 47-year-old white man developed a systemic illness with fever, myalgia, episcleritis, hemoptysis, pleurisy, eosinophilia, and renal impairment. Renal biopsy revealed granulomatous interstitial nephritis, which resolved due to no specific treatment other than withdrawing all medication. Both the severe systemic manifestations and spontaneous recovery are unusual in association with drug-induced granulomatous nephritis. The drugs that may have caused the reaction included dihydrocodeine, phenylpropanolamine, erythromycin, and amoxycillin. Although

Phenytoin hypersensitivity hepatitis and mononucleosis syndrome.

We present a patient who developed a mononucleosis syndrome 6 weeks after starting phenytoin therapy for a seizure disorder. Improvement followed initial discontinuation of the phenytoin, but near-fatal hepatic necrosis resulted from intravenous phenytoin rechallenge. A serious reaction with a 18% overall mortality, phenytoin hepatitis may closely resemble infectious mononucleosis with fever, pharyngitis, lymphadenopathy, and atypical lymphocytosis. Continued administration of phenytoin or later rechallenge may increase the severity of the reaction.

Delayed type hypersensitivity in atopic dermatitis.

Three different aspects of delayed type hypersensitivity in atopic dermatitis (A.D.) were studied. (a) Intradermal testing demonstrated that positive reactions to bacterial vaccines were distinctly lower in patients with A.D. (b) Patch testing in patients with A.D. compared to tests in patients with anal eczema showed a striking difference in results concerning the substances to which positive reactions were found. (c) Patients with A.D. (n = 18), atopics without A.D. (n = 10), patients with contact dermatitis (n = 10), and normal controls (n = 10), were patch tested with various human dander …

Acute hypersensitivity to ingested processed pollen.

Ingestion of commercially processed honeybee-collected pollen produced potentially fatal consequences in a 19 year old asthmatic male. Symptoms of sore throat, facial itch and swelling, difficulty in breathing and stridor lasted for approximately two hours and was followed by clinical respiratory distress with widespread wheeze on auscultation of his chest. RAST and skin test data suggest that these complications appear to be mediated by IgE antibodies directed against the processed pollen, but not bee venom. Consumption of processed pollen by atopic individuals is, therefore, a potentially ha
