Travoprost is a prostaglandin analog used in the treatment of open-angle glaucoma. This drug is safe and efficacious and has a low incidence of systemic and local side effects. Common local side effects are conjunctival hyperemia, iris pigmentation, and hypertrichosis of the eyelashes. The authors present a case of a patient who developed marked hypertrichosis of the cheek vellus 3 months after starting treatment with travoprost.

Congenital generalized hypertrichosis (CGH) is a rare, fully penetrant X-linked dominant trait previously described in a single, multigenerational Mexican family. CGH is a visually striking phenotype characterized by excessive facial and upper torso hair in males and by less severe asymmetric hairiness in females. Hypothyroidism is not commonly associated with acanthosis nigricans (AN). We examined a 13-year-old girl with AN, hypertrichosis, and Hashimoto's thyroiditis. Overt biochemical hypothyroidism, thyroid enlargement, and positive titers of antimicrosomal and antithyroglobulin antibodies confirmed Hashimoto's thyroiditis. A family with a distinct form of congenital generalized hypertrichosis was studied. Males were more severely affected than females, who exhibited asymmetric hair distribution. This finding was attributed to lyonization, since genealogical studies indicated an X-linked pattern of inheritance. A back mutation is postulated as the origin of this new phenotype.

INTRODUCTION: Tacrolimus is widely used in treating vitiligo. Drug-induced hypertrichosis is a well-known side effect of cyclosporine. It is less commonly associated with tacrolimus or with topical calcineurin inhibitors.
