A two-stage approach to the treatment of hyperuricemia in gout: the "dirty dish" hypothesis.

OBJECTIVE: It is commonly accepted that the target serum urate level in patients receiving urate-lowering therapy for dissolution of urate crystals in hyperuricemia of gout is <6 mg/dl, and that patients with gout should continue urate-lowering therapy for the rest of their lives. This study was undertaken to reevaluate whether this stringent therapeutic target to dissolve crystals must be maintained lifelong to prevent new crystal formation. METHODS: In a prospective cohort of 211 patients with gout, urate-lowering therapy was withdrawn after 5 years if no tophus was present at baseline, or …

Gout. Hyperuricemia and cardiovascular disease: how strong is the evidence for a causal link?

An association between high levels of serum urate and cardiovascular disease has been proposed for many decades. However, it was only recently that compelling basic science data, small clinical trials, and epidemiological studies have provided support to the idea of a true causal effect. In this review we present recently published data that study the association between hyperuricemia and selected cardiovascular diseases, with a final conclusion about the possibility of this association being causal.

Rasburicase represents a new tool for hyperuricemia in tumor lysis syndrome and in gout.

Hyperuricemia is a feature of several pathologies and requires an appropriate and often early treatment, owing to the severe consequences that it may cause. A rapid and massive raise of uric acid, during tumor lysis syndrome (TLS), and also a lower and chronic hyperuricemia, as in gout, mainly damage the kidney. To prevent or treat these consequences, a new therapeutic option is represented by rasburicase, a recombinant form of an enzyme, urate oxidase. This enzyme converts hypoxanthine and xanthine into allantoin, a more soluble molecule, easily cleared by kidney. The several types of urate o…

Recombinant urate oxidase for the prophylaxis or treatment of hyperuricemia in patients With leukemia or lymphoma.

PURPOSE: To improve the control of hyperuricemia in patients with leukemia or lymphoma, we tested a newly developed uricolytic agent, recombinant urate oxidase (SR29142; Rasburicase; Sanofi-Synthelabo, Inc, Paris, France), which catalyzes the oxidation of uric acid to allantoin, a highly water-soluble metabolite readily excreted by the kidneys. PATIENTS AND METHODS: We administered Rasburicase intravenously, at 0.15 or 0.20 mg/kg, for 5 to 7 consecutive days to 131 children, adolescents, and young adults with newly diagnosed leukemia or lymphoma, who either presented with abnormally high plas…

[The prevalence of gout and hyperuricemia among the workers of a meat-packing plant complex].

An attempt was made to study the incidence of gout and hyperuricemia among the workers of the Moscow meat-packing plant exposed to high food purine loads. Using extensive clinical material (535 men and 1121 women) the authors established that gout was diagnosed 33 times more often in men of this plant than in the muscular population of other towns of the country. Moderate hyperuricemia was revealed in every second of the examinees. Marked hyperuricemia was noted 1.8 times more often, it being 3.4 times more often in men working at this plant as compared to the whole muscular population of the …

[Evaluation of hyperuricemia caused by fructose in a status of altered uric acid metabolism].

It's reported that oral administration of fructose in normal and gouty patients causes a rise in serum uric acid (see "References": Stirpe et al.). The changes in serum uric acid after administration of fructose to gouty patients and to children of gouty patients were significantly different from the changes observed in normal subjects, both for the extent and for the duration of hyperuricaemia. In this study the authors verify if the fructose-induced hyperuricaemia can help to identify gouty patients from people with different metabolic alteration of serum uric acid levels (i.e. chronic leuka

Single-atom nanozyme-based wireless microfluidic sensing platform for noninvasive hyperuricemia diagnosis.

Hyperuricemia is characterized by abnormally elevated serum uric acid (UA) levels. Persistent hyperuricemia not only triggers gout but also causes damage to the kidneys and cardiovascular system. Therefore, utilizing non-invasive diagnostic methods for early and continuous monitoring is crucial for preventing disease progression. Herein, we reported a portable wireless electrochemical sensing platform constructed from SA-Pt/rGO/MOF catalysts prepared via photochemical deposition of platinum single atoms (Pt SAs) on reduced graphene oxide/metal-organic framework (rGO/MOF) composites. The design…

Drug-induced hyperuricemia: multi-pathway regulation, causative drugs, and individualized management strategies.

Hyperuricemia, a prevalent metabolic disorder, is not only the primary cause of gout but also an independent risk factor for various chronic conditions, including hypertension, cardiovascular and cerebrovascular diseases, and diabetes mellitus, thereby posing a significant threat to multi-organ health. Iatrogenic factors represent a key pathogenic trigger for hyperuricemia. With the expanding spectrum of clinical medications and the widespread adoption of polypharmacy, drug-induced hyperuricemia (DIH) now affects up to 25% of hospitalized patients and over 80% of transplant recipients on cyclo…

Oxyberberine alleviates fructose-induced hyperuricemia by modulating purine metabolism and gut microbiota.

Phellodendri Chinensis Cortex, a time-honored Chinese materia medica, has long been applied in the management of gout and other diseases. Hyperuricemia (HUA) is the vital contributor for the occurrence and progression of gout. Oxyberberine (OBB), an oxidized protoberberine alkaloid, is naturally present in Phellodendri Chinensis Cortex. The work made a pioneering endeavor to unravel the potential anti-HUA effect and mechanism of OBB. Hight fructose-induced experimental HUA in rats was employed. Results indicated that OBB significantly reduced the body weight and UA level, and concurrently miti…

Fucoidan Supplementation Relieved Kidney Injury and Modulated Intestinal Homeostasis in Hyperuricemia Mice.

Hyperuricemia is a metabolic disease characterized by an excessively increased level of uric acid (UA) in the blood, with an increasing prevalence and often associated with kidney damage. Gut microbiota and endotoxins of gut origin are key mediators in the gut-kidney axis that can cause renal impairment. The study was to reveal the protective effects of fucoidan on renal injury caused by hyperuricemia. The hyperuricemia model was established in C57BL/6J mice. After 10 weeks of fucoidan supplementation, we found that the levels of serum UA and creatinine were reduced, and the levels of renal tu…
